BACKGROUND: At present, the relevance of insulin-like growth factors in the progression of gastric cancer (GC) remains elusive. We conducted a cohort study to assess the influence of the IGF proteins on GC. METHODS: We investigated the serum and tissue expression of IGF proteins in patients with GC to determine their biomarker potential. In Cohort 1, the serum levels of IGF1,2, and IGFBP1-3 were measured for survival analysis (N = 142). IGF1,2, IGF-IR,IIR, and IGFBP1-3 in cancer tissues was assessed via immunohistochemistry (N = 28). In Cohort 2 (N = 255), serum IGFBP2 was measured 115 patients diagnosed with gastritis as a control group. In prognostic Cohort 3 (N = 397), IGFBP2 expression was quantitatively assessed in GC tissues. RESULTS: Higher serum IGFBP2 in Cohorts 1 and 2 was associated with worse overall survival (Cohort 1, p < 0.0001; Cohort 2, p < 0.00001). In Cohort 1, IGFBP2 exhibited the highest success staining rate among IGF proteins in GC tissue (92.8%, 26/28; p = 0.006). Cox regression analysis confirmed that higher serum IGFBP2 predicted a worse prognosis (HR: 1.90; p = 0.0011). The serum IGFBP2 levels in GC patients were higher than in the gastritis group (p = 0.0001). Cox regression analysis in Cohort 3 suggested that score 2 and score 1 IGFBP2 expression were independent predictors of poor prognosis (HR: 1.74 for Score 2, p = 0.0079; HR: 1.49 for Score 1, p = 0.039). CONCLUSIONS: IGFBP2, but not other IGF proteins, are associated with GC prognosis.

Outcome specificity may influence the causal interpretation of ceramic versus metal head hemiarthroplasty studies.

METHODS: A retrospective analysis of 95... OBJECTIVE: To investigate risk factors for postoperative acute kidney injury (AKI) in elderly patients undergoing abdominal surgery, and to develop and validate a prediction model. METHODS: A retrospective analysis of 954 elderly abdominal surgery patients was conducted. AKI was defined as serum creatinine increase ≥26.5 μmol/L within 48 h or ≥1.5× baseline within 7 days post-surgery. Independent risk factors were identified by multivariate logistic regression, and a nomogram was constructed. Model performance was assessed using ROC curve (AUC), calibration curve, DCA, and learning curve. RESULTS: The incidence of postoperative AKI was 11.43%. Independent risk factors included age, aCCI≥4, diabetes, hypertension, anemia, hypoproteinemia, low serum albumin and hemoglobin, and pneumoperitoneum duration. The AUCs for the training, validation, and test sets were 0.872, 0.871, and 0.927, respectively. The calibration curve showed good fit, DCA indicated net clinical benefit, and the learning curve confirmed adequate sample size. CONCLUSION: The nomogram prediction model for postoperative AKI in elderly abdominal surgery patients exhibits good discrimination, accuracy, and clinical utility, supporting early identification of high-risk individuals and perioperative renal protection.

Serum angiopoietin-like protein 4 as a predictor of hemorrhagic transformation post-endovascular therapy in acute ischemic stroke: A prospective study.

BACKGROUND AND PURPOSE: Hemorrhagic transformation (HT) is a major complication after endovascular therapy (EVT) for acute ischemic stroke (AIS), closely associated with blood-brain barrier (BBB) disruption. Angiopoietin-like protein 4 (ANGPTL4) has been implicated in BBB regulation. This study aimed to evaluate ANGPTL4's predictive value in EVT-treated AIS. METHODS: In this prospective study, 122 AIS patients undergoing EVT were enrolled. Serum ANGPTL4 levels were measured before treatment. Patients were categorized into non-HT, hemorrhagic infarction (HI), and parenchymal hematoma (PH) groups based on follow-up CT. Baseline clinical, imaging, and laboratory data were collected. Logistic regression identified independent HT predictors. BBB permeability was estimated using the relative permeability-surface area product (rPS) from CT perfusion. Receiver operating characteristic curves assessed ANGPTL4's predictive value. RESULTS: HT occurred in 48 patients (39.3%), including 33 with HI and 15 with PH. Serum ANGPTL4 levels were higher in HT vs. non-HT patients (median [IQR]: 1315.05 [798.30-1997.85] vs. 826.50 [569.02-1246.35] pg/mL, p < 0.001) and showed a stepwise increase across HT subtypes (p < 0.001). Serum ANGPTL4 levels were independently associated with HT (OR per 100 pg/mL = 1.11, 95% CI: 1.03-1.19, p = 0.005) and positively linearly correlated with rPS (β = 0.02, p < 0.001) in fully adjusted models. ROC analysis showed moderate predictive accuracy for PH (AUC = 0.790). CONCLUSIONS: Elevated serum ANGPTL4 levels were independently associated with HT after EVT in AIS and were correlated with BBB permeability, suggesting a potential link between ANGPTL4 and BBB dysfunction.
