Patients will comm...BACKGROUND: Retinitis pigmentosa (RP) is a set of inherited rod-cone degenerative diseases that clinically presents with similar signs and symptoms. Mutations in one of more than 70 genes are involved. Patients will commonly present with bone-spicule pigment formation, waxy optic nerve pallor, and attenuated blood vessels in the posterior pole.Symptoms often begin with progressive night blindness, mid-peripheral visual field defects, and eventual tunnel vision. Central vision loss will ultimately occur following loss of rod function. Complete blindness is uncommon. OBJECTIVE: The aim of this article is to present two cases of retinitis pigmentosa (mother and daughter) trough optalmologic exams in our clinic. The next aim it to show how to menage a low vision service and to treat cystoid macular oedema as a complication of retinitis pigmentosa. METHODS: All medical reports are shown in this article. Every diagnostic tool as well as report is a part from our archived history of the patients and has been throughly analysed. We also reviewed available literature using the key words retinitis pigmentosa, cystoid macular oedema, gene therapy. CASE PRESENTATION: A 38 year old female patient for a low vision consultation. The patient was legally blind secondary to retinitis pigmentosa, which was diagnosed in her late 20s. She reported gradually progressive hazy central vision and decreasing peripheral vision in both eyes as well as severe night blindness. Other than the diagnosis of retinitis pigmentosa in both eyes,the patient had no other remarkable ocular conditions. Findings at that visit included unaided distance visual acuities VOD: 0,04 VOS: 0,06. Pupils were round with brisk responses. Extraocular muscle motility was full in both eyes. Confrontation methode visual fields were noted as temporal loss in the right eye and superior and temporal loss in the left eye. The perimetry test could not be performed due to the lack of correspondece of the patient even after a couple repetitions of the perimetry. She had normal ocular adnexa and quiet lids, conjunctiva, and sclera in both eyes. Corneas in both eyes were noted as clear epithelium, clear stroma, and clear endothelium. Anterior chambers had normal depth, iris with no pathological findings in both eyes; lens incipient sclerotic. Intraocular pressures were noted as 22 mmHg in both eyes with Icare, 21mmHg and 19 mmHg with aplanation tonometry; pahimetry corretional factor was +1 on both eyes. The vitreous was clear in both eyes. Both optic nerves were measured as 0.4 cup-to-disc ratios with no disc edema, disc hemorrhages, notching, or thinning noted.Waxy disc pallor and attenuated blood vessels were observed in both eyes. The macula in both eyes had retinal pigment epithelium (RPE) changes with no edema or hemorrhages. Bone spicule changes were noted 360 in the periphery of both eyes with no holes or tears(Figure 1a+1b+1c+1d). CONCLUSION: We presented two cases of retinitis pigmentosa - the mother with diagnosed RP more than 15 years ago in need for low vision rehabilitation service and the daughter that got diagnosed after our initial examination and with complications in visual impairment through cystoid macular oedema.

OBJECTIVE: To find a new surveillance model of tuberculosis transmission control based on geographic information system data in improving the tuberculosis transmission control and treatment outcome. METHODS: Effectiveness test of the self-monitoring of calendar documentation on the tuberculosis transmission control and outcome treatment with quasi-experimental post-test only with control group design. The analytical unit consisted of 96 tuberculosis cases as the self-monitoring group of calendar documentation (intervention) and 87 tuberculosis cases as the control group sourced of two different primary health care. RESULTS: The self-monitoring intervention of calendar documentation increased the average treatment effect on medicines intake control 0.11 (95% CI: 0.01-0.21), environment control 0.32 (95% CI: 0.19-46), droplets nuclei control 0.49 (95% CI: 0.36-0.61), cured 0.22 (95% CI: 0.09-0.36), completed 0.18 (95% CI: 0.09-0.26), drop out 0.09 (95% CI: 0.01-0.16), and failure of treatment 0.18 (95% CI: 0.09-0.26) of the control group (baseline). CONCLUSION: The final result of this research found a new surveillance model of tuberculosis transmission control in google earth mapping aplication based on Geographic Information system.

