A broad range of temporary, semipermanent, and permanent filler products are on the market. We performed a narrative review on spontaneous and induced degradation of dermal fillers and . Hyaluronic acid-based fillers are the most frequently used temporary fillers. The products differ in their hyaluronic acid content, cross-linking, and rheological parameters. Endogenous hyaluronidase and reactive oxygen species are responsible for the spontaneous degradation of these fillers. Hyaluronidase digests the filler material by cleavage of the -1,4 glycosidic linkage between -acetylglucosamine and -glucuronic acid. The enzyme can be used for treatment of medical and cosmetic adverse events due to hyaluronic acid-based filler. Higher hyaluronidase content and higher degree of cross-linking are major factors contributing to filler persistence over time. Semipermanent fillers are poly-(), -lactic acid and calcium hydroxylapatite. These filler types are decomposed by hydrolysis and osteoclastic enzymes. They usually persist up to 2 years, in single patients even more than 5 years. Sodium thiosulfate can stimulate degradation of calcium hydroxylapatite, but it is slow acting and not effective in case of emergency. Permanent fillers may show some kind of modification , but spontaneous or induced degradation has not been documented. Once implanted the permanent fillers remain lifelong. Intralesional laser treatment supports the removal of permanent filler material as an alternative to surgery. Besides biocompatibility and toxicity, filler materials should also be assessed for degradation to improve patient safety.

However, surgical excision, radiofrequency,...OBJECTIVES: Eyelid margin lesions cause discomfort by impeding both central and peripheral vision, depending on their position on the eyelids, thus hindering everyday routines. However, surgical excision, radiofrequency, electrocautery, and cryosurgery are employed for treatment, although they are complicated and have functional limitations. Carbon dioxide (CO) lasers were used to assess the efficacy, safety, and patient satisfaction with CO laser ablation in managing eyelid margin lesions. MATERIAL AND METHODS: This is a pilot study of patients with eyelid margin lesions presented to a private clinic in urban Gujarat between June 2020 and June 2023. All patients were treated with a 10600 nm CO laser with a power of 1-2 W in continuous mode. The eyelid lesions were classified into three categories based on their size: Small (1-3 mm), medium (4-6 mm), and large (7-10 mm). The clinical response was assessed by the treating physician at week 1 and week 4, and categorized as excellent (complete clearance of lesion), moderate (>50%), or poor (<50%). Patients were asked to rate their level of satisfaction on a three-point scale at the end of the follow-up period: Dissatisfied, somewhat satisfied, or very satisfied. RESULTS: Ten patients of all females with six of them having compound melanocytic nevi, two of them each had cystic hidradenoma, and one each had cyst of zeis and verruca vulgaris. Six of them had small lesions, two had medium, and two had large lesions. The clinical response grading of physicians at both 1 week and 4 weeks was excellent, and the patient satisfaction questionnaire yielded excellent outcomes. CONCLUSION: The ablative CO laser is an excellent device used for the treatment of eyelid margin lesions, particularly when performed by an expert and skilled dermatologist. The application of this treatment approach has proven to be very beneficial and shows excellent results in terms of both the esthetic appearance and structural function of the eyelids.

A total of 44 patie...OBJECTIVES: To study the efficacy and safety of microneedling radiofrequency (MNRF) in the treatment of acne scars. MATERIAL AND METHODS: This is a hospital-based and prospective interventional study. A total of 44 patients (18- 55 years old), skin types III-V, with acne scars, were enrolled. Four sessions of MNRF (Lumenis legend PRO Machine, with non-insulated needle tips GEN6, GEN6L; Israel) were done with an interval of 3 weeks between each session and followed up 1 month after the last session. The outcome was assessed by Goodman and Baron's qualitative and quantitative acne scar grading, physician global assessment (PGA), and patient satisfaction score (PSS). RESULTS: Out of 44 patients, four patients could not complete the study. Goodman and Baron's qualitative assessment showed patients with Grades 2, 3, and 4 were 6 (15%), 17 (42.5%), and 17 (42.5%), respectively. At the end of follow-up, patients with Grades 1, 2, 4, and 4 acne scars were 5 (12.5%), 24 (60%), 10 (25%), and 1 (2.5%), respectively, which were statistically significant ( < 0.05). Seven patients showed no improvement. Goodman and Baron's quantitative assessment showed an initial mean of 12.65 and an after-follow-up mean of 8.3, which was statistically significant < 0.05). PGA and PSS scores showed excellent to good responses of 70% and 82.5%, respectively. Adverse effects such as post-treatment erythema were seen in all patients, and edema was observed in 12 patients. Post-inflammatory hyperpigmentation was noted in two patients. CONCLUSION: MNRF was found to be effective with a better safety profile for acne scars.

