Von Willebrand disease is the most common inherited bleeding disorder and is characterised by bleeding from the skin and mucous membranes. The severity of the disease can vary, and women are often more severely affected than men. Patients with von Willebrand disease can require multidisciplinary follow-up, and special precautions need to be taken during surgery and invasive procedures. We present a clinical review of von Willebrand disease in order to raise awareness of this patient group among doctors in Norway.

BACKGROUND: Delirium is common in patients admitted to hospital, and may sometimes be the first sign of a serious underlying condition. Identifying a specific cause can be complex, especially in patients without clear precipitating factors. CASE PRESENTATION: A woman in her sixties was admitted to hospital with neurological symptoms and acute confusion. Delirium of unknown origin was suspected. Blood tests revealed elevated troponins and ECG showed subtle ST changes, but she reported no chest pain. CT revealed infarctions in the kidney and spleen, and brain MRI showed multiple embolic infarctions. Despite negative blood cultures and absence of other signs of infection, bacterial endocarditis was suspected, and empirical antibiotics were started. Transthoracic echocardiography was initially inconclusive, and transoesophageal echocardiography (TEE) was not feasible due to her cognitive state. PET-CT showed pathological FDG uptake in cervical lymph nodes, and biopsy revealed metastatic adenocarcinoma. As her delirium gradually improved, TEE revealed aortic valve vegetations. Cardiac MRI demonstrated embolic myocardial infarction. INTERPRETATION: With a negative infectious workup and confirmed malignancy, the diagnosis of non-bacterial thrombotic endocarditis was established. Anticoagulation with low-molecular-weight heparin was initiated. This case highlights the importance of a broad diagnostic approach in delirium when initial evaluation does not reveal a clear cause.

BACKGROUND AND AIM: Anaemia is associated with increased morbidity and mortality in surgical patients, but it is often a modifiable condition. International studies have reported a high prevalence of preoperative anaemia (30-40 %), particularly due to iron deficiency. This article aims to describe the prevalence of preoperative anaemia in a Norwegian surgical patient population, which includes urological, gynaecological, orthopaedic and gastrointestinal patients. MATERIAL AND METHOD: Adult patients undergoing elective surgery with a risk of > 500 mL blood loss at Bærum Hospital between 1 January and 30 June 2024 were included. Demographic and perioperative data were collected retrospectively and included surgical category, haemoglobin concentration, ferritin measurement, red blood cell transfusion and iron supplementation. RESULTS: A total of 293 patients were included in the study. Preoperative anaemia (haemoglobin level < 13 g/dL) was identified in 99/293 patients (34 %). Ferritin was measured in 25/99 patients (25 %) with preoperative anaemia. Preoperative iron supplementation was administered to 27/99 patients (27 %) with anaemia. Overall, 29/293 patients (10 %) received red blood cell transfusions during the perioperative period. INTERPRETATION: The prevalence of preoperative anaemia in this surgical population is comparable to that reported in larger European studies. Ferritin measurement is part of the diagnostic evaluation and treatment stratification of anaemia. The low rate of preoperative ferritin testing suggests a potential for improvement in the preoperative optimisation of haemoglobin levels.

BACKGROUND AND AIM: Sexual assault is associated with serious health consequences. Tailored medical follow-up after a sexual assault can be crucial during this difficult time and can help prevent long-term health problems. We aimed to examine attendance and non-attendance at medical follow-up appointments at the Sexual Assault Centre in Oslo, and whether those who initially failed to attend did so after contact had been re-established. MATERIAL AND METHOD: As a quality improvement project, we conducted a retrospective observational study of patients at the Sexual Assault Centre in Oslo in 2022. Medical records up to and including 26 weeks after presentation were reviewed. We recorded attendance and non-attendance for medical follow-up appointments, and whether patients who had not initially attended subsequently did so after contact had been re-established. RESULTS: Of the 579 patients who accepted medical follow-up, 275 (47.5 %) attended their planned follow-up appointments. Attempts were made to re-establish contact with 274 patients, and of these, 169 (61.7 %) attended one or more follow-up appointments. Overall, 444 of 579 (76.7 %) attended one or more follow-up appointments. INTERPRETATION: The findings suggest that by re-establishing contact with patients who miss appointments, the Sexual Assault Centre helps ensure more individuals receive medical care after a sexual assault.
