OBJECTIVE: Aim: The research aims to confirm or reject the hypothesis on the possible relationship between the influence of changes in pancreatic vascular variations on the mechanisms of occurrence of pancreatic diseases based on the collected scientific literature.. PATIENTS AND METHODS: Materials and Methods: The bibliosemantic, bibliographic, statistical research methods were used in the research. Scientometric databases Web of Science, Scopus, PubMed, and archives of scientific papers such as Google Scholar and Research Gate for the last 6 years (2018-2023) were used to form a study basis. CONCLUSION: Conclusions: Based on the analyzed sources, blood supply variants to the pancreas do not influence the development of pathological processes. However, vein and artery features impact treatment choices and surgical outcomes. Main and additional pancreatic ducts, and the common bile duct, may be linked to diseases. Atypical pancreatic artery branching affects surgical tactics and increases bleeding risk.

OBJECTIVE: Aim: To conduct a forensic medical and legal analysis of cases of improper performance of professional duties by medical employees, including incorrect prescription of medical products.. PATIENTS AND METHODS: Materials and Methods: The article is based on the analysis of defects in the performance of professional duties by medical employees, particularly according to data from commission forensic medical examinations, as well as an analysis of international and national legal acts and national judicial practices of Ukraine. Dialectical, hermeneutic, comparative, analytical, synthetic, and system analysis research methods were used. RESULTS: Results: Although the total number of forensic medical examinations tends to decrease, these examinations remain the focus of attention in cases of improper performance of professional duties by medical employees. A more detailed analysis of cases involving violations of treatment strategies revealed instances of incorrect treatment method selection (whether conservative or surgical), along with cases of improper or unjustified prescription of medicinal products. Incorrect prescription of medical products is one significant way medical employees fail in their professional duties. CONCLUSION: Conclusions: Understanding the general dynamics of forensic medical examinations is important, as it helps establish practical cause-and-effect relationships between the performance of professional duties by medical employees and adverse consequences for patients (including cases involving incorrect prescription of medical products).

OBJECTIVE: Aim: To investigate the role of thymidine phosphate and IL-6 in the pathogenesis and survival rate in women with breast cancer.. PATIENTS AND METHODS: Materials and Methods: Sixty women diagnosed with breast cancer (with age ranging between 25-65 years) were included in the current study. Of these, 40 women relapse after 6 months of follow up, while 40 patients were non-relapsed. RESULTS: Results: Statistical analysis pointed out that thymidine phosphorylase may be significantly increased in relapsed women comparing to non-relapsed women (4.48±0.24 ng/ml and 1.12±0.18 ng/ml respectively, p value <0.0001). Regarding IL-6, the current study also found that IL-6 tends to be increased in relapse BC comparing to non-relapsed BC (8.6±0.92 pg/ml vs. 6.82±1.14 pg/ml respectively, p-value<0.0001. There was a high significant positive correlation between thymidine phosphorylase and IL-6 (r=0.368; p-value <0.01). The sensitivity and specificity in predicting relapse in breast cancer were 0.83 and 0.64 for TP and 0.78, and 0.65 respectively. CONCLUSION: Conclusions: It is suggested that thymidine phosphate activity and IL-6 serum levels after six months of follow up, have a dual synergistic impact on the pathogenesis of relapse for BC. These biomarkers can also be used in the prediction of relapse rate in women diagnosed with BC.

