Laser technology represents a major advance in the treatment of vascular changes. Excellent knowledge combined with technological advances in new laser sources form the basis of this safe and effective option in many acquired or congenital vascular changes on the face. With regard to the different laser sources, the pulsed dye laser (PDL) has the best efficacy-to-safety ratio. This type of laser remains the gold standard for vascular lesions. Neodymium-doped yttrium aluminium garnet (Nd: YAG) lasers and diode lasers, among others, as well as intensed pulsed light (IPL), play an important role in addressing vascular changes in the face. Good knowledge of the laser approach to vascular lesions and technological progress have led to safety features that seek to minimize side effects. These advances mainly include the introduction of cooled tips, cooling devices and cooling systems, changes in pulse duration, and the introduction of different wavelengths, among others. However, undesirable side effects can still occur. Although these are mostly discrete and resolve spontaneously, some complications can be serious and difficult to manage. Therefore, the prevention and early management of complications are all the more important in the treatment of vascular changes on the face.

Systemic lupus erythematosus (SLE) is a highly variable disease driven by the tendency to form very different types of antibody. This sometimes leads to an exaggerated respect for the complexity of the disease symptoms and to uncertainty in dealing with affected patients; however, nowadays the most important measures in the diagnosis and management of the disease can be broken down to a manageable extent. In this respect, the new recommendations of the European League Against Rheumatism (EULAR) on SLE and the recommendations on lupus nephritis jointly developed by EULAR and the European Renal Association/European Dialysis and Transplant Association (ERA/EDTA) are helpful, as are the new classification criteria from EULAR and the American College of Rheumatology (ACR). In this article the core points of these publications are summarized and contemporary SLE management is presented. As a rule, SLE can be effectively treated when managed in this way.

BACKGROUND: The importance of the skin microbiome in the pathogenesis of atopic dermatitis (AD) is gaining increasing attention in current research and offers opportunities for new innovative treatment options. OBJECTIVES: This study investigated the efficacy of a probiotic bath additive on clinical symptoms and skin microbiome of patients with AD. MATERIALS AND METHODS: The study was randomized and double-blind: 22 patients applied a 10-min partial bath with 4.5 × 10 or 9 × 10 colony-forming units (CFU) of viable lactic acid bacteria per liter daily over a period of 14 days. The clinical symptoms were documented using the SCORing Atopic Dermatitis (SCORAD) index and a patient questionnaire at day 0, day 7 and day 14. In addition, skin swabs were taken for subsequent nucleic acid extraction for quantitative Staphylococcus aureus polymerase chain reaction and microbiome analysis using amplicon sequencing. RESULTS: Comparable efficacy was documented in both treatment groups: Probiotic baths with a concentration of 4.5-9 × 10 CFU/liter led to a significant reduction in SCORAD scores (start of study: 63.04) and local SCORAD (14.68) on day 7 (SCORAD 47.09, local SCORAD 10.99) and day 14 (SCORAD 35.26, local SCORAD 8.54). Furthermore, the patient-assessed parameters skin dryness and itching improved significantly over the treatment period. At the same time, the mean gene copy number of S. aureus decreased by about 83% and microbiome analyses showed an increase in the richness of the bacterial community. CONCLUSIONS: Topical application of a probiotic bath represents a promising supportive treatment option for AD that alleviates existing dysbiosis.