This is not...In the hilly Balkans, a folk proverb has been circulating for a long time, "It is most difficult to be a prophet in one's own village", which reflects the age-old mistrust of the population towards new ideas. This is not surprising in the least, because since the written history of the peoples of the Balkans has existed, a continuous series of conquerors and local rulers who subjugated the common folks and imposed their world view can be traced. Nevertheless, from time to time, people with great strength appear who not only break the shackles imposed by the powerful, but through their actions find a way to the souls of their compatriots and gain their unreserved trust. One of such spontaneous creators is professor Izet Mašić, who achieved a miracle of medical publishing in his Sarajevo and Bosnia and Herzegovina and traced the path of medical science. There may be thousands of medical journals in the world, more or less reputable, and researchers from the Balkans can publish their work in them, but only domestic medical journals can initiate and direct domestic medical research, and educate young researchers in the right way. Professor Mašić made it possible for authors from Bosnia and Herzegovina and other Balkan countries to present their results to the world and receive an incentive from impartial experts to continue their work and progress more and more by editing and publishing three domestic journals at once, which are visible in the most important world bases,. The progress in research then translated into improving medical practice and health care of the population. The following details from Professor Mašić's biography tell us how this miracle happened. This year, academician Izet Mašić, Editor-in-Chief of a few biomedical journals, including Acta Informatica Medica journal, celebrates his 70th birthday and also 45 years of his academic and scientific work.

BACKGROUND: Medical professionals (doctors and other medical staff) in the field of healthcare everyday must make calculated decisions which have important consequences, impacting patients on the individual level, local...BACKGROUND: Medical professionals (doctors and other medical staff) in the field of healthcare everyday must make calculated decisions which have important consequences, impacting patients on the individual level, local (community), national or global level. Healthcare professionals must at times make these choices with limited information, resources, and knowledge, and yet is is expected that these decisions are highly calculated and accurate. It is important to familiarise oneself with the exact definitions regarding medical decision making. OBJECTIVE: The aim of this study was to describe application of the most important rules to help decision makers to be good or excellent decision makers in medical practice at every level of health care system. METHODS: The author used descriptive method of explanation teoretical and practical issues regarding application of od decision making processes in the praxis, based on searchied scientific literature about this topic deposited in online databases. RESULTS AND DISCUSSION: The author of this paper discussed about important topics: a) the importance of medical decision in emergency situations; b) the varies of decision making with solving problems by medical professionals; c) the limitations when it comes to medical decison making; and d) what doctors need to follow regarding decision making in the praxis. Two factors that have influenced to the decision process: a) degree of uncertainty about future events; b) usefulness of outcomes in any particular case. The clinical decision problem analysis process demands: a) explicit formalization of a decision making problem or the description of the medical problem decision with a registration of all possible actions which have to be undertaken and registration of all the possible so determined outcomes. b) construction of the decision tree which presents all described actions and outcomes with predictions of the probabilities and the choice of the most optimal action based on the probability outcome and its use. Doing this allows us to delve deeper into more intricate options present within medical decision making. Simple put, a decision is a choice between two options. The person or entity conducting that decision is the decision maker. The exact definition is "Under the decision should imply some specific action which is selected from several variables or which satisfies the expectation that is previously set".Many different factors and individuals may be involved in medical decision making, with varying consequences, according to different players and settings. CONCLUSION: A vital component of medical decision making is evaluation. Decision makers must concisely evaluate situations, in order to make better choices. For example, when examining a health care system, their decisions should consider the following questions, such as, what is the health status of the given population? What economic resources are at the disposal of our patients, and government? How effective is the current healthcare model that is already in place? Does the existing social system pay enough attention to the healthcare protection? Does the organisation structure of the healthcare system satisfy? Are the existing practice and the healthcare technologies secure, effective, and suitable? Are the planning, programming, determination and the choice of priority the adequate to the needs of people? How are the monitoring and evaluation of healthcare system quality organised? These are a few examples of evaluation in medical decision making.

