Motivation, Stress and Satisfaction Among Medical Students.

THE AIM: The aim of the study was to identify the relationship between the strength of motivation of medical students, the symptoms they experience, the thoughts of dropping out and the satisfaction with the academic results obtained during their last exam session. The relationship between depression and physical symptoms was also explored. MATERIAL AND METHODS: A number of 91 students from 4th year of study were included in the survey. Depression and the strength of motivation were measured using Beck Depression Inventory and Strength of Motivation for Medical School. The obtained data have been processed using SPSS 14 statistical software. RESULTS: A positive correlation between the strength of motivation and physical symptoms was identified, meaning that a higher motivation is associated with a higher number of symptoms experienced by students in stressful moments (r=0.33, p=0.01). Also, women present a higher level of the strength of motivation compared to men [t (89) =0.47; p=0.004, p < 0.05]. A statistically significant difference [t (89) =0.98; p=0.009, p < 0.05] between the average strength of motivation in students who thought about dropping out (M=53.03) and in students who never thought about leaving medical school (M=58.45) was also identified. Perceived depression was also found to predict the number of symptoms felt by students during stressful times. CONCLUSIONS: Students that are mostly motivated in medical school also get stressed the most because they want to have good results.

Romania is ranked first among mortality road traffic accidents in Europe. The aim of our study was to evaluate some statistical patterns of population group who unfortunately was admitted at an emergency hospital of Iasi, a large city in the North East of the country. Material and Methods: The study was performed on 1623 patients admitted in the Emergency Room (“Sf. Spiridon” Emergency Hospital of Iasi, Romania) suffering from road traffic accidents, during 2009 and 2013. Results: The prevalence of 19-45 age group cases represented more than 50% of all patients, with a high significance (p<0.05), and men were involved more frequently (69.69%) than women (30.31%); most of the patients were injured while driving an automobile; 6 PM was the “rush” hour (6.4%), Monday was the “black” week day (16.02% of total cases), September the “harvest” time (12.7%), and Summer the “hottest” season (48.49%). Conclusions: The “B plan” is imperative to be implemented in Romania, including better roads, better vehicles, better legislation, and better teens’ education, in order to avoid road traffic accidents.

THE AIM: The aim of the research is to analyze the social perception of mentally disabled people and of attitudes toward them among teachers, according to certain variables such as subjects' age, gender, type of educatio... THE AIM: The aim of the research is to analyze the social perception of mentally disabled people and of attitudes toward them among teachers, according to certain variables such as subjects' age, gender, type of education and degree of interaction with mentally disabled people, understood as work experience with mentally disabled children. MATERIAL AND METHODS: A number of 120 teachers (44 men and 76 women) answered to a 26 items of Scale of perceptions and attitudes regarding mentally disabled people investigating the following dimensions: the concept of mental disability, the image of mentally disabled people, integration into society, education. Several variables were taken into consideration. Result analysis and interpretation has been performed by means of the specialized psychological statistics software SPSS 23 for Windows. RESULTS: Female teachers (M=17.00) have a significantly more favorable attitude - t (118)=-3.75, p=.001 than men (M=12.25) who work as teachers and younger teachers and those with no experience with children with mental disability have a more favorable attitude toward mentally disabled people than their colleagues. CONCLUSIONS: Age, experience with mental disabled children and gender of teachers are influencing the perception and the attitudes regarding people with disability.

A holistic approach to the human person, from a somatic, psychological and spiritual point of view, with reference to the harmfulness of alcoholism and smoking in cardiovascular pathology, highlights several therapeutic variants over time. Among them are: the current somatic therapy, which could not offer optimal solution in eradicating this vice, and psychiatry which, using will power, could not solve man’s contradiction with himself, caused by a tempting and treacherous irrationality of senses, instinctive-affective desires, turned into vices, as a “Quasi altera natura”, devoid of any bright and creative horizon. As such we propose an alternative that entails changing one’s lifestyle to eliminate the harmful effect of alcoholism and smoking on cardiovascular pathology, through the perspective of spiritual therapy, accessible to contemporary society, at no cost and with no adverse events.

