Erdafitinib is a fibroblast growth factor receptor 1-4 inhibitor used for locally advanced or metastatic urothelial carcinoma in patients who have experienced disease progression after at least 1 prior systemic therapy.... Erdafitinib is a fibroblast growth factor receptor 1-4 inhibitor used for locally advanced or metastatic urothelial carcinoma in patients who have experienced disease progression after at least 1 prior systemic therapy. Various cutaneous side effects of erdafitinib have been reported, most commonly stomatitis and nail changes such as onycholysis, nail dystrophy, and paronychia. However, onychomadesis is a rarer nail toxicity that can cause significant discomfort and may necessitate treatment interruption. We present the case of a 61-year-old White man with metastatic urothelial carcinoma treated with 8 mg of erdafitinib daily who developed diffuse onychomadesis within a few months of initiating therapy. Awareness of the potential cutaneous side effects of erdafitinib and timely referral for specialty dermatologic care are important to allow treatment continuation.

Necrobiotic xanthogranuloma (NXG) is a rare, chronic non-Langerhans cell histiocytosis that clinically manifests as yellow to orange papules or nodules, most commonly involving the periorbital region. Histopathologic features include necrobiotic collagen, multinucleated giant cells (including Touton and Langhans types), granulomatous inflammation with xanthomatous histiocytes, extracellular lipid, and occasionally lymphoid follicles and plasma cells. NXG has a well-documented association with monoclonal gammopathies and extracutaneous malignancies, although the underlying pathogenic mechanisms remain poorly understood. We report the case of a 72-year-old woman presenting with erythematous plaques on the abdomen and extremities. Despite the absence of classic facial involvement and an initial nondiagnostic skin biopsy, further evaluation revealed a clonal plasma cell disorder classified as borderline between monoclonal gammopathy of undetermined significance and smoldering multiple myeloma. A subsequent biopsy demonstrated diagnostic features of NXG. This case underscores the importance of integrating clinical findings with histopathologic evaluation to achieve an accurate diagnosis.

Pancreatic panniculitis is a rare cutaneous manifestation affecting patients with pancreatic disease. This eruption is associated with acute pancreatitis, pancreatic malignancies, pancreatic pseudocysts, post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis, and pancreatic trauma. Increased efflux of pancreatic enzymes from the diseased pancreas appears to cause extra-abdominal fat necrosis and may manifest in the skin. We report a 59-year-old woman with cutaneous pancreatic panniculitis presenting after ERCP with elevated amylase levels but no symptoms of pancreatitis.

Subepidermal calcified nodule, a rare variant of calcinosis cutis, is a benign lesion typically affecting the eyelid in children. We report a 14-year-old boy presenting with bilateral nodules on the upper eyelids. Clinical examination revealed a papule on the right eyelid and a plaque measuring 0.5 × 1 cm on the left, both pearly white, firm, and non-tender. Dermoscopy showed whitish, flocculent areas with linear vessels. Excisional biopsy confirmed dermal calcium deposits with surrounding collagenous fibrosis, establishing the diagnosis. Serum calcium, phosphorus, and parathyroid hormone levels were normal. No recurrence was observed at 6 months. This case highlights the unusual bilateral and symmetrical presentation of eyelid SCNs and the role of dermoscopy in supporting the diagnosis.

Cystic cutaneous lymphatic malformations are developmental anomalies of the lymphatic system characterized by lymphatic vessel dilatation. Microcystic forms, also known as circumscribed lymphangiomas, present as clusters of translucent or hemorrhagic vesicles, papules, or infiltrated plaques, often appearing in early childhood and primarily affecting the proximal extremities. Although generally asymptomatic, they may cause inflammatory, infectious, or hemorrhagic episodes. We report a 39-year-old woman with a 2-year history of vesicular lesions on the medial surface of both labia majora associated with intermittent pruritus and fluid leakage. Clinical examination revealed bilateral painless swellings, each approximately 6 cm, with translucent and hemorrhagic vesicles. Histology confirmed bilateral subcutaneous microcystic lymphangiomas. Given their benign nature, no treatment was recommended aside from surveillance. Microcystic lymphangiomas have an unpredictable course, with some remaining stable and others developing complications requiring treatment. Management depends on lesion location, symptoms, and esthetic impact, with options including sclerotherapy, CO₂ laser, or surgical excision, if necessary. This case highlights the importance of accurate diagnosis and individualized treatment of microcystic lymphangiomas.