OBJECTIVE: Health Informatics is an indispensable science in light of the massive reform healthcare has undertaken recently. METHODS: This descriptive multiphase study used a questionnaire to assess the need and applied a competency-based postgraduate curriculum to collect the required competencies as well as their proficiency levels. RESULTS: Eighty percent of the respondents agreed that the current training should be advanced. 73.7% of the respondents agreed that this certificate would improve the level of knowledge, and it would impact on the delivery of healthcare positively (78.9%). For the required competencies, 10 competencies scored 1.75 and above, 22 scored between 1.5 and 1.75 and 22 scored below 1.50. The expert panel came to an agreement that the competencies with 1.5 and above should be included. DISCUSSION: The findings emphasize the need to establish PG diploma in comply with previous studies that unveiled the need to provide education and training in Health Informatics. Only the required competencies were included as the program is one-year long. The selected competencies were validated and presented by the panel. CONCLUSION: A PG diploma in Health Informatics is a key element in the evolution of healthcare services, therefore, a competency-based framework was provided and validated by experts.

Diabetes mellitus has been known to be associated with lipid metabolic disorders and macrovascular and microvascular complications. OBJECTIVE: This study assesses the correlation between cardio risk index and metabolic markers in type 2 diabetes mellitus patients. METHODS: This study was conducted on type 2 diabetes mellitus with the cross-sectional analytic method. The inclusion criteria of the samples were all the patients diagnosed with type 2 diabetes mellitus of both sexes. We recorded Body Mass Index, Blood Pressure, disease duration, and family history. The laboratory parameters, including Fasting Blood Sugar, Hba1c, HDL, LDL, TG, and Cholesterol, were examined by Paramita Laboratory Clinic. The data of the samples were processed using a computer with the SPSS program.. RESULTS: There were significantly different mean of age, abdominal circumference, FBG, Hba1c, cardio risk index, and HDL controlled and uncontrolled type 2 diabetes groups, but the mean cholesterol and LDL levels were not different in the two groups.At uncontrolled type 2 diabetic we found that there was a significant correlation between the cardio risk index with Cholesterol (r= 0.3 p=0,004), triglycerides (r=0.5 p=0,001), and negative correlation cardio risk index with HDL (r= -0.5 p=0.012). There was no significant correlation between the cardio risk index with LDL (r=0.157 p=0.101). CONCLUSION: Hba1c and FBS are higher in uncontrolled type 2 diabetes accompanied by lipid profile and cardio risk index levels that are higher than expected values are the risk of complications in type 2 diabetes mellitus.

Its quantification can help to better understand...BACKGROUND: Coronary New blood in the vascular bed after Coronary Artery Bypass Grafting (CABG) may represent a turning point between ischemia and normal tissue nutrition. Its quantification can help to better understand coronary artery hemodynamics after revascularization. OBJECTIVE: Quantification of coronary sinus blood flow changes over time after Coronary Artery Bypass Grafting (CABG) using Transthoracic Echocardiography (TTE). METHODS: Prospective basic research, with repeated measurements on hospital sample of 61 patients whom CABG was conducted. We performed TTE recordings to measure CS flow before and two times after CABG (1 and 6 postoperative day). We measure CS diameter, Velocity Time Integral (VTI) and systemic hemodynamic data. Data needed for LV mass calculation were recorded once. During statistical analysis we define: α = 0,01, β = 0,01 (power = 1-β β= 0,99), Sample size = 60, Effect size= 0,68. We used ANOVA for Repeated Measures as main statistical test in SPSS. RESULTS: Preoperatively we found low overall CS flow of 181 ±72 ml/min (0,68 ±0,30 ml/gram-LV/min). After surgery there was constant increase of CS flow from 276 ±79 ml/min (1,13 ±0,35 ml/gram-LV/min) first postoperative day, to 355 (±99) ml/min (1,30 ±0,46 ml/gram-LV/min) sixth postoperative day. DISCUSSION: Amount of new blood was statistically significant after CABG with P<0,001. Same result was found after classifying patients per number of graft received, with the highest amount of new blood after four bypasses. Amount of new blood was not different if patient gets two or three bypasses. CONCLUSION: There was significantly new amount of blood in coronary bed after CABG, with constant increase over first 6 days.

There are several risk factors for increasing violence, of which personality traits represent the most substantial risk. OBJECTIVE: the current research aims to examine the association among the 'big five' risk factors and involvement in violence among high school students in the . METHODS: The sample comprised 468 students, 242 boys, and 226 girls aged 16 to 20. Two self-report instruments were used for data collection. Firstly, we divided the Maudsly Violence Questionnaire into two factors: acceptance of violence and machismo. Secondly, we used the Big Five Inventory, which includes questions on extraversion, neuroticism, agreeableness, consciousness, and openness. RESULT: the result showed a significant positive correlation between extraversion, neuroticism, and violence involvement. Agreeableness, openness, and consciousness were negatively correlated. In terms of demographic variables, the findings suggest that boys are more likely to be involved in violence than girls. The results indicated that personality traits neuroticism, extraversion, agreeableness, openness, and consciousness predicted violence. CONCLUSION: personality traits significantly impact individuals' behaviors that reflect their lives. The potential for violence is a predictor for detecting and preventing violent actions and could be used in many justices and governmental systems.

