MATERIAL AND METHODS: The white male Wistar rats were distributed into 4 groups of 7 animals each and were treated intraperitoneally as follows: Group I (Control): distilled water in a volume of 0.5ml/100g rat/day for10 days; Group II (Ge): gentamicin 80 mg/kbw/day for 7 days; Group III (Ge+Zn): gentamicin 80 mg/kbw/day for 7 days and ZnCl2 5 mg/kbw/day for 10 days prior to administration of Ge and then another 7 days simultaneously with Ge administration; Group IV (Zn): ZnCl2 5 mg/kbw/day for 17 days. Before starting the experiment (I0) and at 3, 7 and 10 days after the first Ge administration, magnesium, copper and zinc plasma concentrations and urinary magnesium levels were determined. RESULTS: Zn administration significantly decreased (p<0.001) plasma Mg concentrations in Ge+Zn group compared to Ge group after 7 days in the experiment, and induced a lower urinary elimination of Mg in Ge+Zn group (p<0.05) than in Ge group (p<0.01). Also, Zn induced a slight augmentation of Cu concentration in Ge+Zn group (p<0.05) compared to Ge group after 7 and 10 days. CONCLUSIONS: The variation in divalent cation concentrations in the context of renal diseases may be helpful for an early diagnosis and effective alternative therapeutic measures.

NO is synthesized by several cell types, where...Nitric oxide (NO), formerly known as the endothelium-derived relaxing factor, is a mediator with a key role in the body, both in the central nervous system and in periphery. NO is synthesized by several cell types, where it acts as an autocrine and paracrine signaling molecule. Harnessing the impressive therapeutic potential of nitric oxide (NO) remains an ongoing challenge. In order to overcome the limitations linked with the use of nitric oxide and specially to increase the release of the radical in the targeted area, promising therapeutic strategies have been implemented, based on specific technologies which create releasing agents and vehicles for nitric oxide. Organic nitrites are the most known NO donor drugs, used especially in the treatment of cardiovascular diseases. In recent years, technological advances have allowed obtaining variations synthetic derivatives (such as diazeniumdiolates, S-nitrosothiols), which can generate NO in a controlled mode in the body and to chemically stabilize it; these compounds were studied with promising results in various animal models of vasospasm and pulmonary hypertension. Another high value therapeutic path is represented by the development of hybrid drugs (new nonsteroidal anti-inflammatory NO donor agents), with practical applications in inflammatory disorders accompanied by pain. Also, there is increasing evidence of the existence of NO donors with important antioxidant and hepatoprotective effects.

MATERIAL AND METHODS: The study consist...AIM: The purpose of this study was to investigate if high values of DMFT/dmft indices constitute risk factors for installation in various types of malocclusions in mixed dentition. MATERIAL AND METHODS: The study consisted of 600 study-cast with mixed dentitions coming from the children in northeastern Romania. WHO (World Health Organization) dental caries criteria were applied to both groups. The existence of an increased caries risk was deducted from the dmft (decay, missed, filled temporary teeth) and DMFT (Decay, Missed, Filled Teeth) indices related to age. Malocclusion was classified in the three planes in accordance with the major symptom. The statistical study was conducted in SPSS 16.0 for Windows. The risk factors we have identified through calculating the odds ratio and relative risk. RESULTS: Mean value dmft/DMFT (index in mixed dentition was found 3.64±2.686. The highest values of the dmft/DMFT indices were found in patients with normal overjet (4.73± 2.892) and normal overbite (4.04±2.781). In the transverse plane, the lowest value of DMFT/dmft index was recorded in patients with spacing (2.63±1.915), and the highest values were observed in patients with the combined diagnosis of crowding and lateral cross bite (4.97±3.010) respectively of crowding and spacing (4.65 ±2.792).The presence of malocclusions in the three plans is not influenced by increased DMFT/dmft index (DMFT/dmft>5) unless combined diagnoses of crowding and lateral cross bite, and crowding and spacing where the relative risks are over 2.5. CONCLUSIONS: The present study concluded that the prevalence of dental caries in children with mixed dentition is higher in patients with normal overjet and overbite. In terms of the risk of developing a certain malocclusion with dental crowding associate with lateral cross bite, respectively dental crowding associate false spacing are present under a high caries activities.

