A preliminary study revealed that an autopolymerization addition silicone resilient denture relining material (SI) had excellent shock absorption properties similar to those of thermoplastic materials commonly used for mouthguards (ethylene-vinyl acetate: EVA). The aim of the present study was to examine the bonding strength of SI and EVA using a newly-developed adhesive prototype. Delamination tests and tensile strength tests were performed to compare the bonding strengths of SI on EVA prepared under the following four conditions: 1) Control condition (no preparation; C), 2) Sandblasting (S), 3) Bonding with the adhesive prototype (M), and 4) Combination of sandblasting preparation and bonding with the adhesive prototype (SM). The mean bonding strength (S. D.) of the delamination tests under the C, S, M and SM conditions were 0.167 (0.003) N/mm, 0.273 (0.034) N/mm, 0.242 (0.027) N/mm and 0.506 (0.113) N/mm, respectively. The mean bonding strength (S. D.) of the tensile strength tests under the C, S, M and SM conditions were 0.006 (0.011) MPa, 0.081 (0.105) MPa, 0.231 (0.069) MPa and 0.590 (0.041) MPa, respectively. Two-way analysis of variances and Tukey's HSD test detected that the combination of sandblasting preparation and bonding with the adhesive prototype significantly improved the bonding strength between SI and EVA. The results indicate that the self-curing addition silicone resilient denture relining material may adhere to the thermoplastic material prepared by combined application of sandblasting and the adhesive prototype, suggesting the potential of the dental silicone rubber material as a material for repairing mouthguards in clinical practice.

The objective of this study was to clarify the relation between the occlusal force and the tooth displacement path just after unloading, and to estimate the occlusal force during mastication with healthy natural teeth. Three-dimensional occlusal force was calculated from the magnitude of the occlusal force measured by pressure-sensitive film, and from normal vectors of the occlusal contact areas recorded on the study model. The tooth displacement paths were measured using the three-dimensional tooth displacement transducer type M-3. Three male subjects aged 27, 38, and 46 were selected. The subject teeth were their upper left molars. There are two phases of tooth displacement just after unloading: instantaneous recovery, and the subsequent slow recovery. If the maximum tooth displaced position is R, the critical tooth position at the beginning of slow recovery is C, and the original tooth position is O, then re represents the vector RC-->, and rv represents the vector CO-->. The values of re and rv calculated for each unloading tooth displacement path just after clenching the pressure-sensitive film showed different physical properties. The dominant direction of re was to the buccal side, and that of rv was to the coronal side. The coefficient of correlation between the magnitude of occlusal force and re was 0.81-0.87, and that between the magnitude of occlusal force and rv was 0.20-0.63. Based on these physical properties, it was considered that re depends on the recovery of the distortion of alveolar bone, and rv depends on that of periodontal membrane. As described above, the occlusal force during mastication, which is fast movement, can be estimated from re.

The functions of articulation and mastication after glossectomy for tongue carcinoma were sequentially assessed. Longitudinal assessment of articulatory and masticatory functions was analyzed in 64 tongue cancer patients who underwent partial glossectomy (partial glossectomy group: 40 cases) or hemiglossectomy (hemiglossectomy group: 24 cases) in our department between January 2007 and August 2008. The mean age of the 64 patients (37 men, 27 women) was 56.9 years, ranging from 21 to 82 years. Tongue mobility was assessed using a tongue movement function test by Matsunaga's classification. Articulatory function was assessed using the monosyllable Japanese speech intelligibility test, while masticatory function was evaluated using color-changeable chewing gum and occlusal force score using Yamamoto's classification. The functions were evaluated preoperatively, and 1 month, 3 months, 6 months, and 12 months postoperatively. In all groups, both tongue movement and articulatory and masticatory functions were impaired 1 month after surgery, however they gradually recovered between 3 to 6 months and reached a plateau from 6 to 12 months. The values of the parameters in the partial glossectomy group and the hemiglossectomy group at 6 months postoperatively were respectively as follows : the tongue movement function scores were 6.9+/-0.4, 5.2+/-0.8; the speech intelligibility scores were 94.1+/-3.5%, 88.8+/-9.9%; the occlusal force scores by Yamamoto's classification were 5.3+/-0.9, 3.7+/-1.0; and the values of a* determined using the chewing gum were 13.8+/-2.4, 8.6+/-3.5. In addition, there was a correlation between speech intelligibility and the value of a*, and between speech intelligibility and occlusal force scores by Yamamoto's classification.