COVID-19 was recognized in December 2019. It was quickly determined that a novel COVID-19 virus, which is structurally linked to the virus that causes the severe acute respiratory syndrome, was to cause (SARS). OBJECTIVE: The aim of this study is to investigate the presence of effect between the rs179008 (A/T) SNP polymorphism in TLR7 gene and blood group on the severity of COVID-19. METHODS: The study included 90 patients divided into three groups mild, moderate, severe, and experimental research work was conducted during the period of sample collection extended from November 2021 to February, PCR-RFLP technique was used to determine SNP rs179008 polymorphism in TLR7 in the blood. RESULTS: A present study found non-significant differences between patient groups for TLR7 rs179008 (A, T) allele were (p=0.79152) for mild to moderate and severe, (p=0.84872) for mild and moderate and (p=0.58741) for mild and severe. When comparison (AA, AT, TT) genotypes in three groups found a significant difference between mild and moderate groups (p=0.036) for the AA genotype. Found (A blood group) more frequency than other groups but observes no significant difference between patients' group. CONCLUSION: We conclude that the (AA) genotype for TLR7 rs179008 polymorphism was a risk factor and effect on severity of COVID-19 infection, so (AA) can consider an independent risk factor for development of COVID-19.

Mobile health applications for self-care have the potential to help patients living with chronic conditions such as kidney disease, and can be used to manage aspects such as the consumption of substances that are harmful to health. Chronic kidney disease causes significant morbidity throughout Panama, and is also responsible for an increase in cardiovascular disease. OBJECTIVE: In this paper, we present a review of the applications offered by the Android store, based on a search and selection of the most efficient options that fulfill a set of criteria and functionalities. METHODS: We evaluate a big health data model in terms of its usefulness for studies, research and projections of Panamanian patients with this chronic disease. RESULTS AND DISCUSION: We present a mobile application based on the most important standards and functionalities for the Panamanian population affected by this disease. Our analysis also highlights the importance of mobile applications for the self-care of chronic diseases and their usefulness to both patients and health care providers, since they can support better health habits and give good results in terms of following a diet, promoting a healthy lifestyle, and encouraging physical activity. The analysis presented here will form the basis for the development of an application that will be simple, user-friendly and powerful, in the sense that it will empower patients with the resources they need for self-care. . CONCLUSION: Mobile applications are found to show promise for the self-care of chronic conditions, and can improve the quality of life of Panamanian patients. In addition, we intend to develop a data repository for scientific research within Central America.

AIM: To compare the hearing of university students who use personal listening devices (PLDs) at extremes (upper & lower quartiles) with respect to a) Duration of use/day b) The number of years since use c) Volume level. METHODS: This cross-sectional study was conducted in the Department of Physiology, College of Medicine, at Imam Abdulrahman bin Faisal University (IAU), from September 2021 to April 2022. An online survey was distributed among IAU medical students on the subjects' patterns of earphone use. Forty participants volunteered to participate in the hearing tests: Pure Tone Audiometry (PTA), Tympanometry, and Transient Evoked Otoacoustic Emission (TEOAEs) at the ENT clinic of the King Fahd Hospital of the University (KFHU). RESULTS: The pure tone audiometry and TEOAE showed no significant differences in the hearing thresholds/levels of participants in upper quartiles vs lower quartiles of both ears, in any of the PLD use pattern categories. However, in the TEOAE two observation were detected: a) Significantly lower amplitude in the upper quartile of the category "duration of PLD use/day" at a frequency of 1.0 kHz in the left ear (p-value 0.04) b) Significantly lower amplitude in the upper quartile of the category "volume of PLD use" at a frequency of 1.0 kHz in the left ear (p-value 0.03). CONCLUSION: The present findings suggest that hearing threshold/level and cochlear outer hair cells' functions do not differ significantly in the participants belonging to the upper and lower quartiles of PLD use pattern (volume level, duration of use/day, and the number of years since use).