Modern dentistry combines medical treatment with the art of aesthetic restorations. Over the years, the restoration of the facial harmony has changed its character, from random to necessary, and from optional to compulsory, as the aesthetic defects have become less and less accepted. Aim: Contributions to understanding the principles that must be respected in dental-facial aesthetics and their application in various accessible prosthetic techniques that lead to significant improvement of the aesthetic appearance of the final restoration. Material and methods: The study included 22 (aged 40 to 80 years) patients (mean age: 60 years), presenting to the Department of Clinical Dentistry for different types of prosthetic restorations. Results and discussions: Taking into consideration the aesthetic principles and criteria, the prosthetic restorations in two similar clinical situations were performed using a different methodology. Conclusions: To grant the actual interventions the necessary provisions of true clinical success, the dentist must have and develop the skills that allow him/her to be valued as both a scientist, and an artist.

To reduce the bone dimensional changes, localized alveolar ridge augmentation or socket preservation with bone grafting is required. The socket may be filled with synthetic grafts, xenografts, autografts, allografts, autologous platelet concentrate. The use of platelet concentrates for healing improvement and regeneration of the soft and hard tissues in oral cavity is one of the latest achievements in dentistry. One of the newest practices in bone augmentation is the use of platelet rich fibrin, derived from the patient’s own blood and can be very easily accepted by patients due to cost effectiveness, low donor morbidity and low rate of postoperative complications or infection. This biomaterial might be a solution for patients that have strong beliefs regarding the use of allografts or xenografts, or which are afraid of complications during or after autografting procedure. One cannot say that there is an ideal grafting material and must consider a person’s religious affiliation or beliefs when performing bone augmentation.

We present the case of a female patient in “the pediatr...Long-term evolution of HIV because of noncompliance and nonadherence to antiretroviral therapy favors the occurrence of difficult to treat HIV-related malignancies. We present the case of a female patient in “the pediatric cohort” registered at the Iaşi Regional HIV/AIDS Center since year 2000, now with stage C3 AIDS. In 2014, a Burkitt lymphoma was pathologically confirmed, and chemotherapy was initiated as recommended by the hematologist. The clinical course was characterized by multiple complications: hematologic and hepatic toxicities, opportunistic infections and depressive episodes. Highly active antiretroviral therapy associated with sustained psychological support resulted in stabilization of the patient's clinical course (lower HIV viral load and higher CD4 lymphocyte cell counts), anticancer therapy being better tolerated. Currently, patient’s clinical-biological status is quasi-normal. The depressive episodes in this HIV-positive cancer patient undergoing chemotherapy contributed to her non-adherence and non-compliance to treatment, with serious consequences both on clinical and viroimmunological status. Therapeutic strategy in this patient with AIDS and Burkitt lymphoma raised management difficulties as both the drug interactions and cumulative adverse effects had to be considered. Multidisciplinary collaboration and especially psychological intervention are essential for creating a functional team, effective communication being key to achieving long-term adherence to treatment and diagnosis acceptance.

MATERIAL AND METHODS: A number...THE AIM: The aim of the study was to identify the relationship between the level of burnout subdomains and alexithymia among forensic physicians working in forensic institutions in Romania. MATERIAL AND METHODS: A number of 37 forensic physicians were included in the survey. Burnout and alexithymia were measured by using the Maslach Burnout Inventory and Toronto Alexithymia Scale. The obtained data were processed using the SPSS 17.00 statistical software. RESULTS: The subjects obtained an average of 43.27±3.71, which corresponds to a low level of alexithymia. For burnout scores, we have obtained M=14.97±13.13 for emotional exhaustion, M=7.91±6.87 for depersonalization and M=33.18±10.59 for personal accomplishment (low-level for emotional exhaustion and medium-level burnout for the other two factors). Among the socio-demographic variables, only the age correlated positively with the burnout factor personal accomplishment. Positive correlations were identified between burnout factors and TAS-20. Comparative analysis results are important for the presence of insomnia, depression, teaching activity and looking for professional support after critical events. CONCLUSIONS: Scores for forensic physicians are low-level for emotional exhaustion and medium-level for two subdomains and low-level for alexithymia. Factors revealed by the comparative analysis are important to adjust professional activity and to find strategies to cope with stressful professional events.