MATERIAL AND ME...OBJECTIVES: To evaluate the cosmetic result of nasal of an elliptical excision and primary closure on small nasal defect by using Scar assessment using the Stony Brook Scar Evaluation Scale (SBSES) score. MATERIAL AND METHODS: This retrospective interventional study was conducted in a private clinic over a 2-year duration. Patients aged 18-60 years with nasal defects <1.5 cm were enrolled after obtaining informed consent. Exclusion criteria included immunocompromised status, keloidal tendency, local site infection, bleeding disorder, age <18 years, and lack of consent. Scar assessment using the SBSES was performed at the end of 6 months. Statistical analysis included summary statistics, analysis of variance tests, and a significance level of < 0.05. RESULTS: The study included 50 patients, with 54% males and 46% females. The most common age group was 21-40 years, and melanocytic nevi was the most common cause for excision. Lesions were excised mainly from the lateral wall and dorsum of the nose. The mean SBSES score was 3.79 ± 0.467. The best cosmetic outcome was observed in lesions involving the dorsum followed by the root of the nose. No significant difference was found among the SBSES scores for various subunits. CONCLUSION: Elliptical excision with primary closure is a simple and effective technique for small nasal defects, providing good cosmetic results. The location of the nasal defect influences the cosmetic outcome, with better results observed in areas with thinner skin and increased skin laxity.

We present a case series of nonhealing, trophic ulcers with underlying leprous neuropathy, treated successfully with PRF therapy. It was a prospective study, conducted in a tertiary care hospital in the western Maharashtra, India between April 2021 and March 2022. A total of 11 patients with 18 nonhealing trophic ulcers were included in the study and consisted of 8 men and 3 women. The patient's median age was 41 years (range, 33-68), the median size of the ulcers was 4 cm (range: 1-12 cm), the median duration of ulcers was 20 weeks (16-30 weeks), and time to heal after starting treatment was 4 weeks (range: 1-8 weeks). PRF is an easy-to-use, simple, safe, effective, and economical treatment for the management of trophic ulcers.

Here, the hub of the syringe is cut obliquely to create a beveled or slant edge to create a custom nail separator which is a useful device for the easy and less painful separation of the ingrown nail plate from its gutter, which is an important stage of nail splinting.

This study ai...OBJECTIVES: Acne scarring occurs from skin damage following acne healing. Treatments such as chemical peels, dermabrasion, and laser therapy can effectively enhance scars' appearance but could be expensive. This study aimed to reflect participants' awareness of treatment options and their willingness to pay (WTP), evaluating their association with sociodemographics, severity, and quality of life (QoL). MATERIAL AND METHODS: A cross-sectional study was conducted in Riyadh, Saudi Arabia, from June to July of 2023. Study participants were chosen using a non-probability sampling technique. Participants were aged 18 or older, had acne scarring, and resided in Riyadh. Data analysis was performed using RStudio, with < 0.05 indicating statistical significance. RESULTS: A total of 401 adult participants were included in the study. The prevalence of acne scarring among the participants was 63.4%. About 45.8% of the participants expressed a WTP of <1000 Saudi Riyals (SAR), while 42.2% were willing to pay 1000-6000 SAR. The most well-recognized acne scar management modalities were laser resurfacing (66.3%) and chemical peels (64.1%), while the least recognized were collagen/fat filler injections (23.9%) and radiofrequency microneedling (27.7%). CONCLUSION: The most recognized acne scar treatment modalities were laser resurfacing and chemical peels. Participants were willing to pay for reasons such as psychological impact, income levels, and awareness of treatment modalities. The findings could influence dermatologists to start awareness campaigns to debunk misconceptions about acne scar treatment modalities and their efficacy in improving QoL.