To assess the efficacy of several caries detective methods (stained with Caries Detector and probing with sharp probe) for caries affected root canal dentin, microtensile bond strengths of resin composite to caries-affected root canal dentin and failure mode distribution were analyzed in this study. Color and hardness were used for assessment of root caries as follows: Dye stain group (pale pink stained with Caries Detector), Probing group (probing with sharp probe) and Sound dentin group (Control). The bond strengths (mean +/- standard deviation) of the Probing group (64.6 +/- 11.9 MPa) and the Sound dentin group (68.7 +/- 11.1 MPa) were significantly higher than those of the Dye stain group (46.9 +/- 7.9 MPa, p<0.05). However, there is no significant difference in fracture mode between the Dye stain group and the Probing group (p<0.05). This could be attributed to that the thick smear layer caused a loss of hybrid layer strength. In conclusion, the caries removal technique of the root canal dentin affected the bond strength of the resin composite.

Three types of retainers (wrought wire clasp, Akers cast clasp and conical crown telescopic retainer) and three outline forms of denture bases designed for distal extension removable partial dentures were assessed in six patients with Kennedy Class II situations. To evaluate the influence of oral function, mandibular displacement, abutment tooth displacement, and denture base exerted pressure were measured simultaneously during maximum voluntary clenching. The mandibular displacement increased when the connecting rigidity decreased or the size of the denture base was reduced. The abutment tooth displacement increased as the denture base was reduced with the Akers clasp. The denture base pressure increased as the denture base reduced with the wrought wire clasp. The conical crown telescopic retainer did not cause overloading to the supporting tissues even when the denture base was reduced. It was also revealed that the abutment tooth tended to displace distally when a distal occlusal rest was prepared on the abutment tooth.

Purpose of this study was to determine the relationship between food mixing ability and oral health-related quality of life (OHR QoL) in partially edentulous patients with removable partial dentures. One hundred and thirty one patients (mean age 67.1 years) participated in this study. The subjects comprised patients who were treated with removable partial dentures (RPDs) in an undergraduate program (n=52), those who were receiving oral maintenance care (n=57) and those who planned to replace current RPDs with new ones (n=22), and OHR QoL was measured using the oral health impact profile-Japanese version (OHIP-J). To evaluate food mixing ability, the subjects were asked to chew a two-colored paraffin wax cube as a test item. Mixing ability index was determined from the color mixture and shape of the chewed cube. Spearman rank correlation coefficient was used to test the relationship between mixing ability index and scores of OHIP-J. No significant correlations were found between mixing ability index and scores of OHIP-J in each group (p>0.08). The results suggest that association between food mixing ability and OHR QoL life is very weak in partially edentulous patients with RPDs.

The unilateral bolus passage through the upper esophageal sphincter (UES) is often observed in swallow evaluations of patients following neck dissection. Head rotation toward the paretic side was reported to be useful to prevent aspiration; however, we often encounter cases of head and neck surgical patients in which the bolus passes through the non-surgical side. Therefore, we investigated UES flow after head and neck surgery to evaluate the laterality to find an effective treatment technique after a head and neck operation. Videofluoroscopic swallowing studies (VFSS) were conducted in 23 oral cancer patients who underwent unilateral neck dissection but not tongue resection. Patients comprised 12 males and 11 females, with a mean age of 62 (SD=14) years. Preoperative VFSS showed that none had a predominant side for UES flow. The laterality of the bolus flow through the UES was predominantly on the side of neck dissection in post-operation. Lateral VFSS showed elevation of the hyoid and the larynx on the non-dissected side was greater compared to the dissected side. Therefore, head rotation toward non-dissected side can be effective for head and neck surgical patients.