OBJECTIVE: Aim: To investigate the impact of the improved Working Program of the academic subject "Special Physical Training" on the dynamics of indicators of cadets' physical and psycho-emotional state in their training... OBJECTIVE: Aim: To investigate the impact of the improved Working Program of the academic subject "Special Physical Training" on the dynamics of indicators of cadets' physical and psycho-emotional state in their training process.. PATIENTS AND METHODS: Materials and Methods: The research, which was conducted in 2023-2024, involved 167 male cadets of the 3rd and 4th training years of National Academy of Internal Affairs (Kyiv, Ukraine). Two groups were formed: the experimental group (EG, n = 86), whose cadets studied according to the improved Working Program of the academic subject area referred to as "Special Physical Training," and the control group (CG, n = 81), whose cadets studied according to the existing Working Program. Research methods: bibliosemantic, pedagogical experiment, testing, methods of mathematical statistics. RESULTS: Results: The research found that at the end of the experiment, most of the studied indicators of physical and psycho-emotional state in the EG cadets were significantly better than in the CG cadets. The most pronounced effect of the training sessions with the improved Working Program was found in the indicators of anxiety, neuro-emotional stability, self-confidence, stress resistance, and endurance of future law enforcement officers. CONCLUSION: Conclusions: A high level of these indicators will ensure the formation of law enforcement officers' psychophysical readiness for extreme conditions. It will help to improve the efficiency of their performance of service tasks under martial law.

The influence of student classes in psychiatry on the perception of people with intellectual disabilities.

OBJECTIVE: Aim: To gain knowledge about the attitudes of medical students towards people with intellectual disabilities and the impact of psychiatry teaching on changing these attitudes.. PATIENTS AND METHODS: Materials and Methods: The study involved 106 students of medical faculties who had not yet taken a course in psychiatry and 104 who had completed the course and passed the exam. The interviewers who conducted the research were the students themselves. The tool used in the study was a questionnaire of own design designed by a special educator and a psychiatrist. The questionnaire included questions about age and gender, as well as 5 statements of a discriminatory (rejecting) nature and 5 affirmative (accepting) statements indicating full acceptance of people with intellectual disabilities. RESULTS: Results: Most of the people surveyed presented positive attitudes towards people with intellectual disabilities (approx. 88%). Approximately 12% of the respondents presented negative attitudes reflecting social stereotypes. The groups of students before and after completion of the classes in this subject did not differ in the frequency of the 10 selected attitudes studied. CONCLUSION: Conclusions: The vast majority of medical students surveyed present positive attitudes (judgments) regarding people with intellectual disability. Approximately 12% of the surveyed medical students present negative attitudes (judgments) towards persons with intellectual disability. There is no correlation between the completion of psychiatry classes and the frequency of negative attitudes (judgments) towards persons with intellectual disability among the surveyed medical students.

OBJECTIVE: Aim: The purpose of this study was to evaluate sub-species typing by matrix-activated laser desorption/ionisation mass spectrometry on the example of Klebsiella pneumoniae isolated from patients with pulmonary tuberculosis in Karaganda region.. PATIENTS AND METHODS: Materials and Methods: This study evaluated the potential of MALDI-TOF MS for epidemiological identification of Klebsiella pneumoniae isolated from pulmonary tuberculosis patients in Karaganda region. A total of 47 strains of Klebsiella pneumoniae obtained between 2015 and 2019 were typed. Mass spectra were taken using Microflex LT MALDI-TOF (Bruker) according to standard procedure. RESULTS: Results: As a result, MALDI-TOF evaluation of subspecies typing on the example of Klebsiella pneumoniae isolated from patients with pulmonary tuberculosis in Karaganda region, predominantly showed the diversity of isolates, indicating their out-of-hospital nature. Based on all available information, including facts, sources, and experimental results, it can be reasonably concluded that the current study of Klebsiella pneumoniae does not provide a complete picture of the epidemiology of this microorganism. CONCLUSION: Conclusions: The current study provided a limited opportunity to investigate the genetic structure and diversity of Klebsiella pneumoniae subspecies more accurately. The findings suggest that the time-of-flight laser-ionisation ablation mass spectrometry method with magnetic focusing is more likely to detect differences in external characteristics between individual specimens of the same species than internal genetic variation in the microorganisms under study. The practical relevance of this study is to inform the development of more effective infection control strategies in clinical practice and infection control.

