In the context of aging associated with such comorbidities a...The cardiovascular system balance can be disturbed by a various number of factors, some of them modifiable and others, such as age, which cannot be influenced. In the context of aging associated with such comorbidities as diabetes mellitus and rheumatoid arthritis, we wonder if the classical cardiovascular risk factors are enough to assess the probability of cardiac decompensation, or other factors may be involved. We present the case of a 76-year-old patient know with permanent atrial fibrillation, high blood pressure, diabetes mellitus, hypodiastolic heart failure and rheumatoid arthritis who was admitted with cardiac decompensation without evident cause and despite apparent therapeutic compliance. Geriatric assessment revealed marked mnesic and cognition deficit, malnutrition risk, dependency, depression and frailty. In such patients, the role of cardiac sarcopenia and rheumatoid arthritis in the decline of the cardiac function and patient outcome must be assessed and then managed.

The group of atypical Spitz tumors represen...Atypical Spitzoid tumors represent a poorly defined and characterized category of melanocytic tumors with histologic features of both benign Spitz nevi and malignant melanomas. The group of atypical Spitz tumors represents a mixture of Spitz nevi with atypical features and Spitzoid melanomas. We report the case of an eight years old female patient, who was admitted in Dermatology Clinic Craiova for the presence of a skin tumor with 1/0.8 cm diameter, irregular edges, brown - black coloured with irregular pigment distribution. The skin tumor was located on the dorsal foot above the third metatarsal. The skin tumor appeared in the first two years of childhood and above the third metatarsal. The skin tumor appeared in the first two years of childhood and had slow increase. From about six months, the tumor had changed the colour becoming darker and had also a rapid increase in size. The patient had no comorbidities and had no family history of malignancy or other skin diseases. In our clinic we did the surgical excision of the skin tumor and sent the fragment for the histopathological examination and immunohistochenmical tests. After the clinical examination, dermoscopic evaluation, histopathological and immunohistochemical examination, our diagnosis was Atypical Spitz Tumor.

We found bronchial sibilant rales, wheezing, cyanosis, peripheral...We present the case of a 14 years-old male (with poor living condition, non-smoker) hospitalized for a sudden onset of a rest dyspnea, mucopurulent cough. We found bronchial sibilant rales, wheezing, cyanosis, peripheral adenopathies, hepatosplenomegaly, purulent tonsillitis. the chest-x-ray revealed bilateral mediastinal lymphadenopathy and bilateral lung infiltrations. Spirometry: severe mixed ventilator dysfunction without reversibility. Sputum: negative microscopy and culture for Koch bacillus. Stool examination: cysts of Lamblia giardia. The patient refused bronchoscopy and mediastinoscopy, so it was performed axillary lymph node biopsy which confirmed sarcoidosis (non-caseating epithelioid granulomas). The treatment included antibiotics, antiparasites, oral and inhaled corticosteroids (CS), bronchodilators, oxygen, with clinical/functional improvement after 3 months. CS was followed 1.5 years with poor compliance. A relapse occurred after 3 years and the CS were reinserted. The computerized tomography (CT) scan revealed a diffuse interstitial fibrosis with bronchiectasis. The case particularity relies on the atypical early onset of the sarcoidosis, with respiratory failure and progression to lung fibrosis despite CS treatment. The association of proinflammatory risk factors such as multiple infections needs to be noted.