Excessive near work represents the most frequently cited environmental factor assoc...INTRODUCTION: There are scientific evidences that the etiology of juvenile-onset myopia involves both genetic and environmental factors. Excessive near work represents the most frequently cited environmental factor associated with the development of this disease. OBJECTIVES: The aim of our study was to assess some epidemiological aspects of myopia in today`s medical students from “Grigore T. Popa” University of Medicine and Pharmacy, including the prevalence of this disorder, and the differences between generations. MATERIALS AND METHODS: A questionnaire was shared via the internet on the social groups of the University`s Medical Students. Five hundred and seventy-six (576) students filled the survey online: from the 1st year - 105 students; 2nd year - 86; 3rd year - 121; 4th year - 80; 5th year – 86, and from the 6th year - 98 students. RESULTS: 73.8% of the students included in the study suffer from myopia. At a first glance, the analysis of myopic students’ distribution by the university year showed no significant difference: 1st year – 73.3%; 2nd year – 74.4%; 3rd year – 71.9%; 4th year - 77.5%; 5th year - 68.6%, and 6th year – 77.6% (p<0.005). The percentage of the 6th year students that entered the university with myopia was 43.8%. Furthermore, we observed that the number of students that already had myopia before entering the university has progressively increased. To emphasize how important the difference is, this year 64% of the 1st year students already were diagnosed with myopia. CONCLUSIONS: These results open a debate regarding the causes of the increasing number of students suffering from myopia, including the difficult academic requirements, the behavioral changes, the genetic factors or the poor quality of nutrition.

Objective: To characterize aminoglycoside and macrolide resistance mechanisms in MRSA strains in relation to antibiotic susceptibility patterns. Materials and methods: We tested 86 MRSA strains using multiplex PCR for detection of genes mecA, aac(6′)-Ie/aph(2″), ant(4′)-Ia, aph(3′)-IIIa, ermA, ermC and msrA. Results: There was a prevalence of msrA (32.5%), ermC (30.2%) and aph(3')-IIIa (61.6%) genes, which are less frequently reported in MRSA. Most msrA genes was detected in PVL positive strains (92.8%) and was associated only with non-MDR strains, while ermC genes were associated with MDR strains. PVL producing strains were characterized by the presence of aph(3′)-IIIa (93.1%) and msrA genes (93.1%), being phenotypically susceptible to clindamycin. Conclusions: Detection of aminoglycoside and macrolide resistance genes allowed us to establish the concordance between genotypic and phenotypic methods and to correlate the presence of certain resistance genes with the type of circulating strain and the production of virulence factors.

Candidemia is one of leading bloodstream infections associated with healthcare situated on fourth position and USA with high mortality rates especially for immunosuppressed patients. Demonstrating the level of cleaning using adenosine triphosphate bioluminescence technique we can improve the cleaning and hand hygiene and can influence decreasing of invasive fungal infections transmitted by hands. Material and methods: We have evaluated 200 samples from hands and patient environment on 2 hospitals with pneumology and infectious diseases profile (I and II) using bioluminescence to establish the status of hygiene and to determine measures of cleaning. Results: Regarding hand hygiene the level of relative light units over the values recommended by producer (FAIL) was 66% (6 of 9) in unit I and 75% in unit II (3 from 4). Regarding the level of cleaning in other places in direct contact with patients like toilets, bed bar, commodes the results were FAIL over 60%. Conclusions: In this moment in hospitals the hygiene is evaluated visually and no objective method was implemented beside the sanitation tests in Romania. ATP Bioluminescence evaluates in 15 seconds the level of cleaning in comparison with other methods and generates input in cleaning process, influencing indirectly decreasing hospital acquired infections including invasive fungal infections, together with other measures like education, antibiotic stewardship, increasing patient safety. In hospitals evaluated cleaning of hands and surfaces need improvement and investment in cleaning products and processes monitoring.