OBJECTIVE: Aim: This study explores the feasibility and psychosocial outcomes of using somatic methods as a mental and psychosocial health intervention in the context of active violent conflict.. PATIENTS AND METHODS: Materials and Methods: Project leaders taught an 8-day program in somatic methods to local mental health care providers in the Donbas region of Ukraine. In the following seven months, 20 mental health care providers applied the somatic methods in their work with clients. The progress of clients participating in the study was tracked utilizing a software designed for the project. Longitudinal data was collected from 460 clients between December 2020 and May 2021 and was analyzed using mixed linear models to estimate changes in outcomes of depression, functionality, anxiety and alcohol problems. RESULTS: Results: The program was successfully implemented despite the challenges posed by the ongoing conflict. Demographic data on participants and survey results were successfully collected and analyzed. Data demonstrated significant declines over time for depression and anxiety as well as a significant increase in daily functioning. CONCLUSION: Conclusions: The results of this pilot study support the feasibility of developing a subsequent randomized controlled trial study to further assess the impact of somatic methods on psycho-social wellness in Ukraine.

OBJECTIVE: Aim: The aim of the present study is to analyse and summarize the experience of the Department of Reconstructive and Plastic Microsurgery of the National Scientific Center of Surgery named after A.N. Syzganov in the field of treatment of hand injuries.. PATIENTS AND METHODS: Materials and Methods: The study sample included 295 patients who applied to the Department of Reconstructive and Plastic Microsurgery of the National Scientific Center of Surgery named after A.N. Syzganov from the beginning of 2021 to the end of 2022, in relation to which general clinical methods of research, including the method of dynamic observation and innovative microsurgical techniques, were applied. Innovative minimally invasive surgical techniques were developed and successfully applied to the flexor tendons of the fingers, ensuring precise juxtaposition of tendon ends, reducing tissue trauma, and facilitating smooth passage of the repaired tendons in bone-fibrous canals. RESULTS: Results: The application of the developed methods demonstrated high clinical efficacy, as evidenced by the high percentage of good and satisfactory treatment results - 98.8% in one group and 93.7% in the other. Nevertheless, in 3.4% of cases, the development of flexion contractures of fingers of different severity was observed, which was associated with insufficient intensity of rehabilitation, lack of proper dynamic control and indiscipline of some patients. CONCLUSION: Conclusions: The result of the work was a comprehensive analysis of the experience of microsurgery of the hand obtained in the Republic of Kazakhstan during the study period.

OBJECTIVE: Aim: This study aimed to evaluate the serum levels of key biomarkers, including parathyroid hormone (PTH), vitamin D3, calcium, phosphate, and renal function tests (RFTS), in patients with chronic renal failure over three years.. PATIENTS AND METHODS: Materials and Methods: A cohort of 83 patients with chronic renal failure and 70 healthy controls were followed, with data collected at one, two, and three years. RESULTS: Results: Significant differences were observed in the levels of creatinine, urea, phosphate, calcium, vitamin D, and parathyroid hormone between patients and controls. the study found a progressive increase in phosphate and parathyroid hormone levels, alongside a decrease in calcium and vitamin D levels in patients over time. strong correlations were observed between these biomarkers, highlighting the complex interplay between mineral metabolism and renal function in chronic renal failure. CONCLUSION: Conclusions: These findings confirm the need for integrated management strategies to address these biochemical disturbances and improve the life quality of patients with chronic renal failure.