MATERIALS AND METHODS: The study was conducted on a group of subjects with type 2 diabetes. The collected data included: anthropometry, lifestyle, blood measurements, and the mean daily nutrient intake assessed by the EPIC food frequency questionnaire. RESULTS: The study group included 101 subjects with type 2 diabetes mellitus (DM) with a mean age of 60.6±10 years, of which 45 men (59±10.6 years) and 56 women (62±9.6 years). The average energy intake was 1714.4±713.9 Kcal/day, with a statistically significant gender difference (1877.7±850.9 kcal/day for men, 1583.1±554.9 kcal/day for women, p = 0.039, and the average daily micronutrient intake was 2.44±1.44 μg/day for vitamin D, 724.5μ263.2 mg/day for Calcium (Ca), 266.7±98 mg/day for magnesium (Mg), which was low compared to dietary recommendations. Calcium intake was correlated with intake of carbohydrates, saturated fatty acids (SFA), monounsaturated fatty acids (MUFA), fiber and energy. Vitamin D intake was correlated only with protein intake (R=0.695), while magnesium intake correlated with intake of fibers, energy, carbohydrates and SFA. CONCLUSIONS: Our study in patients with type 2 diabetes showed a reduced average intake of calcium, magnesium and vitamin D compared to standard recommendations, requiring an individualized approach.

MATERIAL AND METHODS: Parents of children aged up to 24 months were asked on a three-day recall...OBJECTIVES: To define the normal frequency and consistency of stools of healthy Indian children between 0-24 months of age. MATERIAL AND METHODS: Parents of children aged up to 24 months were asked on a three-day recall basis about their child's usual bowel habits and dietary history. Bowel habit was recorded in terms of number of stools the child passes per day, stool consistency, the age at which night bowel movements stopped, and age of regular bowel movements. Feeding type was recorded as exclusive breast feed, mixed milk feeds or on solid feeds. The bowel habits were correlated with the age of the child and type of feeding. RESULTS: Children in first six months of life had stools which were predominantly "pasty" and "runny like cream" with a frequency of one to two per day. On analyzing the combined effect of type of milk feed and age on bowel frequency and consistency children beyond one month of age either on exclusive breast feed or on mixed milk feed had similar frequency and consistency of stools. By one year of age more than 90% of children had attained regular stool pattern and no night time bowel movements. CONCLUSIONS: First of its kind study from India which describes the stool pattern of normal health children less than two years of age.

MATERIAL AND METHODS: We conducted a...AIM: To analyze the correlations between inflammation markers and ApoB100 and angiotensin converting enzyme (ACE) gene polymorphism and the severity of coronary artery disease (CAD). MATERIAL AND METHODS: We conducted a study in 58 patients with acute coronary syndromes (ACS) who underwent coronarography at the Iasi "Prof. Dr. George I.M. Georgescu' Institute of Cardiovascular Diseases. the patients included in the studies were selected from those who needed a coronarography for unstable angina or acute myocardial infarction. The data were uploaded and processed using the statistical functions in SPPS 18.0 at a 95% materiality threshold. RESULTS: Elevated inflammation markers were found in all study patients, with small differences in distribution. None of the study patients presented ApoB100 gene mutations. As to ACE polymorphism, a predominance of genotype II in unicoronary patients and ID and DD genotypes in bicoronary and tricoronary patients was found. CONCLUSIONS: The results of our study confirm the role of genetic and epigenetic factors in the severity and progression of the coronary disease, leaving room for larger and more comprehensive studies and new research perspectives.

Teaching cardiopulmonary resuscitation in Romanian schools is a joint initiative of the Acute Cardiac Care Working Group of the Romanian Society of Cardiology, the Romanian National Resuscitation Council and the Romanian...Teaching cardiopulmonary resuscitation in Romanian schools is a joint initiative of the Acute Cardiac Care Working Group of the Romanian Society of Cardiology, the Romanian National Resuscitation Council and the Romanian Society of Emergency Medicine and Disaster. Teaching young generations to save people in cardiac arrest requires a structural approach, whose starting point is the launching of a large-scale campaign in favor of introducing the basics of cardiopulmonary resuscitation (CPR) in the school curriculum. The training stages can be set according to the principle of repetition (i.e. every two years). Teachers seem to be the best candidates for the instructor position, taking into account the knowledge and skills they acquired throughout the teacher training programs, as well as, their continuous contact with the students; consequently, training teachers to become CPR instructors represents a long term investment. All acquired knowledge will implicitly spread to a secondary level (trainee's parents, relatives, etc.) and the number of individuals gaining access to CPR specific information will rise as a consequence. Entering the CPR training program should be promoted as a criterion for the accreditation and/or evaluation of primary schools, middle schools, and high schools. To begin with, this program must be implemented in Romania at first as part of the non-formal context of the special education week called "Şcoala altfel" ("School otherwise").