A surgical guiding system with simulation software has recently been used for safe implant treatment. We report a clinical case in which we successfully used this system for implant treatment of a congenital tooth missing region with severely restricted mesiodistal space. The patient was a 19-year-old female whose mandibular right deciduous canine was wobbling without a successional permanent tooth and who desired implant treatment. CT images showed that this site was narrow buccal-lingually and only five millimeters in mesiodistal width. The implant position was planned using simulation software (SimPlant : Materialise Dental) based on CT data and a surgical guide template (SurgiGuide : Materialise Dental) was fabricated prior to the implant surgery. During the implant surgery, the deciduous canine was extracted under local anesthesia. The implant bed was prepared with the surgical guide template and an implant was installed. One month after surgery, the position of the implant was examined with a cone-beam CT, which demonstrated that the implant position was exactly the same as planned. At 5 months, a provisional prosthesis was delivered and optimal soft-tissue condition was formed by modifying the provisional prosthesis. Then, the final impression was taken and a ceramo-metal implant-supported crown was delivered. In this case of a congenital tooth missing region with narrow space, implant treatment with simulation software and the surgical guide system provided excellent clinical results.

For better understanding of the structures comprising the human body and in view of the possible need for future revision, Latin anatomical names of skeletal foramina in Terminologia Anatomica were analyzed and classified, and compared with the corresponding Japanese anatomical names. The words following Foramen or Foramina indicated:1) the form of the foramen, 2) the state of the foramen, 3) the absolute size of the foramen, 4) the structure to which the foramen belongs, 5) the position of the foramen in the skeletal system, 6) the part of the human body in which the foramen exists, 7) the structure that transmits the foramen, or 8) the function of the foramen. Analysis of Latin names and comparison with Japanese names clarified some characteristics of both names and revealed some problems in them.

Although there have been several reports about fiber-reinforced composite (FRC) fixed partial denture designs, different framework designs have not been sufficiently assessed. This study analyzed the differences between varying framework designs using FRC materials by two-dimensional photoelastic observation. Seven study models with a three-unit fixed partial denture replacing the first molar were fabricated. One without FRC was used as a control, and the other six were divided in two groups: one with the framework partially covering abutments, and the other with the framework completely covering abutments. For each group, the middle part of the framework was made with three different designs: straight bar, curved bar and curved-thickened bar. A vertical force of 400 N was applied to the occlusal center of each model. The resultant stress was observed photoelastically and recorded photographically. We found that the FRC framework provided strength support to the overall structure, and that the framework completely covering abutments with the curved-thickened bar showed the lowest stress level.

To clarify the extent to which patients feel anxious about dental treatment, anonymized data of a self-completed questionnaire obtained at a dental clinic in Saitama Prefecture were used for this study. The subjects were 278 patients older than 15 years of age and who had given informed consent The questionnaire contained 71 questions concerning the following seven characteristic factors: sense of hearing, sense of sight, sense of smell, sense of taste, sense of touch, sense of pain, and other situations. Each question was rated on a four-point scale. Exploratory factor analysis of the 262 answers was performed with the principal factor method and normalized varimax rotation. Six factors were extracted using the Kaiser Criterion and the Screeplot These factors were interpreted as follows: anxiety before treatment, unpleasantness of pain and bleeding, unpleasantness of smell and taste or feeling of the tongue, a sound or appliance that recalled pain, a meaningful sound such as talking by the dentist, and an unpleasant sound from an appliance. The senses of smell, taste and touch in the oral cavity were extracted as the same factor. The sense of hearing was divided into the factor of dissonance and the factor of anxiety recalled by sound in memory. However, no factor concerning the stimulation of sight was extracted. Therefore, the sense of sight might be related with the recognition method of the patient based on past experience and knowledge.

