Granuloma annulare (GA) is an inflammatory skin disease typically characterized by an erythematous eruption consisting of papules and annular plaques. Granuloma annulare (GA) is an inflammatory skin disease typically characterized by an erythematous eruption consisting of papules and annular plaques. GA can have a severe impact on quality of life, especially when widespread. GA can be difficult to treat and often recurs after treatment is stopped; there remain no FDA-approved therapies. In recent years, there has been significant progress in understanding the epidemiology, disease associations, and molecular pathogenesis of GA. Patients with GA are more likely to have hyperlipidemia, diabetes mellitus, and autoimmune diseases including thyroiditis, rheumatoid arthritis, and systemic lupus erythematosus. These associations further underscore the importance of recognizing GA and evaluating for comorbid disease. Accurate diagnosis of GA requires distinguishing it from its clinical and histologic mimics, some of which share annular morphology or granulomatous inflammation. Molecular work has suggested a T-cell mediated pathogenesis and identified key cytokines and other signals that drive macrophage accumulation and activation in tissue. Emerging therapies that block these cytokine signals are showing promise in the clinic. In this article, we provide an updated overview of the epidemiology, disease associations, clinical and histopathologic characteristics, molecular pathogenesis, and treatment of GA.

BACKGROUND: Early detection of skin cancer improves outcomes, but the utility of dermatologist-performed total body skin examinations(TBSEs) in unselected populations remains uncertain. OBJECTIVE: To evaluate the diagnostic yield, biopsy burden, and temporal trends of dermatologist-led TBSEs in an academic screening program. METHODS: This retrospective cohort included 3,127 encounters from 1,572 University of Miami employees and spouses who underwent annual TBSEs between January 2019-June 2024. Primary outcomes were detection of premalignant(actinic keratoses) and malignant lesions (basal cell carcinoma[BCC], squamous cell carcinoma[SCC], melanoma). Secondary outcomes included number needed to biopsy(NNB), predictive value of patient-reported lesions, and changes across screening rounds. RESULTS: Across TBSEs, premalignant, in-situ, and malignant lesions were detected in 11.0%, 0.22%, and 2.6% of exams. Of the screenings with malignant findings, 69 found BCC while 12 found SCC. Detection was highest among older adults and those with prior nonmelanoma skin cancer. Patient-reported "spots of concern" had low predictive value (PPV 2.7% for premalignant, 0.5% for in-situ, 1.2% for malignant). Overall NNB was 4.83, improving from 6.10 to 2.79 across rounds. LIMITATIONS: Participation was voluntary, introducing selection bias; biopsy practices varied, and external procedures may have been missed. CONCLUSIONS: Dermatologist-performed TBSEs identified early-stage skin cancers, with higher yield in higher-risk groups.

OBJECTIVE: To evaluate Co... BACKGROUND: Acute herpes zoster (HZ) neuralgia impairs quality of life and may progress to postherpetic neuralgia (PHN). Existing analgesics are often limited by adverse effects in older adults. OBJECTIVE: To evaluate Cobratide injection for moderate-to-severe acute HZ neuralgia and explore its potential effect on PHN incidence. METHODS: In this multicenter, randomized, double-blind, double-dummy, active-controlled exploratory trial, 80 patients aged 50 to 70 years were assigned 1:1 to intramuscular Cobratide injection or oral Pregabalin for 28 days. The primary endpoint was change in average Numeric Rating Scale (NRS) pain score from baseline to day 28. No formal power calculation was performed. RESULTS: At day 28, least-squares mean NRS reductions were 6.00 with Cobratide injection and 5.63 with Pregabalin (between-group difference, 0.37; P=.186). At week 12, PHN occurred in 0/27 and 3/27 evaluable patients, respectively (P=.236). Treatment-emergent adverse events occurred in 35.9% and 51.2% of patients, respectively. LIMITATIONS: Exploratory design, small sample size, and lack of systematic collection of zoster vaccination history and rash-onset timing limited inference. CONCLUSIONS: Cobratide injection showed a similar magnitude of pain reduction to Pregabalin, with a favorable tolerability signal. Larger adequately powered trials are needed.