AIM:...INTRODUCTION: Pancreaticoduodenectomy (PD) is the procedure of choice in patients with tumors of pancreatic head or periampullary region. Hepatic artery anatomic variants (HAav) are frequently encountered during PD. AIM: To evaluate the incidence of HAav in a series of PDs and their consequence on technical tailoring and outcome. MATERIAL AND METHODS: Forty-five patients with HAav were identified in a consecutive series of 140 PDs for periampullary and pancreatic head tumors performed between January 1, 2007 and December 31, 2015. The groups with or without HAav were compared in terms of operative approach, complications and survival. RESULTS: All patients underwent Whipple procedure, with right posterior approach for those having HAav. HAav was spared without damage in 41 cases. Four patients in whom the HAav were either sacrificed (2) or damaged (2) required arterial reconstruction. CONCLUSIONS: HAav are frequently encountered during PD. Its safeguarding is mandatory but in malignancy it can be damaged or sacrificed, hence arterial reconstruction is required. HAav have no negative impact on surgical morbidity and outcome.

The accuracy of three-dimensional (3D) ultrasonography and hysteroscopy were compa...OBJECTIVE: Hysteroscopy is the most accurate method for diagnosing intrauterine pathologies related to abnormal bleeding and infertility. The accuracy of three-dimensional (3D) ultrasonography and hysteroscopy were compared in identifying uterine cavity abnormalities. MATERIAL AND METHODS: A total of 139 cases of abnormal uterine bleeding or infertility first had two-dimensional and 3D ultrasound performed before hysteroscopy. Findings on 3D ultrasound and hysteroscopy were compared, and sensitivity, specificity, positive predictive value, negative predictive value, and likelihood ratio were calculated. RESULTS: Mean patients’ age was 36.5 (+/- SD 9.04). Three-dimensional ultrasound had a sensitivity of 88%, specificity of 94%, a positive predictive value of 96%, negative predictive value of 84%, likely ratio of 5,5, and accuracy of 90% in diagnosing uterine cavity abnormalities. Three-dimensional ultrasound had a high sensitivity and specificity for polyps (97% and 97%, respectively), congenital uterine malformations (100% and 99%, respectively) and submucous myoma (87% and 100%, respectively), but a low sensitivity and high specificity for uterine synechia (41% and 99%, respectively). CONCLUSIONS: For diagnosing uterine cavity anomalies, 3D ultrasound is a useful tool and could replace diagnostic hysteroscopy for certain conditions, such as congenital uterine anomalies.

Cancer cells are initially eliminated by the immune system, but progressively can develop escape mechanisms. The balance between elimination and escape depends on the prevailing activity of cytotoxic Th1 or immunosuppressive Th2 lymphocytes. Surgery has the potential to eradicate the disease, but, along with other perioperative circumstances, can induce a variable degree and period of immunosuppression. By modifying the Th1/Th2 ratio, these perioperative circumstances may favor the escaping mechanisms and, consequently, promote cancer recurrence and metastasis altering long term prognosis of cancer patients.

However, asymptomatic cases of EC have been reported. The incidence of EC has been estimated to 1.3-3.05 per 1,000 screened postmenopausal asymptomatic women, the most significant ultra-sonographic feature being the thickened endometrium. We reviewed the literature on the accuracy of endometrial thickness measurement by transvaginal sonography (TVS) in predicting EC in asymptomatic postmenopausal women. The use of endometrial thickness as a sole screening test was found not to be of high predictive value and it should be combined with the evaluation of EC risk factors and followed by hysteroscopy and biopsy in suspicious cases. The lack of a universal cut-off value for endometrial thickness that could be correlated to EC is pointed out.