The objectives of this study were to elucidate the influence of occlusal hypofunction on the histological structure of periodontal ligament (PDL) and on the cell proliferation and alkaline phosphatase (ALP) activities of cultured PDL cells isolated from PDL. Forty-two male Wister rats were divided into three groups: two experimental groups and one untreated control group. Occlusal hypofunction was generated in the molar region in the experimental groups for 3 or 7 days. Paraffin sections of the upper molar region were stained with H.E. or ALP for light-microscope observation. PDL cells isolated from molars of each group were cultured for 7 days for cytological analyses. Cell proliferation (BrdU labeling) and ALP activity of the three groups were compared. Change in the width of the PDL space, loss of functional arrangement of PDL fibers and reduction of PDL cell number were observed among molars of the experimental groups compared with the control group. ALP activity was increased in the molar region where significant enhancement of bone morphogenesis had not yet occurred. The ratio of BrdU positive cells was significantly lower and ALP activity was higher than that of the control group among cultured cells from molars of the experimental groups and the control group. The results clearly showed that occlusal hypofunction alters the histological characteristics of PDL even in a short time. Occlusal hypofunction reduces the cell proliferation and increases the ALP activity of PDL cells. The influence of occlusal hypofunction memory on cells is retained for at least 7 days in vitro.

Hemokinin-1 (HK-1) is a novel peptide described as a member of the tachykinin family. Substance P (SP), a representative member of the tachykinin family, has been well characterized and is thought to play a part in inflammation and pain. While several studies indicate that HK-1 is involved in inflammation and pain, the biological functions of HK-1 are not fully understood. In the present study we investigated the expression of HK-1 mRNA (TAC4) and SP mRNA (TAC1) in the dorsal spinal cord of rat after inducing peripheral inflammation by administering complete Freund's adjuvant (CFA) into the hind paw, using real-time RT-PCR. In the behavioral studies, the thresholds of withdrawal response of the hind paw to thermal stimulation significantly decreased on the ipsilateral side, but not on the contralateral side, 6 hours after CFA injection and thermal hyperalgesia persisted until 4 days after CFA injection. The level of HK-1 mRNA expression significantly increased on the bilateral sides of the dorsal spinal cord 6 hours after CFA injection and returned to the base level 1 day after injection. On the other hand, the level of SP mRNA expression did not change in the spinal cord 6 hours and 1 day after CFA injection. These results indicate that HK-1 may contribute to inflammatory pain, in the early phase, in a different manner from SP.

The aims of this research were to examine the effectiveness of feedback (FB) study of cavity preparation using a virtual reality system (VRS) and to discuss the evidence from an educational standpoint Thirty-nine dental undergraduate students of the fifth grade of Tokyo Medical and Dental University were randomly divided into the FR group and no-FR group (FB group n=21, no-FR group n=18). All subjects of each group performed cavity preparation of class II on the lower left first molar using VRS (DentSim) four times every week (EXO-3). At session EXO, all subjects performed a pre-test to assess their basic skill leve. At sessions EX1 and EX2, the FR group received feedback from the instructor based on a computer-assessment system. The no-FR group practiced their self-judgment without any feedback. At the last session, EX3, the preparation test was administered. All cavity preparations were graded by the VRS. The results showed that the FR group obtained significantly higher scores than the no-FR group, such as total score, outline shape, outline centralization, outline smoothness, wall incline, wall smoothness, proximal clearance, and box width. By session EX2, the FR group tended to spend longer preparation time than the no-FR group, however, at session EX3, there was no difference between both groups. These resdlts confirmed the effectiveness of cavity preparation with feedback study using VRS. It is suggested that this method of learning cavity preparation techniques is suitable for novice undergraduate dental students at the initial stage of cavity preparation practice.

[Observation of pulpal blood flow and bone mineralization after alveolar distraction osteogenesis in a canine cleft model].