At present, the concept of national security has also acquired a medical dimension, because the political and economic security of a country can be threatened also by the rapidity with which infectious diseases spread in the present global world or by the impact of climatic changes. The present paper intends to illustrate the main threats to/for population health, international organizations and their regulations in the field of medical security, the impact of epidemics on the population, economy and health systems, methods to prevent pandemics, as well as the situations in which the post-communist Romanian health system impacted politically the country image at national and international level. The improvement of the health infrastructure in Romania, motivating the medical personnel by creating better working and payment conditions, as well as the recent experience of other states in the epidemics that occurred in Asia and Africa can all contribute to the successful management of medical emergencies and infectious outbreaks.

XRCC1 polymorphisms may alter the individual’s capacity to repair DNA damages and may increase the risk for developing different types of cancer, including Non-Hodgkin Lymphoma (NHL). The purpose of our study was to investigate the association between XRCC1 Arg194Trp and Arg399Gln polymorphisms and risk of NHL, in a case-control study consisted of 81 patients with NHL and 113 healthy controls. The polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) methods were performed to determine the XRCC1 genotypes. A statistically significant association was observed between the XRCC1 Arg194Trp polymorphism and risk of NHL (p < 0.0001, OR = 7.01, 95% CI = 2.488-19.753) in both male and female. We found no association between NHL and control groups with respect to XRCC1 Arg399Gln polymorphism (p = 0.4867, OR = 1.346, 95% CI = 0.5822-3.111). Also, we did not find differences between the XRCC1 polymorphisms and NHL risk taking into account the IPI score, ECOG performance status, Ann Arbor stage and the treatment outcome (p > 0.05, for all comparisons). Our results suggested an increased risk for NHL regarding the XRCC1 Arg194Trp polymorphism in a Romania population, while XRCC1 Arg399Gln polymorphism is not associated with NHL. Further work is needed to validate these results on a larger sample study.

Chronic Kidney Disease (CKD) is a globally public health problem taking into account its direct role in morbidity and mortality. The essential risk factors that contribute to the development and progression of CKD are hypertension, diabetes and dislipidemy. Besides hypertension, variations in blood pressure caused by baroreflex control dysfunction can lead to chronic kidney disease progression. The causes of impaired baroreflex dysfunction control in chronic kidney disease are not fully known. Understanding the mechanisms that contribute to increased blood pressure variability due to baroreflex dysfunction in chronic kidney disease may represent a key element in diminishing the physio pathological processes that contribute to the development and progression of renal injury.

The current case report comes to show that laparoscopic surgery is the key for persistent organic tumors of the ovary, after a complete diagnostic of the cyst; not the expectant management or hormonal therapy, but laparoscopic cystectomy after transvaginal punction and drainage of the fluid is minimal requiring as recovery timing, medical care period, low costs, and not the least, ovarian functionality, after suppressing ovulatory function.

In the practice of our surgical unit it is the first and only case of synchronous bilateral male breast cancer. The patient was followed up regularly for the last 4 years after a modified radical bilateral mastectomy with axillary lymph node clearance followed by adjuvant chemotherapy and endocrine therapy and he showed no signs of local recurrence or metastatic disease.

We report a rare case of a 71-year-old female who presented to the First Clinic of Surgery from “Sf. Spiridon” Hospital Iasi, Romania with a mass in the left iliac fossa that started to discharge a feculent secretion five days prior to presentation. The patient had a history of constipation, several episodes of hematochezia associated with significant weight loss over the past 6 months. Following evaluation, a diagnosis of enter cutaneous fistula secondary to a locally advanced sigmoid cancer was made. The patient underwent semi-elective resection of the sigmoid tumor (measuring 20 cm) with 10 cm safe margins, and of the involved lymph nodes and abdominal wall. A termino-terminal colo-rectal anastomosis was performed. The patient was discharged the 10th postoperative day after an uneventful evolution for oncological evaluation. The authors presented this case in order to illustrate the challenges associated with diagnostic and therapeutic management of advanced colorectal cancer that has been discovered through a parietal abscess.

