Many patients in this group exhibit reduced vaccine responses and require tailored vaccination regimens. Following the launch of the national Adult Immunisation Programme, all doctors must stay abreast of current guidance on vaccination in immunosuppressed patients. This clinical review article summarises interdisciplinary knowledge on patients with a reduced vaccine response.

BACKGROUND AND AIM: High-dose melphalan and autologous haematopoietic stem-cell transplantation (ASCT) is effective against multiple myeloma. The type of treatment patients receive before and after ASCT may affect time to relapse and survival. The aim of this study was to compare the treatments administered before and after ASCT at the four hospitals offering ASCT in Norway during the period 2008-2020. MATERIAL AND METHOD: All patients who received ASCT as first-line treatment for multiple myeloma in Norway in the period 1 January 2008-31 December 2020 were included in the study. Data on demographics, disease and treatment were recorded. Patients were followed until disease progression, death or study completion. RESULTS: Patients who received ASCT at St Olav's University Hospital received almost exclusively bortezomib-cyclophosphamide-dexamethasone as induction therapy, while bortezomib-lenalidomide-dexamethasone dominated at the three other hospitals after 2017. Maintenance therapy was given to 27 % of patients receiving ASCT in Oslo, compared with 2-16 % at the other centres. Median progression-free survival ranged from 29 to 38 months, and median overall survival ranged from 102 to 120 months across the centres. INTERPRETATION: Significant variations were observed in the treatments administered before and after ASCT, especially after 2017. National guidelines allowed for multiple treatment options, which has resulted in heterogeneous treatment practices. These different treatment approaches may have contributed to differences in progression-free and overall survival.

BACKGROUND AND AIM: In the flipped classroom model, students prepare for lessons in advance, enabling teaching sessions to be used for discussion, reflection and other active learning activities. The aim of this qualitative study was to explore the impact of preparatory tasks on medical students' motivation, engagement and perceived learning. MATERIAL AND METHOD: Two focus group interviews were conducted in May 2024 with 14 medical students at the University of Oslo, Campus South. The students had experience with both traditional lectures and the flipped classroom. The interviews were analysed using a reflexive thematic approach, with a focus on students' experiences with preparation and learning. RESULTS: Student motivation increased when expectations were clearly expressed and preparatory tasks were adapted to the scope of the teaching session. Students preferred brief preparatory tasks that were provided well in advance. Active incorporation of preparatory work into lessons, in the form of quizzes, discussions and case studies, promoted engagement and self-efficacy. Students felt that the flipped classroom approach deepened their understanding of the material and increased their confidence in academic discussions, particularly in relation to complex topics. They also felt that the teaching model supported the development of professional identity and preparation for clinical practice. INTERPRETATION: Combining well-defined preparatory tasks and active incorporation of these into the flipped classroom promotes students' motivation, engagement, self-efficacy and reflective thinking.