We pres...Antiphospholipid syndrome (APS), one of the most common states of acquired hypercoagulability, is diagnosed by the persistent presence of antiphospholipid antibodies and recurrent episodes of vascular thrombosis. We present the case of a 39-year-old man late-presenting for cardiac rehabilitation treatment after primary percutaneous coronary intervention (PCI) performed for anteroseptal myocardial infarction. He was a nonsmoker, with no prior personal history of other cardiovascular diseases (CVD) or cardiometabolic syndrome. The 60% thrombotic occlusion of the left anterior descending artery (LAD) leading to the acute cardiac event was the only abnormality that was found. The only etiological explanation was the late measurement and the positive tests for antiphospholipid antibodies. In young patients with no history of thrombotic disorder, such as cancer, cardiovascular or metabolic diseases, the unexpected onset of myocardial infarction by thrombotic coronary occlusion can be attributed to silent, undiagnosed autoimmune condition.

We report the case of a 52-year-old woman who developed rhabdomyolysis following an epileptic seizure. Because a single epileptic seizure does not commonly cause rhabdomyolysis, we tried to identify other contributing factors. The only finding was an electrolyte imbalance due to vomiting. This case had a favorable outcome. Our report highlights the importance of considering rhabdomyolysis in a patient with a recent epileptic seizure, especially when several trigger factors for rhabdomyolysis act simultaneously in the same patient.

The study was aimed at analyzing the profile and identifying the frequency, nature, and contributing factors of suicides. MATERIAL AND METHODS: This retrospective study included all patients who attempted suicide admitted to the Emergency Department (ED) of ‘St Spiridon’ Hospital Iasi - Romania, during 2015. They were analyzed by sex, age, residence, suicide method, associated pathology, potential risk factors and interventions. RESULTS AND DISCUSSION: The study included 368 patients (53.81% female and 46.19% male). 63.58% were admitted to ED for drug ingestion, 14.13% for ingestion of organophosphorus compounds, rat poison, herbicide and corrosive substances and 22.28% for traumatic suicide attempts. Drug ingestion (42.11%) was preferred by women while self-inflicted cut injuries (14.94%) and corrosive substance ingestion (8.42%) were more common in males. Benzodiazepines ingestion (20.10%) followed by self-inflicted cut injuries (13.85%) and corrosive substance ingestion (6.52%) were the most frequently used suicide methods. At the time of admission to ED31.52% of patients were on psychiatric treatment and 65.48% were at the first suicide attempt. The admission rate was 57% and mortality rate 0.27%. CONCLUSIONS: Suicidal behavior can be prevented by psychotherapeutic, pharmacological or neuromodulatory treatments for mental disorders. Also, regular follow-up of people who attempted suicide by mental health services is key to preventing future suicide attempts.

Material and methods: Analysis consisted of a kinematic gait study, with kinematic point tracking and software data processing in MATLAB. The analysis was performed on a patient with L5-S1 discectomy. The purpose was to track progress and change in trunk and hip during the rehabilitation program and to establish a relationship between trunk and hip movement during gait, in regards to the patient’s rehabilitation program. Results: Trunk and hip analysis during gait showed significant differences in patient hip and trunk rotational movement between the three analyzed readings of the patients. Changes were found in both left and right hip flexion-extension coupling and flexion in terminal swing. The rotational movement of the trunk is reduced in a more physiological margin from -13.4 to -1.0 anterior-posterior movement which correlates with unimproved in transversal rotation. The analysis made for hip and trunk synchronization revealed an increased synchronization after the end of the functional reeducation program. Conclusions: This method allowed to evaluate changes in trunk and hip motion during gait. Tracking and visualizing the motion between trunk and hip during gait gives a better understanding of the importance of each movement and the strong link between gait phase coordination and motion angle amplitude. It gives a new perspective on the rehabilitation process that targets lumbar stabilization.

