EFFECTS OF A SINGLE NIGHT SHIFT ON HEALTHCARE PROFESSIONAL WORK ABILITY - A COHORT STUDY.

These effects could increase with age, especially due to workforce aging. The aim of the study was to examine the effect of a single night shift on the Work Ability Index (WAI) scores of hospital healthcare workers. A prospective survey was conducted on a convenience sample of 49 on-call residents working 12- or 16-hour night shifts and 47 nurses working 12-hour night shifts in the emergency department of an urban teaching hospital. The study included analysis of sociodemographic, occupational and medical data collected through a questionnaire during a morning shift change. The participant work capacity was assessed using the short form of the WAI Questionnaire. The mean WAI score for nurses was 40.98 before a night shift and 37.15 after a night shift, which was a statistically significant decrease (p<0.01, 95% CI: 2.39-5.27). Similarly, among residents, the mean WAI score decreased from 43.02 before a night shift to 38.76 after a night shift (p<0.0001, 95% CI: 2.95-5.58). There was a statistically significant difference in WAI scores between nurses and residents (p<0.05). Our results showed that even a single night shift negatively affected the work ability of emergency healthcare workers. We recommend scheduling shift work with fewer night shifts to preserve healthcare worker work ability.

This research was conducted as a randomized study with the objective of determining the effects of breast milk, dry-keeping, and chlorhexidine methods used in umbilical cord care on the time of umbilical cord separation and on omphalitis and bacterial flora development. The study was carried out in 97 newborns divided into three groups, as follows: breast milk was used for umbilical cord care in group 1 of 32 newborns, dry-keeping was applied in group 2 of 35 newborns, and chlorhexidine was applied in group 3 of 30 newborns. Infants were assigned to three subgroups through randomization performed using a computer program. Swabs for culture were obtained from umbilical cord on postnatal day 2 and day 5. While difference in the results of the care methods applied was not significant on day 2, significant difference in the results was found on day 5. It was determined that there was no bacterial growth in 46.7% of the chlorhexidine group, while maximum bacterial growth was recorded in the dry-keeping group. In the chlorhexidine group, timing of umbilical cord separation was significantly shorter than in other groups. Study results indicated that the most effective method was the use of chlorhexidine. Dry-keeping can be risky since it was inefficient in preventing umbilical colonization, and in cases where the cost of chlorhexidine is an issue, the use of breast milk can be recommended.

The most frequent surgical procedure in oral surgery is extraction of mandibular third molars, which could be associated with postoperative complications such as pain, swelling, and trismus. We aimed to investigate the association of salivary urea and antioxidative enzyme activities with the intensity of postoperative pain after wisdom tooth extraction. This cross-sectional prospective study included 66 subjects (median age 25, range 17-47 years). Salivary urea and superoxide dismutase (SOD) were determined by enzymatic colorimetric tests. The questionnaire for study participants included demographic and history data, as well as data on personal experience of pain in grade 0-10 according to the visual analog scale (VAS). Grading 1-3 was considered as low (VAS-1), 4-6 mild (VAS-2) and 7-10 as severe pain (VAS-3). Moderate positive correlation was recorded between urea concentration and SOD activity in saliva (r=0.46). Moderate negative correlations were found between urea salivary concentration and VAS grading value reported after 12 hours (r=0.51). The concentrations of urea significantly differed among the three VAS categories (p<0.001), whereas salivary SOD activities did not differ among the categories. Preoperative salivary urea concentration and SOD activity might be associated with postsurgical pain after surgical removal of mandibular third molars.

This study aimed to compare the renal resistive index (RI) as an early marker of renal damage between normoalbuminuric children with type 1 diabetes mellitus (T1D) and normal age-matched controls. A total of 76 children with T1D (aged 10-18 years) and 76 healthy age-matched controls were enrolled. Anthropometric parameters, blood pressure, glycated hemoglobin (HbA1c), serum creatinine (sCr), estimated glomerular filtration rate (eGFR), serum uric acid (SUA), urine albumin/creatinine ratio (ACR), and degree of pubertal development were determined in all participants. RI was measured using Doppler ultrasound in both groups. The T1D group had lower sCr and SUA values and higher ACR and mean RI values than the control group. Using the expectation-maximization method, 2 clusters of T1D patients were identified. Cluster 2 with higher RI values, eGFR and HDL cholesterol was more prone to albuminuria and could carry worse prognosis. In conclusion, renal RI is increased in the hyperfiltration phase of diabetic nephropathy in children with T1D. Renal RI could be helpful as a marker for diagnosing diabetic nephropathy in the early preclinical course of the disease in normoalbuminuric children with T1D.