OBJECTIVE: Distraction osteogenesis has been widely used for the craniofacial region and even applied to alveolar bones to reduce the cleft width of cleft lip and/or palate patients. However, there have been no reports on the limitations on the total amount of lengthening that can be accomplished by distraction osteogenesis in this area. Therefore, we investigated how different extents of distraction osteogenesis affect the healing process using a canine cleft palate model. METHODS: Six beagle dogs were used in this study. A bony segment including the canine was prepared and translocated into a 10-mm bone defect area, imitating a cleft, at a rate of 1 mm/day for six or ten days, resulting in two groups (6- and 10-mm groups). Canine pulpal blood flow was monitored for 100 days by Doppler flowmetry. The animals were then sacrificed and the regenerated bone area was evaluated using peripheral quantitative computed tomography (pQCT) and a histological process. Statistical significance was confirmed by the Mann-Whitney rank sum test. RESULTS: Pulpal blood flow recovered to the original levels significantly earlier in the 6-mm group. It was shown by pQCT evaluation that cortical bone density in the regenerated bone was significantly higher in the 6-mm group. The amount of regenerated bone in histological sections was also significantly higher in the 6-mm group than in the 10-mm group. CONCLUSION: We revealed that the amount of bone lengthening affects the progress of healing. These results suggest the importance of limited distraction osteogenesis in the maxillofacial area.

Photodynamic diagnosis (PDD) is a form of cancer detection based on the administration of an exogenous photoactivated compound that accumulates in malignant cells, followed by appropriate photo-irradiation. 5-aminolevulinic acid (5-ALA), which is one of these compounds, is a prodrug of the photosensitizer protoporphyrin IX (PpIX). This report describes a spectroscopic study of 5-ALA-generated PpIX fluorescence in human oral cancer cell lines. After transplanting five human oral squamous carcinoma cell lines into scid mice, the degree of tumor differentiation was evaluated by hematoxylin-and-eosin staining. Two, one, and two cell lines were poorly, moderately, and well differentiated, respectively. Under cell culture conditions, well and moderately differentiated cancer cells showed higher positive staining rates of AE1/AE3 than poorly differentiated ones in immunohistochemistry. 5-ALA-induced PpIX fluorescence intensities of five cell lines, which were cultured in Dulbecco's Modified Eagle's Medium (DMEM) treated with or without deferoxamine mesylate (DFO), were measured. In all cell lines, the maximum 5-ALA-generated fluorescence intensity was found in the presence of DFO. The fluorescence intensity varied with the degree of tumor differentiation in the following order: poorly differentiated, moderately differentiated, well differentiated. These results suggest that the fluorescence intensity of oral cancer cells differs according to the degree of tumor differentiation. However, even with well differentiated cancer cells, addition of DFO could increase the photodynamic sensitivity of 5-ALA as a result of greater cellular accumulation of PpIX.

This study analyzed the educative efficiency of the class on nosocomial infection control (NIC) by comparing achievements pre- and post-test, and discussed appropriate education planning on the NIC for dental students. Sixty-two fifth-grade students in the 2007 academic year sat the pre- and post-tests; the mean score and standard deviation of these tests were 5.30 +/- 1.26 (n = 56) and 8.59 +/- 1.18 (n = 59), respectively. There was a statistically significant difference between them (paired t-test, p < 0.01). Another finding was that students with high scores in the post-test did not necessarily achieve high ratings in the pre-test. It is suggested that the introduction of pre- and post-tests and the clarification of main points in the class as a theme of NIC could be a useful tool for increasing the comprehension of students on the theme. Since students at lower grades will attend clinical practice in the university hospital, it is thought that students should be given NIC training early in the clinical course, and the current curriculum should be improved to increase the opportunity for students to study this important issue.

This study examines the psychological states and QOL in oral cancer patients during the perioperative and postoperative survival periods. Those patients who were scheduled for and had undergone primary surgery at the Oral and Maxillofacial Surgery Section, Tokyo Medical and Dental University Hospital, Tokyo, Japan were selected for this study. They took two different kinds of tests, i. e. the Hospital Anxiety and Depression Scale (HADS) and the Japanese-language version of the Functional Assessment of Cancer Therapy Head and Neck (FACT-H & N) version 4. In a longitudinal study, interval assessments were done at one day before surgery, one week after surgery and one month and six months each after being discharged from the hospital, respectively. In a cross-sectional study, outpatients during the postoperative follow-up periods were evaluated. Statistically, before surgery psychological anxiety became highest, while after surgery depression grew and QOL in the somatic and specific domains decreased. After discharge from the hospital, the patients with longer-term postoperative survival had better psychological states and QOL. These results provide important information regarding psychological states and QOL in oral cancer patients for caring and supporting based on perioperative and postoperative periods.