AIMS: Informations regarding the prognostic value of right ventricular function changes in the setting of a first acute ST elevation myocardial infarction irrespective of the site of the necrosis and of the left ventricu...AIMS: Informations regarding the prognostic value of right ventricular function changes in the setting of a first acute ST elevation myocardial infarction irrespective of the site of the necrosis and of the left ventricular systolic function are scarce. Purpose of the study was to assess the relation between parameters reflecting global and systolic right ventricular function assessed by conventional, speckle tracking and three-dimensional echocardiography and in hospital major cardiac events (MACE). MATERIALS AND METHODS: We have prospectively analyzed a cohort of 44 consecutive patients (mean age 62,71 years, 70.5 % males) presenting with a first STEMI (2,3 % Topol 1, 38 ,6 % Topol 2, 20,6 % Topol 3, 31,8 % Topol 4, 6,8 % Topol5) treated by primary angioplasty. Patients with previous history of cardiac or pulmonary diseases were excluded. All patients underwent during hospitalization conventional 2D echocardiography and special techniques ( 2D speckle tracking echocardiography and also 3D echocardiography) RV global function was quantified by RV myocardial performance index (RV MPI) determined by PW Doppler ,whereas RV systolic function was studied using regional parameters like TAPSE , pulsed Doppler S wave and RV free wall 2D strain and global parameters like RV fractional area change (RV FAC) or RV ejection fraction ( RVEF) determined by 3D echocardiography . LV systolic function was described by LV ejection fraction (LVEF). The combined endpoint of major adverse cardiovascular events (MACE) was defined by all cause mortality, reinfarction, need for revascularization and occurrence of heart failure during hospitalization. The association between MACE and RV functional parameters was assessed by bivariate correlation analysis followed by binary logistic regression. RESULTS: Initially, regardless of the site of necrosis, the only RV functional parameter correlated with MACE was RV MPI (OR 9.17; 95% CI: 1.03 -83.7). After adjustment for LVEF all RV functional parameters were correlated with MACE: TAPSE (OR: 1.83; 95% CI : 0.41- 8.23), RV MPI (OR: 8.07; 95% CI : 0.9- 72.07), RVFAC (OR: 1.22; 95% CI : 0.25- 5.98) , RV free wall strain (OR : 1.04; 95% CI : 0.21- 5.08) , S wave (OR: 2.46 ; 95% CI : 0.14- 42.82), RVEF (OR: 0.83 ; 95% CI : 0.20- 3.43). CONCLUSIONS: Our study reveals that RV functional parameters are predictive for in hospital MACE beyond LV systolic function and regardless of the culprit coronary artery. Among these parameters, RV MPI seems to have the greatest predictive value for short term MACE in STEMI patients.

MATERIAL AND METHODS: We conducted...AIM: In the present study, we aimed to evaluate serum Mg concentration in children with nephrotic syndrome and highlight the relationship between Mg concentration and acute nephropathy. MATERIAL AND METHODS: We conducted a clinical study in 27 patients, aged 2 to 17 years, admitted to the Nephrology clinic of the Iasi "Sf. Maria" Children′s Hospital between 2011-2015, with the diagnosis of idiopathic nephrotic syndrome (NS), first episode or relapse. In each patient, we investigated serum urea and creatinine, total cholesterol, total serum proteins, serum magnesium, urinary proteins, creatinine clearance and renal histopathology. We also used a control group of 14 children with normal renal function. RESULTS: 12 patients had NS in the acute phase, 6 steroid responsive and 6 steroid resistant. The remaining 15 patients were in the remission phase of NS, 7 steroid responsive and 8 steroid resistant. Significantly decreased serum Mg levels were found in NS active group compared to control group. Renal histopathological analysis in steroid responsive patients revealed the following pathological aspects: optically normal glomeruli (1 case), minimal mesangial changes (5 cases), IgM nephropathy (4 cases), mesangioproliferative glomerulonephritis (4 cases). CONCLUSIONS: The variations of serum and urinary Mg levels in children with acute nephropathy may be useful for early diagnosis and improving therapy.