Here we report a case of a 5-year-old boy with Ebstein anomaly and supraventricular tachycardia. He was diagnosed with Ebstein anomaly at the age of 3 months during the workup for afebrile convulsions, and has been followed by a pediatric cardiologist since. Electrocardiography recorded a small Rr` pattern in V1 on a few occasions, without other abnormalities. At the age of 2.5 years, the first episode of supraventricular tachycardia was recorded and stopped with adenosine. He was started on atenolol and was asymptomatic until a year later when he presented with the second recorded supraventricular tachycardia. He was referred to our institution for an electrophysiology study with ablation. Tachycardia was entrained and all the maneuvers were in accordance with atrioventricular reentry tachycardia. Tricuspid annulus was mapped during tachycardia. The earliest atrial signals appeared in the posteroseptal region of the valve, mechanical blocking of the tachycardia was recorded in that region, and early fractionated signal was present during ventricular retrograde pacing. The pathway was successfully ablated. Basal electrocardiogram was without change after ablation and the patient did not experience recurrence of tachycardia during follow-up. This case shows the efficacy and importance of ablation in Ebstein anomaly, without x-ray or intracardiac echocardiography.

The onset of multiple sclerosis (MS) in childhood is a diagnostic challenge. The aim of our study was to analyze the parameters of MS with an early onset in relation to the symptoms and course of the disease in adult patients. This study included patients treated with signs and symptoms of initial central nrvous system demyelinating disease, which were divided into a group of 52 patiensts who developed the disease before the age of 18 (children) and another group aged 20-55 years (adults). Children rarely had polysymptomatic onset (p=0.020). The most common initial symptoms in children were optic neuritis (35.3%) and ataxia (35.3%). In the group of adult patients, sensory disturbances (41.6%) were the most common initial symptoms, followed by pyramidal symptoms (37.6%). Monitoring the time of second relapse revealed it to have occurred in 40 (78.4%) pediatric patients and 79 (78.2%) adult patients after a median of 12 months and 9 months, respectively. In conclusion, the initial manifestations of MS in children do not differ much from MS in adults according to the disease characteristics.

Microdiscectomy is one of the surgical methods for the treatment of herniated intervertebral disc in patients with low back pain. The aim of the research was to evaluate the presence of psychological and cognitive-behavioral factors, including anxiety, fears and fear-avoidance beliefs of physical activity and work and their correlation with the pain and functional disability in patients after lumbar microdiscectomy and subsequent physical therapy. The research was performed on 198 patients (95 men and 103 women), mean age 50.20±10.26 years. The following questionnaires were used in the study: Spielberger Anxiety Inventory-State and Trait; Fear-Avoidance Beliefs Questionnaire (Physical activity and Work); for intensity of pain, visual analog scale and Oswestry Low Back Pain Disability Questionnaire. These assessments were carried out after microdiscectomy, as follows: just before rehabilitation treatment, and 1, 3 and 6 months after microdiscectomy. The pain and functional disability had significant correlations with the following factors: anxiety-state (p<0.01), anxiety-trait (p<0.01), fear/avoidance beliefs - physical activity (p<0.01) and fear/avoidance beliefs - work (p<0.01). The pain and functional disability in patients after lumbar microdiscectomy showed significant correlation with anxiety, fears and fear-avoidance beliefs. The mentioned psychological and cognitive-behavioral factors can predict the degree of functional recovery and indicate additional therapy after lumbar microdiscectomy.

The aim was to assess the frequency and success rate of prenatal diagnosis of congenital heart diseases (CHDs) in live-born neonates using ultrasound, as well as the occurrence of different types of CHD in the tertiary m... The aim was to assess the frequency and success rate of prenatal diagnosis of congenital heart diseases (CHDs) in live-born neonates using ultrasound, as well as the occurrence of different types of CHD in the tertiary medical care institution in the period from January 1, 2016 till December 31, 2020. In this retrospective study, data were collected by reviewing medical history of both mothers and newborns. During the study period, there were 13,932 childbirths, and 42 newborns were diagnosed with CHD. Excluding the diagnoses of persistent duct of Botallo (n=4) and CHDs arising as a consequence of maternal condition (n=2), out of 36 newborns, 31 (86%) were diagnosed prenatally and 5 (14%) postnatally. The prevalence of CHDs was 30 10,000 births (3.0‰). In conclusion, the rate of prenatal diagnosis of CHDs was very high. The prevalence of CHDs was substantially lower than the European average published in the EUROCAT study for the 2016-2019 period. Additional education and examination algorithms need to be implemented to achieve more successful rates of prenatally diagnosed CHDs recorded in the best centers across Europe and the world.