Introduction: Acute aortic dissection is the most common cause of death in patients with Marfan syndrome and untreated aortic root enlargement. Emergency surgery for replacement of the ascending aorta has the potential of life saving procedure, but is associated with high morbidity and mortality. Long-term outcomes after surgical repair of acute aortic dissection type A in patients with Marfan syndrome are limited. Material and methods: We made a retrospective study concerning emergency surgical intervention for acute aortic dissection type A, by Bentall procedure, performed in Emergency Institute for Cardiovascular Diseases „Prof dr. C. C. Iliescu” Bucharest between January 2005 and July 2014. We included 332 patients with type a acute aortic dissection divided into two groups: group A - 16 patients with Marfan syndrome and group B - 316 patients with other etiologies. We analyzed differences between these two groups regarding perioperative characteristics, surgical technique and short and long-term morbidity and mortality. Results: The patients from group A were significantly younger than those in group B (35.1±12.7 years vs. 56.8±7.1 years; p<0.001). Arterial hypertension was three times more common in group B as compared to group A (p<0.001). The incidence of postoperative morbidity, and intraoperative and 30-days mortality death rates were similar between groups, but overall mortality at 10 years was lower in group A (31.3%) vs. group B (44.9 %). Conclusions: Emergency surgical in Marfan syndrome, by Bentall procedure could improve perioperative morbidity and mortality of patients with acute aortic dissection, but need an early diagnosis, proper medical therapy and imagistic surveillance.

AIM: Our study had as a major objective the highlighting of more objective criteria in establishing the morphological diagnosis and the evaluation of prognosis elements in endometrial hyperplasia and endometrial carcinoma, representing a specific pathology for the premenopausal and postmenopausal women. Endometrial adenocarcinoma is a malignant tumor, rare in women under 40 years of age, but the incidence increases after menopause, gradually reaching a maximum between 70-79 years. MATERIAL AND METHODS: The study included 291 patients with endometrial adenocarcinoma. The patients were admitted in 2005-2010 to "Cuza-Voda" Obstetrics and Gynecology Hospital in Iasi. The study group was diagnosed and investigated on the basis of the clinical examination and the specialized complementary explorations. The histopathological diagnosis was obtained by the processing of the hysterectomy specimen. RESULTS: The results of the study that we conducted highlight the fact that nearly half of the patients diagnosed with endometrial adenocarcinoma were 50-59 years old. In 78.01% cases the uterus was of normal size, between 4-6cm, in 10.31% cases was increased over 6 cm and 11.68% it was of small size, less than 4 cm. CONCLUSIONS: The study recommends surveillance of cases with endometrial hyperplasia especially if are associated with incriminated risk factors in the etiology of carcinoma of the uterus.

The management of blood loss in primary hip arthroplasty is a controversial topic. Aim: To evaluate the efficacy of reinfusion drains in terms of hemoglobin levels and volume of red blood cell transfused postoperatively. Material and Methods: 295 patients who underwent primary hip arthroplasty were retrospectively assessed. After applying the exclusion criteria, 94 patients were included in the study and were divided into two groups: 45 patients received a reinfusion drain and 49 a suction drain. The following were analyzed: demographic characteristics of patients, preoperative hemoglobin level, 12-h and 24-h postoperative hemoglobin levels and their variations, number of transfused units of packed red blood cells, and postoperative complications. Results: Kruskal Wallis analysis revealed the homogeneity of the study groups (Chi-square=2.40, df=2, p=0.301). A statistically significant lower decline in mean Hb24 was found in suction drain group (p=0.001). Kruskal Wallis test revealed a significantly more frequent postoperative use of a higher number of packed red blood cell units in the suction drain group (Chi-square=28.70, df=2, p=0.001) compared to reinfusion drain group. Conclusions: We failed to demonstrate the superiority of reinfusion drains versus suction drains in maintaining hemoglobin levels.

Vaginal hysterectomy was mentioned before our era by Soranus of Ephesus, and it is one of the most frequent surgical interventions in gynecologic practice; performed for the first time by Sauter of Constance, in 1822, who practiced a vaginal hysterectomy without ligation of vessels, carrying out the hemostasis with a swab dipped in alum. The essential argument for vaginal surgery is the advantage it brings. Current indications and contraindications encourage the frequent use of vaginal hysterectomy in benign pathology of the uterus, while the indication (Crossen, Rouhier, Campbell techniques) depends most of the times on the surgeon's experience and preference rather than on the critic evaluation of results. Material and methods: The study group included 84 patients, hospitalized from January 2013 to December 2015 in the Third Obstetrics Clinic. For the evaluation of results, we used longitudinal retrospective clinical-statistical method. Results and discussion: Practicing the surgery on vaginal route is mainly indicated in patients with severe anemias and different organic tares. When removal of the cervix is not indicated, or when or when there is an enlarged uterus or one with a small diameter, it is recommended to use laparoscopic hysterectomy. Conclusions: Vaginal hysterectomy is a technique that allows rapid removal of the uterus, with a minimal impact on the patient, indicated in the pathology benign uterine tumors (uterine fibromatosis associated with metrorrhagia, fibromatous uterus with cervical dysplasia, or fibromatous uterus associated with different degr4ees of uterine prolapse).