In contrast to the obligatory paraneoplasias, which are usually rare, diseases such as Sweet's syndrome, thrombophlebitis or herpes zoster are more common. But they are less frequently associated with neoplasia. The risk of overlooking an associated neoplasm is therefore greater. In this article, prototypic diseases are presented with their clinical appearance, possible pathogenesis and treatment options; this is accompanied by raising awareness of potential associations with cancer. The emphasis on the distinct features of the neoplasm-associated forms should enable more reliable detection of these variants and hopefully contribute to an earlier diagnosis of associated neoplasms. These peculiarities include the jumping or wandering thrombophlebitis occurring as Trousseau's syndrome, recurrent, severe courses with involvement of the oral mucosa in Sweet's syndrome or the necrotizing, gangrenous clinical course, often with a multisegment distribution pattern of herpes zoster. Studies on the association of facultative cutaneous paraneoplasias with certain tumors are presented. However, no general recommendation for tumor screening in patients with herpes zoster, Sweet's syndrome or thrombophlebitis can be given. In atypical courses, particularly severe manifestations or the absence of other causes, more extensive diagnostic procedures appear appropriate in order not to miss possibly associated neoplasms.

As part of the reappraisal of the legacy of Wismut AG, 12 patients with silica-induced scleroderma among underground uranium ore mine workers (Wismut AG) under long-term exposure to silica fine dust, as well as radon and its daughter products, during the 1960s and 1970s are reported on. Silica-induced scleroderma is clinically, serologically and immunologically indistinguishable from idiopathic systemic sclerosis. In experimental studies, endothelial cells, monocytes and fibroblasts, as well as their synthesis rates and the release of cytokines and chemokines, were activated by silica fine dust in a way that is consistent with the pathophysiological processes in idiopathic systemic sclerosis. It was not possible to achieve recognition of silica-induced systemic sclerosis as an occupational disease in Germany.

This is confirmed by data from insurance companies as well as increased search queries on Google. There is a controversial discussion in the scientific literature whether the mite has become resistant to standard therapy with permethrin. One case report and a group of cases (N = 12) from a mother-child facility are described in the following demonstrating decreased effectiveness of permethrin therapy. Dermatoscopy can be helpful in diagnosis and in assessing effectiveness of therapy. Dermatoscopic criteria are shown and therapeutic concepts are critically discussed.

Generalized pustular psoriasis is a rare, often relapsing and usually severe inflammatory skin disease with concomitant systemic symptoms in many cases. The disease is in part due to mutations in the gene of the interleukin-36 receptor antagonist (IL36RN) explaining the autoinflammatory nature of this condition. Here, we report on a 39-year-old pregnant woman who stopped her medication with secukinumab due to her pregnancy. Subsequently, she suffered from severe exacerbation of her disease and was admitted to our hospital. The patient had a known mutation in the IL36RN (Ser113Leu) gene. At the 33rd week of her pregnancy therapy with certolizumab was initiated and resulted in a rapid remission within a few weeks. After the patient gave birth, remission could be maintained under the given therapy with this biologic.

Clinical practice guidelines are systematically developed decision aids for specific medical conditions. In Germany, national dermatology guidelines are developed chiefly under the aegis of the German Dermatological Society in collaboration with the Professional Association of German Dermatologists. European and international dermatological guidelines also exist and are developed by a range of organisations, such as the European Centre for Guidelines Development, which was founded by the European Dermatology Forum in 2018. In the years 2019 and 2020, new or updated German national guidelines were published on topics such as pathological scars (hypertrophic scars and keloids), cutaneous lupus erythematosus, pyoderma grangrenosum, anal pruritus, anal eczema, anal canal and anal rim carcinomas, as well as the prevention of HPV-associated neoplasms through vaccination, syphilis and the systemic treatment of neurodermitis. A new European guideline on lichen planus closes a gap in the spectrum of guidelines available in Germany. Key recommendations and relevant changes in the guidelines are presented in this article.

Hereditary tumor syndromes are characterized by a familial occurrence of tumors/cancer. A hereditary tumor syndrome should be suspected if a familial occurrence of cancer is seen and/or persons at younger age are affected. Some of the currently known tumor syndromes are associated with specific skin symptoms that can aid the physician in establishing the correct diagnosis. Examples are fibrofolliculoma in Birt-Hogg-Dubé syndrome, epidermal cysts, sebaceous cysts, neurofibroma in Gardner syndrome and sebaceous neoplasms or keratoacanthoma in Muir-Torre syndrome. If a genetic tumor syndrome is suspected, genetic testing and counselling should be performed in the index patient and is also recommended for family members. Affected patients should be offered regular clinical surveillance by the appropriate medical disciplines. Since curative therapy does not exist so far, preventive screening is of great importance.