The main diagnosis of a patient with postinfectious irritable bowel syndrome was till this summer, clinically based on Roma III criteria. The Rome IV criteria brought some changes that involve also the post infectious irritable bowel syndrome, recognizing further the postinfectious IBS as a specific entity according to the multidimensional clinical chronic mucosal inflammation triggered by enteric infection, may underlie persistent bowel symptoms in patients who develop postinfectious irritable bowel syndrome.

Colorectal neoplasms and/or pre-neoplasms can be prevented by interfering with the various steps of oncogenesis, which begins with uncontrolled epithelial cell replication, continues with the formation of adenomas and eventually evolves into malignancy. The knowledge described herein will help to reduce and prevent this malignancy, which is one of the most frequent neoplasms in some developed countries. Genetics, experimental and epidemiologic studies suggest that colorectal cancer results from complex interactions between inherited susceptibility and environmental factors. Primary prevention involves the identification of genetic, biologic, and environmental factors that are etiologic or pathogenic in the development of cancer, and subsequent complete or significant interference with their effects on carcinogenesis.

Liver biopsy is considered the gold standard in the diagnosis and progression of the disease and its usefulness cannot be neglected in terms of research. But in current clinical practice, liver biopsy tends to be replaced by less expensive and noninvasive methods allowing the identification of cardiovascular and metabolic risks. Given the fact that a small percentage of individuals with nonalcoholic fatty liver disease will progress to cirrhosis, attention will be focused more on cardiovascular risk as nonalcoholic fatty liver is now regarded as a distinct component of the metabolic syndrome. The aim of the clinician is to identify the early stages of fatty liver, using in this purpose simple and easily accessible methods. Many techniques have been proposed for the diagnosis of nonalcoholic fatty liver, from simple clinical factors (anthropometric indices measuring,blood pressure) laboratory biomarkers, imaging methods and scores, which should allow early treatment. This review describes different methods for identifying nonalcoholic fatty liver and various stages of this disease.

Several similarities have reported between patients suboptimal level of vitamin D and patients with Chronic Obstructive Pulmonary Disease (COPD): both conditions are more prevalent after the 6th decade and are associated...Several similarities have reported between patients suboptimal level of vitamin D and patients with Chronic Obstructive Pulmonary Disease (COPD): both conditions are more prevalent after the 6th decade and are associated with impaired defense mechanisms, with skeletal muscles dysfunction and with an early ageing process. We have reviewed the literature concerning the interrelationship between the main pathophysiological processes of both conditions, in order to emphasize the influence of vitamin D deficit on COPD development and evolution. Epidemiological, clinical and experimental data supporting this association are presented. We concluded that the identification of common pathophysiological mechanisms for the vitamin D deficiency and COPD substantiates the hypothesis of the involvement of this hormone in the chronic obstructive pulmonary disease.

The prevalence of sarcopenia is still uncertain because of the lack of homogeneity of the studied populations, and also because of the variety of techniques. The development of biological markers that can be used in a cost effective manner to guide diagnosis and facilitate monitoring patients with sarcopenia, would mark an important step in managing the care of geriatric patients. Nutrition combined with physical activity is the key component of the management of sarcopenia, with a synergistic effect that helps combat malnutrition and improve the quality of life.

Although statins are the mainstream treatment for lowering LDL-C level, additional LDL-lowering therapies are needed to reduce residual cardiovascular risk, especially in patients at very high risk, or with hereditary lipid disorderes or statin intolerance. The proprotein convertase subtilisin/kexin type 9 (PCSK9) is a key regulator for LDL-Receptor activity and an attractive target for the treatment of hypercholesterolaemia. From its discovery in 2003, several therapeutic approaches to the inhibition of PCSk9 have been proposed. Monclonal antibodies that bind to PCSJ9 received marketing approval in 2015 (alirocumab and evolucumab) or are being evaluated in phase III trials (bococizumab). Many other molecules are in preclinical studies, phase I or II clinical trials. Another point of interest carefully investigated is the cardiovascular benefit of reducing LDL-C using these new molecules. High hopes are invested in them.