Cryotherapy is a good option for treating the toe intertrigo, but its availability is a major concern. Here as an alternative to cryotherapy, trichloroacetic acid (TCA) cautery and Sodium hypochlorite (NaOCl) (a broad spectrum anti-infective agent) solution is advocated as a simple, feasible, and cost-effective treatment of chronic inter digital intertrigo.

MATER...OBJECTIVES: This study aimed to evaluate the efficacy of autologous non-cultured non-trypsinized melanocytekeratinocyte grafting (JT) with suction blister membrane as biological dressing in stable vitiligo patches. MATERIAL AND METHODS: The study was conducted at Mathura Das Mathur Hospital (MDM), Jodhpur, on 43 patches of Stable vitiligo. It was a prospective single-arm interventional study. All patients were examined clinically and history and informed consent were taken before starting the procedure. RESULTS: About 95% of patches showed >50% repigmentation with 83.72% having good to excellent repigmentation. About 88% of patches showed good color matches with an excellent decline in Dermatology Life Quality Index (DLQI). No serious complication was observed in the present study. The most common complication observed both in the donor and recipient area was pain. CONCLUSION: Our study indicates that combining JT with suction blister epidermal grafting is a comparatively effective technique in terms of good repigmentation (≥75% extent of repigmentation), color match, fewer side effects, good patient satisfaction, and DLQI reduction.

The chemical reconstruction of skin scars (CROSS) technique has been used with high concentrations of trichloroacetic acid (TCA) which often produces unacceptable side effects. There is a dearth of data, with 50% glycolic acid (GA) for the same indication in the management of acne scars in the Indian population. This study aimed to assess the clinical response of acne scars after the application of 50% GA with that after the use of 65% TCA in a similar manner. MATERIAL AND METHODS: An institution-based prospective comparative study was performed with patients aged 16-45 years of either sex with acne scars and not been treated within the past 1 year. Subjects were assigned to receive one of the formulations (50% GA [Group A] and 65% TCA [Group B]). Grading of scars was done on day 1 and day 35 based on patients' assessment on a four-point visual scale and physicians' assessment by the Goodman-Baron qualitative global acne scar grading scale. The procedure was repeated every fortnightly for three such. Data were analyzed by Statistica version 6 (Tulsa, Oklahoma: StatSoft Inc. 2001). RESULTS: The visual improvement scale of patients showed 41.38% ( = 12) fair improvement in group A, whereas 58.06% ( = 18) showed good improvement in group B. Grading by the Goodman-Baron scale showed 9.68% ( = 3) showed four grade changes in group B. CONCLUSION: Improvement is best observed with 65% TCA. Adverse effects were noted more with 65% TCA, especially acneiform eruption which was lacking in the previous studies. GA can be a safer alternative to TCA with acceptable results. Our study opens the scientific window for future research on different concentrations of GA as a CROSS agent.