OBJECTIVE: . Aim: To evaluate the effectiveness of a multilevel rehabilitation program integrating physiotherapy (PT), transcranial electrical stimulation (TES), and endonasal breathing of a singlet-oxygen mixture (EBSO)... Aim: To evaluate the effectiveness of a multilevel rehabilitation program integrating physiotherapy (PT), transcranial electrical stimulation (TES), and endonasal breathing of a singlet-oxygen mixture (EBSO) on pain syndromes (PS), quality of life (QL), and levels of depression in professionals with type 2 diabetes mellitus (T2DM) and high psychophysical stress. PATIENTS AND METHODS: Materials and Methods: Sixty professionals with high psychophysical stress were randomly divided into three groups (n=20 each). Group 1 received standard rehabilitation. Group 2 underwent targeted PT, including ultrasound therapy for the pancreas projection and magneto-laser therapy for the liver projection and Th5-Th7 zones. Group 3 received the same PT as Group 2, supplemented with TES (alpha rhythm frequency) and EBSO. The rehabilitation program lasted two weeks, comprising 10 sessions. Outcomes were assessed using the QL questionnaire - SF-36 and Beck Depression Inventory-II (BDI-II). RESULTS: Results: Group 3 demonstrated significant improvements in QL by SF-36 score, particularly in Physical Functioning (from 49,2 ± 2,6 to 70,3 ± 3,8), Bodily Pain (from 48,1 ± 2,6 to 69,3 ± 3,1), and Mental Health (from 38,0 ± 3,3 to 59,3 ± 3,6; p < 0,01). Depression score decreased from 30,6 ± 2,1 to 18,2 ± 2,0 (p < 0,01), with notable improvements in cognitive-affective and somatic subscales. Group 2 showed moderate improvements, while Group 1 exhibited minimal changes. CONCLUSION: Conclusions: Combining targeted PT with TES and EBSO significantly improves somatic and psychological outcomes in T2DM patients compared to standard rehabilitation or PT alone.

OBJECTIVE: . P... OBJECTIVE: . Aim: The aim of this ambispective, two-center, observational study was to evaluate the clinical characteristics and 3-years prognosis of patients with non-STelevation myocardial infarction (NSTEMI) and AF. PATIENTS AND METHODS: Materials and methods: Patients hospitalized with a diagnosis of NSTEMI were included. Baseline demographic and clinical features, in-hospital procedures, and occurrence of cardiovascular events in patients with sinus rhythm (SR) and AF confirmed before admission were analyzed. RESULTS: Results: A total of 273 patients with NSTEMI were enrolled between 2012 and 2015, of whom 102 had AF (37.3%, 60.8% men) and 171 had SR (64.3% men). Patients with AF and NSTEMI were significantly older: 76.6±8.4 vs. 65.9±11.1 years (p<0.0001). The permanent pattern of AF was observed in 54 patients (52.9%) and paroxysmal in 48 patients (47.1%). They more often had heart failure with higher NYHA class (p<0.0001), type 2 diabetes (p=0.0001), abnormal renal function (p<0.0001), and history of stroke (p<0.0001). The presence of AF was not correlated with the number of stenotic coronary vessels. Significantly more NSTEMI patients with AF had a pacemaker implanted before admission (p<0.0001). Duration of hospital stay of NSTEMI AF patients was longer: 10.5±9.9 vs.6.6±4.2 days and it was related to age, Rho=0.5,p=0.0002. During the 2-year and 3-year follow-ups, AF was significantly related to higher mortality, both cardiovascular disease and all-cause mortality (p<0.0001). Independent long-term all-cause mortality predictors were heart failure, liver insufficiency, and age. At the 3-years follow-up, there were no significant differences in all-cause and cardiovascular mortality between groups with permanent AF and paroxysmal AF (p=0.696). CONCLUSION: Conclusions: Patients with atrial fibrillation and NSTEMI were older and more likely to have comorbidities. During the 2-year and 3-year follow-ups, AF was related significantly to higher mortality, both cardiovascular and of all causes. The long-term prognosis of patients with AF was adversely affected by heart failure, liver insufficiency and age.