The XVIth century is considered a period m...The present paper aims, exploring the history of Renaissance medicine, to evoke the figure and work of the priest, surgeon and anatomist, Guido Guidi (Vidus Vidius) (1509-1569). The XVIth century is considered a period marked by artistic and scientific effervescence in the western part of Europe and Guido Guidi was a first order personality, grandson of Domenico Ghirlandaio and friend of Benvenuto Cellini. He was appointed by the King Francis I the first professor of anatomy and surgery at the newly founded College de France. On demand of the King, he wrote Chirurgia j Graeco in Latinum conversa Vido Vidio Florentino interprete, cum nonnullis eiusdem Vidii comentariis (1544), a beautifully illustrated original surgery book that became for the following two centuries the main source in teaching surgery. Our study realized a detailed assessment of the book and especially of its illustrations belonging to Francesco Salviati. Exploring the life of Guido Guidi, we were also able to point out other significant contributions in the field of anatomy and clinical medicine as De anatome the first book where are presented disarticulated, the bones of the skull base and also the discovery of the chickenpox. Some surgical personalities attributed to him both the elaboration of the term appendix vermiformis and the first description of an aneurysm, he treated with the help of Fallopio. Although forgotten today, Guido Guidi was a leading figure of the Renais sance medicine both in France and Italy.

This is an example of the use of thermoplastic materials in a high-tech medicine field, oncology radiation therapy, in order to produce the rigid masks for positioning and immobilization of the patient during simulation...This is an example of the use of thermoplastic materials in a high-tech medicine field, oncology radiation therapy, in order to produce the rigid masks for positioning and immobilization of the patient during simulation of the treatment procedure, the imaging verification of position and administration of the indicated radiation dose. Implementation of modern techniques of radiation therapy is possible only if provided with performant equipment (CT simulators, linear accelerators of high energy particles provided with multilamellar collimators and imaging verification systems) and accessories that increase the precision of the treatment (special supports for head-neck, thorax, pelvis, head-neck and thorax immobilization masks, compensating materials like bolus type material). The paper illustrates the main steps in modern radiation therapy service and argues the role of thermoplastics in reducing daily patient positioning errors during treatment. As part of quality assurance of irradiation procedure, using a rigid mask is mandatory when applying 3D conformal radiation therapy techniques, radiation therapy with intensity modulated radiation or rotational techninques.

Bilateral breast cancer incidence is appreciated to be between 0.3 to 12% and is determined either by a hereditary load associated with chromosomal instability under the effect of environmental factors, or by the evoluti...Bilateral breast cancer incidence is appreciated to be between 0.3 to 12% and is determined either by a hereditary load associated with chromosomal instability under the effect of environmental factors, or by the evolution in a particular hormonal context which gives biological aggressiveness. We present the case of a patient, aged 38 years, clinically, imagistic and bioptic diagnosed with left axillary lymph node metastases of breast carcinoma NST invasive G3, IHC-RE = 60%, RP = 30%, HER2neu = 2 +, Ki67 = 20%, in August 2013. Patient followed neoadjuvant chemotherapy treatment during September-October 2013. In December 2013 she was clinically and imaging diagnosed with bilateral breast cancer, for which surgical intervention was done which consisted of bilateral radical Madden mastectomy with bilateral axillary lymphadenectomy. BAP-invasive carcinoma NST: left breast-pT2mN3a G2, right breast--pT3mN3a G2, IHC-RE = 90%, RP =70% HER2neu = 2 +, Ki67 = 50%. During the period of January-March 2014, the patient followed adjuvant chemotherapy and Herceptin. Bilateral breast ultrasound assessment in April 2014 revealed: left axilla--liquid blade 29 / 6mm; right axilla--oval ganglion 9/5 mm. Abdominal and pelvic ultrasound: empty uterine cavity, bosselated contour; at left ovary level multiple cystic formations. During the period of May-June 2014, adjuvant radiation therapy and ovarian irradiationwas administered to the patient. Subsequently hormone therapy was initiated. Following CHT / ovarian irradiation patient continues to experience intermittent uterine bleeding, which is why a total hysterectomy with bilateral ovariectomy was done, and BAP: cervical, endometrialand left ovary with tumor multifocal infiltration with histopathological aspect of invasive breast carcinoma NST. Periodic imaging evaluations do not reveal local or distant recurrence. The particularity of this case is synchronous bilateral breast cancer diagnosis in a young patient complicated in its evolution by ovarian metastases. This form of metastasis is rare in young women and occurs in advanced stages of the disease.