Many theories have been proposed to explain its etio...Vitiligo is an acquired depigmenting disorder due to the destructive loss of melanocytes, clinically presenting as hypopigmented or depigmented macules and/or patches. Many theories have been proposed to explain its etiopathogenesis among which cell-mediated immunity is one of the crucial links. Estimation of vitiligo activity and extent in a patient is important in tailoring an optimal treatment regimen. Interleukin-6 (IL-6) and high-sensitivity C-reactive protein (HsCRP) are sensitive indicators for systemic inflammation and are found to be relevant in determining vitiligo disease activity. This study was conducted to estimate serum levels of IL-6 and HsCRP in vitiligo patients and to correlate them with the disease activity and extent in order to assess if these serum markers serve as objective indicators of vitiligo disease activity. This was a hospital-based cross-sectional study of 58 vitiligo patients diagnosed clinically irrespective of age, gender, and any ongoing or past treatment. Disease activity and extent were calculated using the vitiligo disease activity (VIDA) score and vitiligo area severity index (VASI), respectively. Serum levels of IL-6 and HsCRP were obtained and their correlation with VIDA and VASI values were statistically analyzed. A weak negative statistically insignificant correlation was found between IL-6 and VIDA (P = 0.092). No correlation was found between VIDA and HsCRP (P = 0.998). A weak positive, statistically insignificant correlation was found between VASI and IL-6 as well as between VASI and HsCRP (P = 0.175 and P = 0.238, respectively). Although statistically insignificant, the patients who were not on immunosuppressive therapy showed higher mean values of IL-6 and HsCRP compared to those who were on immunosuppressive therapy. In contrast to the findings of previous studies, our study found a weak negative correlation between VIDA and IL-6 levels possibly attributable to the difference between the mean levels of IL-6 among the subgroups of patients who were, and were not on immunosuppressive therapy. The VIDA score and HsCRP levels did not show any statistical correlation. However, patients who were not on immunosuppressive therapy showed a higher albeit statistically insignificant mean value of HsCRP. Our observations suggest that any ongoing and/or treatment in the recent past, especially immunosuppressive therapy, and any co-morbidities should be essentially considered while investigating for sensitive serum markers of inflammation as determinants of vitiligo disease activity.

Forensic anthropologists routinely compare the morphological characteristics of the hair samples to determine a transfer. Assessment of parameters such as hair density, hair diameter, follicular units, and empty hair follicle/yellow dots are useful for the diagnosis of various hair disorders, monitoring treatment outcomes, and research purposes. The study aimed to analyze the relationship between age and hair density. MATERIAL AND METHODS: The present cross-sectional observational study was carried out at the Department of Dermatology at a tertiary care center. All the subjects attending the Dermatology Outpatient Department fulfilling inclusion criteria were studied. Data were collected after the approval from the Institutional Ethical Committee. The procedure was explained and a written consent was taken. Each patient's hair was parted in the middle and images from a point of roughly 5 inches from the patient's glabella were taken. The parameters such as hair thickness, hair density, hair count, follicular units, and empty hair follicles/yellow dots were noted. RESULTS: In the present study, the majority of the study subjects belonged to the age group 26-35 years (38%), followed by 18-25 years (34.7%), 36-45 years (20.7%), and least in the age group of 46-55 years (6.7%). In the present study, the mean unit density in males was 104.78 ± 14.33 and that of females was 108.36 ± 17.60. CONCLUSION: Mean hair density varies concerning age group in the study population.

The objective of this study was to compare the efficacy and safety of carbon dioxide laser versus microfractional radiofrequency in treatment of acne scars in skin type 3,4. MATERIAL AND METHODS: Thirty-two patients of skin types 3-4 with grade 3 and 4 atrophic acne scars, of which 16 were treated with fractional carbon dioxide (CO) laser and 16 were treated with microfractional radiofrequency were retrospectively analyzed. All patients received either treatment 4 sessions at an interval of 4-6 weeks. Objective assessment was done with qualitative and quantitative Goodman and baron acne scar grading system by assessment of photographs at baseline and 3 months after last session. RESULTS: All the data collected were entered in a Microsoft Excel worksheet and analyzed using R Software R-4.2.1. The study variables acne scar grades were described using frequency with percentage, acne scar scores were described using Mean with Standard deviation, and Median with interquartile range (IQR) (Q1-Q3). Since the data were ordinal, which is non-normal, hence, non-parametric tests are applied. To compare grades between the groups, Mann-Whitney U-test was applied. CONCLUSION: There was no difference in the last session (after the treatment) between the groups, the Mann- Whitney U-test was 109.5 with -value of 0.484 >0.05. Therefore, there was no statistically significant difference between the microfractional radiofrequency and carbon dioxide laser group. There were 2 (12.5%) cases reported with post-inflammatory hyperpigmentation in the CO laser group and zero cases in the microfractional radiofrequency group.