The aim was to investigate patient attitudes regarding psychiatrist attire, sociodemographic characteristics, and mutual addressing. A total of 340 both inpatients and outpatients at a psychiatric hospital in Croatia were assessed about their preferences regarding psychiatrist wearing a name badge, gender, age, ways of mutual addressing, and attire. The majority of patients, inpatients significantly more often, thought that a psychiatrist should wear a name badge. Female patients preferred female psychiatrist more than male and . Inpatients also preferred female psychiatrist. Patients with affective and anxiety disorder were more likely to choose female psychiatrists, patients with addiction more often had no preferences, and patients with psychotic disorders preferred male psychiatrists. Younger patients preferred younger psychiatrists and inpatients preferred psychiatrists in the 35-50 age group. Patients with anxiety disorder more often opted for psychiatrists older than 50. Patients with anxiety disorders would rather be addressed by their first name, whereas patients with psychotic disorders more often preferred to be addressed by their title and surname. Patient attitude about how they should address the psychiatrist did not differ according to any of the variables assessed. Most patients thought that white coat was most appropriate attire for psychiatrist and psychiatrists in white coat were considered most competent, empathic, accessible, and friendly in comparison with psychiatrists wearing suits, smart attire, or casual clothing. Other than preference for white coat, patient preferences significantly differed according to their sociodemographic characteristics and diagnosis. Psychiatrists should bear that in mind to establish better rapport in doctor-patient relationship.

Leukotriene receptor antagonists are a class of drugs used in maintenance therapy of asthma. Among the adverse effects induced by them, particularly by montelukast, is eosinophilic granulomatosis with polyangiitis (EGPA), which manifests as a form of vasculitis co-occurring with severe asthma and eosinophilia. Our aim was to point to the importance of this particular diagnosis by analyzing the published cases of montelukast induced EGPA and to analyze them in order to identify if this form of EGPA presented some specific characteristics. Leukotriene receptor antagonist induced EGPA is a frequent adverse event compared to EGPA induced by other drugs, and specialists should be aware of it. We observed that more than half of patients were female. Montelukast induced EGPA occurred after a variable period of time. The most common clinical manifestation was neuropathy and most of the patients had a good outcome after immunosuppressant treatment. The present diagnosis should be considered by physicians when patients are prescribed montelukast followed by worsening of symptoms.

Since the introduction of 3D mapping systems, the use of x-rays in electrophysiology procedures has significantly decreased. The goal of this study was to analyze the frequency of x-ray use in pediatric electrophysiology in relation to age or body mass, indication for the procedure, and arrhythmia substrate. This retrospective, cross-sectional study included all pediatric procedures in two centers, Gottsegen Cardiovascular Center, Budapest, Hungary and Sestre milosrdnice University Hospital Center, Zagreb, Croatia. Patient data included weight, age, sex, presence of structural heart disease, indication for the electrophysiology study, arrhythmia substrate, and x-ray use. A total of 914 participants were analyzed, mean age 13.25 years and mean mass 53.20 kg. Of all procedures, 62.04% were performed without and 37.96% with fluoroscopy. There were no statistically significant differences in the frequency of x-ray use when comparing the groups by sex (p=0.181), age (p=0.380) or mass (p=0.120). Left-sided accessory pathways required x-ray use most frequently, whereas unknown substrates and of the known substrates atrioventricular nodal reentry tachycardia required it least frequently (p<0.001). Younger or smaller children were not irradiated more frequently during electrophysiology procedures performed with 3D mapping systems. In conclusion, there is no reason to postpone the procedure based on radiation frequency use.