OBJECTIVE: Aim: To investigate the effectiveness of a therapeutic and prophylactic complex based on mineral metabolism markers in a peri-implantitis model under calcium deficiency conditions.. PATIENTS AND METHODS: Materials and Methods: A peri-implantitis under calcium deficiency was modeled in experimental animals. The study evaluated the effects of a therapeutic and prophylactic complex, consisting of the oral gel "Perioimplantgel" and the dietary supplement "Minerol," on markers of mineral metabolism in the blood and periodontal tissues of rats. RESULTS: Results: Modeling peri-implantitis in experimental animals with calcium deficiency caused a significant decrease in magnesium, calcium, and phosphorus levels in the blood and periodontal tissues, accompanied by increased osteoresorption and alkaline phosphatase activation. The oral gel "Perioimplantgel" significantly reduced enzyme activity, while the combination with the dietary supplement "Minerol" stabilized all mineral metabolism markers, returning tchem to levels comparable to intact animals. These effects were more pronounced with prolonged use of the complex before and after implantation, suggesting its high osteoregenerative properties and the need for extended use. CONCLUSION: Conclusions: The applied experimental model of peri-implantitis in the conditions of calcium deficiency allowed for revealing the positive effect of the oral gel "Perioimplantgel" on the state of periodontal tissues during the installation of dental implants, as well as determining the indicators of mineral metabolism for its greatest effectiveness when used in combination with the dietary supplement "Minerol".

OBJECTIVE: Aim: The aim of this study was to investigate and correlate psychosocial risks and musculoskeletal disorders (MSDs) faced by nurses in the hospital work environment.. PATIENTS AND METHODS: Materials and Methods: A cross-sectional study was conducted among 90 nurses (response rate: 56%) working in a General Hospital of Central Greece, from January to March 2023. A self-administered questionnaire was used for data collection, which included demographic information, characteristics of the nursing unit, the Copenhagen Psychosocial Questionnaire Version III (COPSOQ III), and the Nordic Musculoskeletal Questionnaire. RESULTS: Results: No statistically significant differences were found between individual factors and the COPSOQ scales under examination, except for gender. Professional characteristics, however, were associated with psychosocial risks. For example, an increase in the number of nurses was positively associated with work demands. Conversely, night shifts were negatively associated with freedom of movement, as was the length of employment with opportunities for career development and the assessment of leadership quality. No statistically significant differences were found between MSDs and individual factors. Regarding the correlation between psychosocial risks and MSDs, the study revealed several associations among COPSOQ scales, such as work demands, work-life balance, freedom of movement, social support from colleagues and supervisors, job insecurity and satisfaction, interactions with others, health assessment, and mental exhaustion, with the occurrence of MSDs in body areas such as the neck, shoulders, elbows, wrists, hips, upper back, and ankles. CONCLUSION: Conclusions: The hospital work environment entails numerous psychosocial risk factors, and ensuring its safety requires their identification and evaluation. Interventions such as ergonomics training, acquiring ergonomic equipment, avoiding manual lifting, and training on the use of patient handling devices can increase risk awareness, reducing the frequency and intensity of musculoskeletal pain.

OBJECTIVE: Aim: The purpose was to determine the morphological features of collagen fibers of the dermis and regenerate filling the cavity of the experimental skin wound during its closure by different methods.. PATIENTS AND METHODS: Materials and Methods: The experimental study was conducted on 60 rats of the WAG population weighing 250-300 grams. Five groups were formed (12 rats in each group). Rats of groups 1-4 underwent a 1.5 cm long skin incision on the lateral surface of the neck. The formed defect in rats of group 1 was sutured with an interrupted suture, in group 2 it was sutured with an intradermal suture, in group 3 it was closed with skin glue based on 2-octyl cyanoacrylate, in group 4 it was welded with an electrocoagulator PATONMED EKVZ-300 (Ukraine). Group 5 included intact rats that did not undergo any manipulations. On the 7th and 14th day, 6 animals were removed from the experiment in groups 1-4. In group 5, all animals were removed from the experiment on the 7th day. The material for morphological study was a skin sample from the lateral surface of the neck. Histological, histochemical, morphometric and statistical research methods were used. RESULTS: Results: In survey microscopy, collagen fibers located in the regenerate and surrounding dermis, in cases of experimental wound closure with nodal or intradermal sutures, had different directions of location, mostly looked thickened, and were interconnected, which led to the disappearance of intercellular spaces. In cases of wound closure with 2-octyl cyanoacrylate-based skin glue or by welding with the PATONMED EKVZ-300 electrocoagulator, collagen fibers looked mostly thinned, chaotically arranged in a dense intertwining network. The density of collagen fibers in the regenerate and the surrounding dermis did not differ depending on the different methods of wound closure. The collagen content in the collagen fibers of the regenerate increased on day 14 compared to day 7 for all methods of experimental wound closure. In cases of wound closure using a nodal or intradermal suture, the collagen content was higher compared to cases when the wound was closed with 2-octyl cyanoacrylate-based skin glue or by welding with the PATONMED EKVZ-300 electrocoagulator. In the collagen fibers of the dermis surrounding the wound, the collagen content was higher in cases of wound closure with a nodal or intradermal suture. In cases of wound closure using 2-octyl cyanoacrylate-based skin glue or by welding with the PATONMED EKVZ-300 electrocoagulator, the collagen content in the dermis surrounding the regenerated tissue corresponded to the control value. CONCLUSION: Conclusions: Closing the surgical wound with a nodular or intradermal suture is likely to lead to the formation of skin scars in the future. In cases of wound closure using 2-octyl cyanoacrylate-based skin glue or by welding with the PATONMED EKVZ-300 electrocoagulator, all conditions for the manifestation of organotypic skin regeneration are formed.