Advancements in trustworthy techniques have made it feasible to alter the visual characteristics of the lips through the utilization of various injectable substances and surgical procedures. However, there is a lack of standardization in these techniques. The purpose of this review is to focus on novel dermal filler injection and surgical techniques for lip augmentation. We did a scoping review of the literature using PubMed, Embase, and the Google Scholar database to identify all original articles published up to June 2023, assessing new techniques that focus on lip augmentation. In our review, only two studies out of seven included studies were surgical whereas the other five studies were non-surgical. This suggests that injection of dermal fillers is currently the most opted technique performed. This review suggests that incorporating perioral rejuvenation into the plan for lower facial surgery can help to complete the youthful look sought by patients. Injectable fillers provide excellent but short-term benefits and additional options such as Modified upper lip lift and French kiss techniques exist that offer the patient permanent improvements at a low cost.

MATERIAL AND METHODS: An open-label study was conducted to evaluate the efficacy and safety of MNRF for the treatment of androgenetic alopecia. Patients diagnosed with patterned hair loss received three MNRF treatment sessions once monthly. Efficacy was assessed by comparing before and after photographs and trichoscopy. RESULTS: Twenty-four patients were recruited. Excellent improvement in hair growth was observed in 14 patients, fair improvement in seven patients, and no improvement in three patients without significant adverse effects. Slight pain lasting for a day was the most common adverse event following MNRF. CONCLUSION: Overall, significant improvement was observed in the majority of the patients without significant side effects.

Most of the available literature on chemical peels focuses on their role in skin rejuvenation and hyperpigmentation. There is a paucity of well-conducted studies that have compared peels in the treatment of acne scars in Asian patients. MATERIAL AND METHODS: The study aimed to compare the efficacy of 70% glycolic acid (GA) and 30% trichloroacetic acid (TCA) chemical peels in the treatment of facial atrophic acne scars. A longitudinal, right-left study was conducted to assess the effectiveness and side effects of 70% GA and 30% TCA in 30 patients with acne scars. A total of four peeling sessions were performed every 4 weeks. Evaluation of the response was done using Goodman and Baron's quantitative global acne scarring grading system (GBASG), physician visual analog scale (VAS), and patient VAS at baseline and 4, 8, 12, and 16 weeks. Relevant statistical tests were employed to study the effectiveness of both TCA and GA peels. RESULTS: Significant reduction was noted in mean GBASG scores on both sides at 8, 12, and 16 compared to baseline score ( = 0.000). Mean GBASG scores decreased from 12.67 ± 3.19 to 8.97 ± 2.73 on the 70% GA side while that on the 30% TCA side reduced from 13.20 ± 3.56 to 6.83 ± 2.60 ( = 0.003). The results were much better on the TCA peel side as compared to GA peel as per physician VAS ( = 0.000) and patient VAS ( = 0.000). Side effects such as post-inflammatory hyperpigmentation and acne were seen on both sides while dryness and crusting were more common on the TCA peel side. CONCLUSION: A 30% TCA peel is efficacious and well-tolerated for mild-to-moderate acne scars. About 70% GA peel is an effective alternative to TCA peel, especially for patients not tolerating TCA or requiring lesser downtime.

Since ancient times, it has been used to treat acute wounds. However, the efficacy of aloe vera for burns and split-thickness skin graft (STSG) donor sites is inconclusive. In this study, we have applied pure aloe vera gel extract as an adjunct to dressing second-degree (superficial) burn wounds and assessed the time for complete epithelialization. The time taken for complete epithelialization of the wound was 11 days when compared to the normal (evidence-based) time of 14 days.