Coronary artery aneurysms or ectasia develop in approximately 15 to 25% of untreated children with the disease and may lead to myocardial infarction, sudden death, or ischemic heart disease. Despite an overlap of clinical features with toxic shock syndrome, children with Kawasaki disease generally do not develop shock. OBJECTIVE: Here we present two adolescent boys who had KDSS and discuss their differentiating features from MIS-C. CASE REPORTS: Two adolescent children presented with a toxic shock-like illness, and were subsequently diagnosed with Kawasaki disease shock syndrome when coronary artery abnormalities were found on transthoracic echocardiography. CONCLUSION: Pediatricians and paediatric cardiologists alike should be aware of this potentially severe form of manifestation of the Kawasaki disease which needs to be differentiated from the multisystem inflammatory syndrome in children.

Taking into account recent situation with COVID-19 pandemics, digital technologies allowed to improve management of viral infections via remote monitoring and diagnostics of infected patients. Up to date, various mobile health applications (apps) have been proposed, including apps for the patients diagnosed with cardiovascular pathologies. OBJECTIVE: The presented review aimed at the analyses of a range of mHealth solutions used to improve primary cardiac care. In addition, we studied the factors driving and hindering the wide introduction of mHealth services in the clinics. METHODS: The work was based on the guidelines of the preferred reporting items for systematic reviews and meta-analyses (PRISMA) statement. The publication search was carried out using PubMed, Web of Science, Cochrane Library, Scopus, and Google Scholar databases. Studies published during the period from 2014 until January 2022 were selected for the analysis. The evaluation of risk of bias for the included studies was conducted using the Cochrane Collaboration Risk of Bias tool. RESULTS AND DISCUSSION: An overall 5513 studies were assessed for eligibility after which 39 studies were included.. The main trend in the mobile health for cardiological applications is the use of different types of wearable devices and Artificial Intelligence-platforms. In fact, mobile technology allows remotely to monitor, interpret, and analyze biomedical data collected from the patient. CONCLUSION: The results of this literature search demonstrated that patients diagnosed with cardiovascular disorders can potentially benefit from the application of mHealth in cardiology. However, despite the proven advantages of mHealth for cardiology, there are many challenges and concerns regarding effectiveness, safety, reliability and the lack of official regulation and guidelines from official organizations. Such issues require solutions and further work towards a wide implementation of mHealth technologies in cardiac practice.

Researchers and scientists have been searching for a novel genetic marker to detect or recognize the genetic malfunction that causes sperm abnormalities. Micro-RNA (miRNAs) are small non-coded RNA molecules that present intra and extra-cellular and regulate gene expression. OBJECTIVE: This studies began to search for a relation between miRNA expression levels and other diseases that may be related to them, considering that the main role of miRNAs was the down-regulation of genes. METHODS: The main technique used in this study was to synthesize a complementary DNA (cDNA) (revers transcription method) of extracted total RNA by TRIzol then amplification of candidates' miRNAs genes by Reverse Transcriptase Quantitative Polymerase Chain Reaction RT-qPCR. RESULTS: Studies found that miRNAs have a role in defining sperm qualities such as sperm count, motility, and shape. In this study, we chose the most miRNAs referred to in the previous study as a potential seminal fluid marker (miR-10a, miR-10b, miR-135a and miR-135b) to test them as potential infertility-related miRNAs markers (Asthenospermia AS, Oligospermia OS, Astheno-Oligospermia ASOS) in addition to health normal sperm NS. CONCLUSION: the main aim of this study was to find the miRNAs expression pattern to find a way to help scientists track the genetic causes of male infertility issues and a novel method to distinguish infertility genetically diseases. CONCLUSION: The findings may serve as a potential genetic marker for male infertility and provide a background for future research that targeted miRNAs as a molecular marker for medical and forensic fields, also as an infertility disease potential treatment.