Many individuals with trans...UNLABELLED: Probably the most important factor in evaluating a patient's prosthesis is quality of life. Transtibial amputations, are among the most frequently performed major limb amputations. Many individuals with transtibial amputations successfully achieve rehabilitation at or near their preamputation levels. Discomfort in prosthetic sockets continues to be a critical challenge faced by both prosthetists and amputees. MATERIAL AND METHODS: This paper proposes a fusion Graphic User Interface that combines two types of information (pressure and temperature). Data from pressure sensors and thermistors (an electrical resistor whose resistance is greatly reduced by heating, used for measurement and control) placed on the stump in transtibial prosthetics are collected in real time using a National Instruments Data Acquisition device. RESULTS: All the data stored in files are available for offline processing. The user has the possibility to analyse the signal by zooming or positioning the marker and window on different parts of signal. A complex analysis that involves the pressure and temperature for a location (where both sensors are placed) is available in time domain. CONCLUSIONS: Blunt-prosthesis interface is characterized by few parameters among the most important are pressure and friction. The action of these parameters during static and dynamic stage is very important because their actions can produce lesions of skin at the level of interface. Despite the advancements in surgical techniques and prostheses, much still needs to be done. We made certain that the sensors were in the same location by pressing on specific cells on the residual limb during various stages of the experimentation. The highest pressures recorded were during the stance phase of walking. The curve that shows the temperature evolution or pressure in one point could differ in different points from patient to patient.

MATERIAL AND METHODS: The study groups included 61 pa...AIM: The aim of our study was to determine the impact of premature loss of temporary lower molars upon the longitudinal axis of the first and second permanent molars. MATERIAL AND METHODS: The study groups included 61 patients, 6-9 year olds with premature loss of primary molars and a control group of 24 patients with intact temporary teeth. We evaluated the angle between longitudinal axis of first and second lower permanent molars and occlusal plane. RESULTS: It was observed that premature loss of lower second deciduous molar modifies more the vertical axis of first and second permanent molars than the premature loss of first lower primary molar. Reducing space occurs mainly through mesial inclination of molars that separates the edentulous breach. Temporary loss of both lower first molars on the same quadrant causes an accelerated eruption of both premolars increasing the prevalence of eruption sequence: "4-5-3-7". CONCLUSIONS: The preservation of the occlusal morpho-functional complex using space maintainers mainly when the premature loss of the second primary molars occurs is the best interceptive treatment option.

MATERIAL AND METHODS: Prospective study of pregnant women gestational age 11 weeks and 0 days--13 weeks and 6 days, consisting in the determination of pregnancy associated plasma protein (PAPP-A), mean platelet volume (MPV), and ultrasound appearance of placenta. DISCUSSION AND CONCLUSIONS: This study suggests that screening by detailed history and PAPP-A and MPV determination during the first trimester of pregnancy in women at risk for IUGR makes possible the prophylactic treatment and monitoring of pregnancy according to a given protocol and thus neonatal morbidity and mortality to be reduced.
