Immune checkpoint inhibitors (ICIs) have markedly advanced oncology by enhancing the immune system's ability to combat malignancies. However, their use is increasingly associated with immune-related adverse events, including immunobullous dermatoses. These disorders, such as bullous pemphigoid, pemphigus vulgaris, linear immunoglobulin A bullous dermatosis, mucous membrane pemphigoid, paraneoplastic pemphigus, dermatitis herpetiformis, and lichen planus pemphigoides present significant diagnostic and therapeutic challenges. This clinical review explores the pathogenesis, clinical presentation, diagnosis, and management strategies of immunobullous dermatoses triggered by ICIs, highlighting the complex relationship between effective cancer treatment and immune regulation. ICI-induced immunobullous dermatoses closely mimic their conventional autoimmune counterparts but often have unique clinical considerations, such as delayed onset, mucosal predilection, and persistence post-discontinuation of medication. Diagnostic evaluation relies on clinical suspicion, histopathology, direct and indirect immunofluorescence, and enzyme-linked immunosorbent assays. Management typically involves corticosteroids and immunosuppressants, with biologics such as rituximab, dupilumab and omalizumab reserved for refractory cases. Decisions regarding the continuation of ICIs require individualized assessment, balancing cancer control with autoimmune toxicity. Dermatologic and oncologic teams must navigate dual priorities of mitigating autoimmune toxicity and preserving oncologic benefit.

BACKGROUND: Hematoporphyrin monomethyl ether-mediated photodynamic therapy (HMME-PDT) is an emerging treatment for port-wine stain (PWS). OBJECTIVE: To compare the effectiveness and safety of HMME-PDT with pulsed dye laser (PDL) for PWS on the face and neck. METHODS: A retrospective study was conducted comparing 728 patients treated with HMME-PDT and 655 with PDL. Logistic regression was used to assess effectiveness. An excellent improvement was defined as ≥75% lesion clearance. RESULTS: Before adjustment, 41.5% of patients showed excellent improvement after a median of 2 HMME-PDT sessions and 29.2% after a median of 3 PDL sessions. After adjusting for confounders, the HMME-PDT group was more likely to achieve excellent improvement (odds ratio: 2.90, 95% confidence interval: 2.02-4.15, P < .05) than the PDL group. Effectiveness was primarily influenced by lesion size, thickness, number of treatment sessions, and prior treatment. HMME-PDT showed higher incidences of hyperpigmentation (5.4% vs 1.8%, P < .05) and scarring (3.3% vs 0.5%, P < .05) than PDL. LIMITATIONS: The retrospective design and lack of long-term follow-up. CONCLUSION: HMME-PDT showed superior effectiveness than PDL for treating facial and neck PWS, but it carried a higher risk of hyperpigmentation and scarring.

Interleukin-1 (IL-1) plays a key role in inflammasome activation, keratinocyte signalling and neutrophil recruitment, and is a driving force behind many neutrophil-rich and suppurative dermatoses. There is robust evidence supporting the use of IL-1 blockade as a disease-modifying therapy in cryopyrin-associated periodic syndromes and other monogenic periodic fever syndromes, as well as in the treatment of Schnitzler syndrome, where it can rapidly control urticarial and neutrophilic eruptions. Cohort- and series-level data demonstrate high efficacy in IL-1 receptor antagonist deficiency and steroid-refractory pyoderma gangrenosum, whereas benefits appear more heterogeneous in hidradenitis suppurativa, pustular psoriasis and PSTPIP1-associated autoinflammatory diseases. In contrast, conditions such as IL-36 receptor antagonist deficiency, SAPHO syndrome, Sweet syndrome and VEXAS syndrome are supported only by scattered case reports or theoretical rationale, with highly variable or uncertain responses to IL-1 blockade, highlighting the importance of careful clinical and, where possible, genetic phenotyping. We summarise practical considerations for selecting agents, pediatric and adult dosing and safety. IL-1 antagonists are indispensable for a subset of IL-1-driven disorders and represent a rational rescue option for selected neutrophilic dermatoses.