The exact mechanism by w...BACKGROUND: Angiotensin-converting enzyme 2 (ACE2) is not only an enzyme but also a functional receptor on cell surfaces through which Severe acute respiratory syndrome coronavirus 2 (SARS-CoV2). The exact mechanism by which arterial hypertension (particularly regulated) could affect the presentation and outcome of Coronavirus disease-19 (COVID-19) has not been fully elucidated. OBJECTIVE: The aim of this study was to analyze the parameters of patients with verified COVID-19 and existing arterial hypertension at the time of hospital admission and to develop neural network model. METHODS: The research had a cross-sectional descriptive and analytical character, and included patients (n=634) who were hospitalized in the General Hospital "Prim. dr. Abdulah Nakas" in Sarajevo, Bosnia and Herzegovina, in the period from 01 Sep 2020 to 01 May 2021. From the hospital information system, which is used in everyday clinical work, laboratory parameters at admission were verified, along with demographic data, the comorbidities, while the outcome (recovery, death) was recorded thirty days after the admission. RESULTS: Out of the total number, in 314 patients (200 males), arterial hypertension was verified, out of which, 56 (17.83%) patients died. Patients were divided into two groups, according to outcome, i.e., whether they survived COVID-19 infection or not. A significant difference in age (p = 0.00), erythrocyte count (p = 0.03), haemoglobin (p = 0.05), hematocrit (p = 0.03), platelets count (p = 0.00), leukocytes (p = 0.01), neutrophils (p = 0.00), lymphocytes (p = 0.00), monocytes (p = 0.00), basophils (p = 0.00), eosinophils (p = 0.00), C-reactive protein (p = 0.00) and D-dimer (p = 0.01) was noted. When patients who died and had hypertension were compared with those who died and did not have hypertension (n = 15), out of alll the analyzed parameters, the only significant difference was established in the patient's age (p = 0.00). In case when patients with hypertension who died were compared to patients with hypertension and diabetes mellitus who died no significant differences were found between features. CONCLUSION: Patients with hypertension and COVID-19 who died were older, had higher values of erythrocytes, hemoglobin, hematocrit, leukocytes, neutrophils, CRP and D-dimer, and lower values of platelets, lymphocytes, monocytes, basophils and eosinophils count at admission. Compared to deaths without hypertension, the only difference that was established was that patients with hypertension were older.

OBJECTIVE: The present study reviewed the ef...BACKGROUND: Computerized Provider Order Entry (CPOE) systems developed based on clinical guidelines are believed to greatly reduce chemotherapy medication prescription errors. OBJECTIVE: The present study reviewed the effects of guideline-based CPOEs on the chemotherapy order process. METHODS: PubMed, Scopus, Embase, Web of Science, and IEEE Xplore databases published up to 1 June 2020 were systematically searched for studies investigating the effect of guideline-based CPOEs on the chemotherapy order process. Moreover, the bibliography of relevant retrieved publications was also checked. RESULTS: Nineteen articles from the five databases met the eligibility criteria and were reviewed. Eleven out of 19 (58%) articles investigated the effect of CPOEs on medication errors, and other studies examined other aspects of CPOE efficacy, including time required for chemotherapy prescriptions; Safety, policy compliance and communication between health care providers; physicians prescribing behavior; quality and safety of treatment; workflow; direct patient care time; and adherence to guidelines. In addition, 15 out of 19 mentioned the use of specific clinical guidelines. CONCLUSION: Evidence indicates CPOEs can positively affect the quality of healthcare service delivery for cancer patients, but there is still a dearth of clinical outcome evaluation data about the effects of these systems on patients undergoing chemotherapy. Moreover, there is limited information about guideline compliance errors, which highlights the needs for further research in this area.

The aim of a scientometric study is to study the various citation-based metrics of scientific articles, such as parameters pertaining to authors (including institutions and country of origin), articles, journals, and other citation related metrics. OBJECTIVE: In this second part of our scientometric analysis of the 6 major neurosurgical journals from 2011-2020, we study journal and author trends, yearly publication trends, and citation related metrics in , , , , , and METHODS: We analyzed parameters, including article and journal metrics (total articles published per journal per year, breakdown of the Bradford's law distribution of journals, and Lotka's law, journal impact factors), author metrics (country of origin, collaborations), citation totals, and keyword counts. RESULTS AND DISCUSSION: The highest number of published articles from all journals occurred in 2011, the lowest in 2016. published the most. The author collaboration index has declined since 2016 with a notable drop in 2020 when the COVID-19 pandemic began. Impact factors remained stable, except for Operative Neurosurgery, which experienced a steep decline in 2020, and , which experienced a mild decline in 2020. Canadian authors were the most likely to participate in multi-country collaborations. CONCLUSION: The most articles were published in , followed by , , and .