Congenital muscular torticollis (CMT) is one of the most common congenital musculoskeletal deformities, and is defined as tilted head position with hypertonic sternocleidomastoid muscle and limited cervical spine mobility. Incidences up to 16% have been recorded, more often in male children. Clinical features are characterized by an inclined position of the head on the diseased side and a gaze directed towards the healthy side, deformities such as plagiocephaly, and various complications. In order to prevent this, it is recommended to screen all infants up to four months of age. Diagnosis is usually made through history and physical examination. CMT treatment is carried out conservatively, and the habilitation program is created individually, depending on the degree of CMT. The habilitation program includes neck stretching exercises, neck muscle strengthening exercises, motor activities that encourage a symmetrical movement pattern, adaptation to the environment, and education and support of parents or guardians to ensure a daily, intensive home program. Early treatment is one of the main prognostic factors on which the outcome and duration of treatment depend. As this is the only prognostic factor that we can act on, our goal is to speed up the diagnosis and therapy implementation until the cure.

This study aimed to evaluate prognostic values of the serum lipid panel data for development of macrovascular complications (MVC) in patients with type 2 diabetes mellitus (T2DM) alone and those with comorbid hypothyroidism (HT), diffuse non-toxic goiter (DNTG), or a combination of these disorders. The study included 596 inpatients. Receiver operating characteristic (ROC) analysis was used to identify prognostically significant values of total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), non-HDL-cholesterol (non-HDL-C) and remnant cholesterol (RC). The following cut-off points that determine the relative risk of MVC development were established: TC >5.11 mmol/L, TG ≥2.03 mmol/L, LDL-C ≥2.97 mmol/L and non-HDL-C ≥4.29 mmol/L in T2DM patients with comorbid HT; TC ≥4.97 mmol/L, TG ≥2.54 mmol/L, LDL-C ≥3.21 mmol/L and non-HDL-C ≥4.20 mmol/L in T2DM patients with comorbid DNTG; and TC ≥4.89 mmol/L, TG ≥1.56 mmol/L, LDL-C ≥2.93 mmol/L, non-HDL-C ≥4.04 mmol/L and RC ≥1.14 mmol/L in those with comorbid HT and DNTG. Thus, serum levels of TC, TG, LDL-C, non-HDL-C and RC can be used for stratification of T2DM patients with comorbid thyroid dysfunction into the category of increased risk of MVC development.

The specificities of student population may lead to an increased risk of adverse mental health outcomes during the COVID-19 pandemic. The aim was to evaluate the levels of perceived stress, anxiety, and depression in medical students during COVID-19 lockdown, and the relationship of these constructs with various sociodemographic variables. The study was conducted online from April 20 to April 29, 2020, during the first COVID-19 pandemic lockdown (using sociodemographic questionnaire and DASS-21 scale), on a sample of 420 students (female 81.7%, M=22.53) at the University of Kragujevac. The majority of participants (55%-58%) had no self-reported symptoms of depression, anxiety or stress, and the mean values were in-between the categories of 'without any symptoms' and 'with mild symptoms'. Predictors of exhibiting depression symptoms were previous psychological problems (odds ratio [OR]=3.16), family history of mental illness (OR=2.12), and relationship status (OR=0.61). For the symptoms of anxiety, predictors were female gender (OR=2.38), reported previous psychological problems (OR=3.91), and age (OR=0.92). Predictors of the symptoms of stress were female gender (OR=2.31), previous psychological problems (OR=3.18), and age (OR=0.91). The results of our study imply that certain factors influence the development of adverse mental health outcomes during this period, which should be taken into account when planning psychological interventions and services.

Regular physical activity and maintaining fitness reduce the risk of chronic cardiovascular and metabolic diseases, reduce the risk of exacerbation of existing diseases, maintain physical and mental health, and contribute to healthy and successful aging in the elderly. It is of particular importance during the prolonged COVID-19 pandemic. The aim of this review is to present relevant knowledge about the effects of restrictions and social distancing established with the aim of suppressing the COVID-19 pandemic, on physical activity and mental health of elderly people. During the pandemic, numerous authors have reported a decrease in the share of physically active population and those that used to exercise regularly. The time spent in activity decreases, energy expenditure decreases, and the time spent sedentary increases although the results in all segments related to physical activity are not consistent. Lockdown measures significantly worsen insomnia, sleep quality, feelings of loneliness, anxiety and depression in older people, especially in women. During the restrictive measures caused by the COVID-19 pandemic, physical activity and exercise significantly contribute to the maintenance and improvement of physical and mental health.