Skin is commonly affected by neuroendorine paraneoplastic syndromes (PNS). This is due to the expression of receptors in the skin by which abnormally secreted neuroendocrine hormones and mediators elicit directly, and indirectly, cutaneous key signs and thus facilitate early diagnosis of these diseases. In acromegaly, induction of the growth hormone-insulin-like growth factor‑1 axis results in trophic changes of the acral portions of the skin and mucosal membranes including cutis verticis gyrata. The skin signs of non-iatrogenic Cushing syndrome are identical with those of exogenous prolonged intake of glucocorticoids: centripetal accumulation of adipose tissue, plethora and striae distensae. Episodic flushing of the face and trunk (together with explosive diarrhea) is a key feature of carcinoid tumors. Fibrotic remodeling of the heart and retroperitoneal space, and less commonly of the skin, are important complications mediated by abnormally secreted 5‑hydroxytryptamine (serotonin, 5‑HT), the latter eliciting profibrotic responses on HT-receptor-expressing fibroblasts. Androgen-secreting tumors lead to well-established receptor-mediated cutaneous signs of peripheral hyperandrogenisms: seborrhea, acne, hirsutism, and androgenetic alopecia. In contrast, the pathogenesis of necrolytic migratory erythema as a key feature of glucagonoma remains incompletely understood and is thought to be related to hypoaminoacidemia. This review summarizes the clinical features of neuroendocrine PNS with skin involvement, elucidates its underlying pathophysiology, lists differential diagnoses, and explains key diagnostic steps and principal therapeutic options. An interdisciplinary approach is essential to provide the best care of all patients with neuroendocrine PNS.

Hyaluronic acid (hyaluron; HA) is an important constituent of the extracellular matrix and has an age-dependent decrease in tissue concentration. While natural HA is rapidly degraded, filler cross-linked HA achieve a half-life of 6-12 months. Such fillers enjoy increasing popularity in esthetic medicine and are versatile tools. Although HA fillers can activate fibroblasts it is of minor importance for long-term results. Fillers are injected into subcutaneous adipose tissue. Their interaction with somatic stem cells and mature adipocytes has gained increasing interest. Cross-linked HA can indeed support proliferation of pre-adipocytes, stimulate their differentiation into mature adipocytes and protect the mature cells from senescence, apoptosis and lipolysis. This view offers a new perspective on the development of future fillers for esthetic medicine.

BACKGROUND: Hyperplasia-associated cutaneous paraneoplasia is an important differential diagnosis in everyday clinical practice. An early diagnosis of the underlying tumor disease can significantly improve the patient's prognosis. PATHOGENESIS: Hyperplasia is probably mainly cytokine-mediated. The primary tumor and/or the metastases release growth factors and transcription factors which, via epidermal growth factors, lead to hyperproliferation of keratinocytes. ACANTHOSIS NIGRICANS MALIGNA: Symmetrical mainly intertriginous hyperpigmentation with partially verrucous hyperplasia and lichenification mostly in association with gastric adenocarcinoma. Special forms are florid cutaneous papillomatosis and tripe palms. Pseudoacanthosis nigricans is to be distinguished (metabolic and hormonal disorders). LESER-TRéLAT SYNDROME: Eruptive occurrence of seborrheic keratosis associated with visceral tumors. ACROKERATOSIS BAZEX: Erythema and scaling initially at the bridge of the nose, ear helix and acra with later spread, associated with tumors of the upper aerodigestive system. It should be clinically differentiated from psoriasis. THERAPY: The treatment of the primary tumor is decisive, which also leads to a decrease of cutaneous symptoms. Reappreance suggests tumor recurrence.