It includes the lack the information on the impact of digital technologie...BACKGROUND: An assessment of the level of adherence of patients diagnosed with hypertension to therapy has not been conducted in Central Asia yet. It includes the lack the information on the impact of digital technologies on hypertension management. OBJECTIVE: The study aimed at the evaluation of the effect of using a mobile application on adherence to therapy in patients with diagnosed arterial hypertension. METHODS: A multi-centre randomized controlled study was conducted on 517 patients with diagnosed arterial hypertension (in the out-patient clinic, Almaty, Kazakhstan). Patients were randomly assigned to intervention (IG) and control (CG) groups. IG patients used a mobile application, which allowed creating an individual schedule for taking medications, including visual and audio notifications. The assessment of adherence to antihypertensive treatment was carried out using the Lebanese Medication Adherence Scale-14 (LMAS-14), consisting of 14 items. Data were collected before therapy (T1), 3 months (T2), 6 months (T3) and 12 months (T4) followed the first dose. Adherence was assessed on 101 patients in the CG group and 165 respondents in the IG group. RESULTS: In the period T1 in both groups CG and IG, a moderate adherence to treatment was recorded with indices equal to 35.9 ± 1.2 and 35.9 ± 1.3, respectively (p ≤ 0.05). A decrease in adherence in the CG group was determined after 3 months (35.9 ± 1.2) (p ≤ 0.05) and six months later (36.5 ± 3.2) in comparison with relatively high adherence among respondents in the IG group in periods T2 (39.5 ± 1.2) and T3 (40.5 ± 1.2) (p ≤ 0.001). After 12 months (T4) on the adherence scale in patients who used the mobile application "MyTherapy" (IG), despite a slight decrease in scores, the results were relatively higher (40.3 ± 1.3) compared with the CG group (33.6 ± 1.9) (p ≤ 0.001). CONCLUSION: The analysis of adherence of patients with primary health care in Almaty (Kazakhstan) with chronic arterial hypertension showed the effectiveness of using the 'MyTherapy' mobile application in increasing patient adherence. The obtained data on the positive effect of the use of digital technologies require further research to assess the possibility of wider implementation in healthcare.

Stu...BACKGROUND: Acute left ventricular free wall rupture (LVFWR) is a life-threatening complication of myocardial infarction that requires urgent intervention. Surgical repair has continued to be the treatment of choice. Studies suggest a posterolateral or inferior infarction is more likely to result in free wall rupture than an anterior infarction. LVFWR generally results in death within minutes of the onset of recurrent chest pain, and on average was associated with a median survival time of 8 hours. Prompt diagnosis and management can lead to successful treatment for LVFWR. OBJECTIVE: The aim of this article was to present an emergency case with an LVFWR in a COVID-19 patient who suffers from AMI and was treated with PCI stents in the ramus intermedius and circumflex coronary artery. CASE REPORT: We present an emergency case with an LVFWR in a COVID-19 patient who suffers from AMI and was treated with PCI stents in the ramus intermedius and circumflex coronary artery. Although dual antiplatelet therapy introduction and good outcome of PCI were achieved, soon after instant thrombosis of both stents appear to result in transmural necrosis and LVFWR. Urgent catheterization was performed and diagnosed in-stent thrombosis where the ventriculography confirmed LVFWR of the posteroinferior wall. Urgent surgery was performed. Transmural necrosis was noticed alongside the incision line. The incision is sawn with 4 U-stitches (Prolen 2.0 with Teflon buttressed stitches). Another layer of fixation was made by Prolen 2.0 running stitches reinforced with Teflon felts from both sides. A large PTFE patch was fixed to epicardium over the suture line by Prolen 6.0 running stitch and BioGlue was injected in-between patch and LV (Figures 8 and 9). After aortic cross-clamp removal, the sinus rhythm was restored. CONCLUSION: Despite the high mortality, the urgency and the complexity of surgical treatment the early diagnosis plays a key role in the management of postinfarction LVFWR patients presenting a case of preserved postoperative left ventricular function and accomplished good functional status, as presented in our case.