We aimed to explore variations in dietary habits in cancer patients according to clinical and demographic characteristics. We conducted a cross-sectional survey on cancer patients treated with systemic therapy. The survey included 42 items concerning demographic and clinical data, observed dietary modifications and oncologist implication. All patients gave their informed consent to participate in the study. Statistical Package for Social Sciences for Windows software version 20 was used on statistical analysis. The study involved 110 patients (female 71.8% and male 28.2%), mean age 53.4 years. They were treated mostly for breast cancer (58.2%). Patients reduced or stopped taking canned products (70%), milk (59.1%), red meat (80%), processed meat (76.4%), fatty food (88%) and sugar (83.6%). Conversely, preferred food were fish (73.6%), fruits and vegetables (95.5%), and honey (54.5%). Weight loss of 5 kg or more was higher among patients on a sugar restrictive diet (p=0.026). Thirty-five patients discussed diet with their oncologist. Patients received information on diet from the internet and other patients. Our findings suggested that patients were aware of the role of a balanced diet and made many changes. However, most of the patient information was derived from informal sources.

Patients are aiming to relieve pain as fast as possible and return to everyday activities. The aim of the study was to investigate the effect of preoperative physical education on postoperative rehabilitation in patients after total hip arthroplasty (THA). There were 60 patients divided in two equal groups. One group underwent 3-day preoperative education and the other did not. The two groups were compared according to their preoperative and postoperative visual analog scale (VAS), range of motion (ROM), Harris Hip Score (HHS), Western Ontario and McMaster Universities Arthritis Index (WOMAC), and 36-Item Short Form Survey (SF-36). Patient general characteristics were similar in the two groups. Patients that received preoperative training had lower VAS scores, greater ROM, higher HHS and lower WOMAC score on postoperative day 42 (p<0.05 all). These patients had no complications. There were no between-group differences in the SF-36 questionnaire. In conclusion, patients that underwent preoperative rehabilitation protocol before THA had lower pain and disability scores and better functional outcomes.

To the best of our knowledge, no previous study has reported characteristics of C-reactive protein (CRP) levels in COVID-19 patients with comorbidities such as sciatica and lumbar syndrome. The objective of this study was to examine whether CRP levels rose significantly in COVID-19 patients with acute sciatica as compared to COVID-19 patients without it. This prospective study was performed on 147 patients previously diagnosed with COVID-19 infection who were admitted to the COVID hospital. Myalgia (p=0.00008) and lower back pain (p=0.00001) occurred much more frequently in the study group than in the control group. There was a high probability that patients with a previous history of sciatica may experience it again during their stay in COVID-19 hospital and that patients without it would not (p=0.00001). There was a significant correlation between the severity of sciatica and CRP levels (r=0.19, p=0.0174). Our results suggest that serum CRP levels are significantly higher in COVID-19 patients with concomitant sciatica in comparison to patients without sciatic pain, indicating a connection between the increased systemic inflammation in COVID-19 and sciatica. Given that the occurrence of COVID-19 in patients with sciatica is not rare, the knowledge about this matter is of high medical relevance.

Psychotherapy is an independent, interdisciplinary science that encompasses psychotherapeutic directions based on scientific principles and methods. Through the psychotherapeutic process, psychotherapy aims to achieve personal growth, remove or lessen the intensity of existing psychosocial, psychosomatic and behavioral problems and suffering, as well as help develop more functional patterns of behavior and experiences of interpersonal relationships or value systems. Research has shown that the COVID-19 pandemic has had a strong impact on psychotherapy in terms of the way it is carried out, change in the environment in which it is performed, and online psychotherapy has been introduced. The aim of this study was to find information, through available literature, on the impact of the pandemic on mental health and psychotherapy, and also to spread awareness about the current identified risks connected to the greater morbidity of psychiatric disorders and diseases and the need of psychotherapeutic interventions. Previous studies have shown that the pandemic increased the incidence of psychological disorders and diseases and the need of psychological intervention. Available data have shown that in the times of strict epidemiological measures during the COVID-19 pandemic, certain modifications of the psychotherapeutic process were made in terms of the use of online psychotherapy.

Fibrous dysplasia is a benign fibro-osseous lesion which rarely undergoes malignant transformation, frequently after radiotherapy. Osteosarcoma is the most common malignancy arising in fibrous dysplasia. We report a case of 67-year-old female patient with osteosarcoma developing in long lasting fibrous dysplasia without prior radiation. The patient underwent surgery with postoperative chemotherapy. She developed two relapses over the course of four years after the initial therapy. With this case, we want to underline the importance of noticing any significant change in the otherwise unremarkable course in patients with fibrous dysplasia.