Efficiency of Treatment for Female SUI (Stress Urinary Incontinence) Using the TVT-O® (Tension-Free Vaginal Tape Obturator) Technique.

AIMS: The modern Tension-Free Vaginal Tape Obturator (TVT-O®) technique is used worldwide to treat Stress Urinary Incontinence (SUI). The subject of the paper is timely and with a major practical potential. The paper presents an ample retrospective study on a series of patients hospitalized at the Second Clinic of Obstetrics-Gynecology of "Cuza-Vodă" Obstetrics and Gynecology Clinical Hospital from Iasi, for a period of 3 years. In the period January 2010-January 2012, 164 female patients were subjected to surgical procedures to treat type II SUI, inserting TVT-O® tapes. MATERIAL AND METHODS: Patients were selected based on anamnesis, clinical examination and positive Bonney's test. When genital prolapse was associated with SUI, simultaneous with strip insertion, other interventions were practiced, too. Cases were followed in the immediate postoperative period, at 2 months, 6 months, and 1 year, in order to analyze the efficiency of the method. 164 female patients aged between 38 and 65 were included in the study, of which 60 cases had isolated SUI and in 104 of the cases SUI was associated with genital prolapse. RESULTS: In 85% of the cases, the healing rate was satisfactory, and in 15% of the cases, it was partially satisfactory, keeping the percentage at 6 months and 1 year. In respect of complications at 16 female patients, SUI was improved, but persistent in 14 cases the mictional necessity was described as absent before intervention; in these cases, we used oxybutynin, in 4 of them-tape sectioning and in 4 cases the exteriorization of the tape was relieved after 2 months, performing the secondary vaginal suture. The simple operatory technique, the reduced hospitalization period, the short convalescence and the immediate results support the TVT-O® procedure in the treatment of SUI. Postoperative complications are rare and do not compromise results. The main goal of "Sling" interventions is the creation of a reliable closure mechanism, supporting the passive urinary retention by urethral compression. CONCLUSIONS: Our point of view with regard to the controversy of using TVT-O® tapes is that the technique does not have to be abandoned, being extremely efficient in academic centers with ultra-specialized staff.

Most of the studies showed that IBD patients inflammatory bowel diseases (IBD) with CDI have more of the whole range of short- and long-term worst outcomes than those without CDI. Initial infection with the BI/NAP1/027 epidemic clone was found to be a significant risk factor for relapse. However, current literature is suggesting increasingly that for patients with infections that fail to resolve with traditional antibiotic regimens, FMAT's average cure rate of >90%. We report a case of a 40-year-old man, diagnosed with ulcerative colitis (UC) in 2012 who presented in our clinic for 20 watery stools per day with mucus and blood, hypogastric pain, pyrexia and chills. Rectosigmoidoscopy and histopathological examination diagnosed a ctive lesions of ulcerative colitis with Clostridium difficile toxins A/B enzyme immunoassays (EIA) testing initially negative. The patient was non-responder at day 10 of intravenous (iv) corticotherapy and received induction therapy with Infliximab 5 mg/kg. EIA testing for Clostridium difficile was repeated at day 12 of hospitalization with positive results for toxins A/B, and associated oral therapy with Vancomycin and Metronidazole was initiated without clinical response in day 7, reasons for what intravenously therapy with Tigecycline was started with good response. Patient was discharged after 10 days of Tigecycline, but came back twice for two relapses of Clostridium difficile colitis treated successfully with Tigecycline, reasons for what fecal transplantation was performed in Matei Bals Institute, which induced remission of both CDI and UC.