OBJECTIVE: Aim: To assess the level of occupational safety, health and hygiene of healthcare workers in the terms of the COVID-19 pandemic.. PATIENTS AND METHODS: Materials and Methods: bibliosemantic, bibliographic, analytical, statistical methods as well as a systemic approach have been used in the research. The data of the Cabinet of Ministers of Ukraine, the Ministry of Health of Ukraine, the State Labour Service and the scientific research findings from open sources have been analyzed. CONCLUSION: Conclusions: The safety of healthcare workers with regard to infection is essential for a secure hospital environment. From an infection prevention perspective, PPE and sanitary requirements are fundamental to ensuring a safe environment for healthcare workers. The analysis revealed inadequate provision of PPE for healthcare workers during the pandemic, with levels ranging from 61,25% to 63,63%. The risk of contracting the novel coronavirus among healthcare personel during the pandemic was 12 times higher than in the general population. The risk of developing an occupational disease among those who treated patients infected with SARS-CoV-2 was 58,1 times higher. The analysis revealed that the provision of personal protective equipment (PPE) to healthcare workers during the study period was only two-thirds of the required amount, which consequently elevated the risk and incidence of morbidity among healthcare workers.

OBJECTIVE: Aims: This review aims to synthesize the latest literature on physical treatments for wounds, focusing on the use of ozone therapy, topical oxygen therapy, pulsed electromagnetic field therapy, and red light therapy in oncology patients. It evaluates the indications, benefits, and contraindications of these therapies, especially concerning cancer.. PATIENTS AND METHODS: Materials and Methods: A literature search was conducted in publicly available online databases, covering publications in English and Polish from 2010 to 2024. The inclusion criteria comprised clinical trials, systematic reviews, meta-analyses, and review articles on physical treatments for wounds in oncology patients. The selection process involved pre-selection, title and abstract review, and full-text review to ensure compliance with the inclusion criteria. Data were analyzed to identify mechanisms of action, therapeutic efficacy, and potential risks associated with these therapies in oncology patients. Ozone therapy showed potential in reducing bacterial load and tumor hypoxia, enhancing chemotherapy efficacy. Topical oxygen therapy was effective for chronic wounds, with careful application near tumor sites. Pulsed electromagnetic field therapy demonstrated promising anticancer effects, inducing apoptosis in cancer cells. Red light therapy, while beneficial for managing side effects of cancer treatments, was contraindicated in areas with active tumors due to the risk of stimulating cancer cell proliferation. CONCLUSION: Conclusions: Physical therapies offer benefits in wound management but require careful consideration in oncology patients. Personalized evaluation and further research are essential to establish safe and effective protocols for oncology patients, maximizing therapeutic benefits while minimizing risks.

It may seem reasonable because some of them kill instantly. But others can kill too, simply silently and in a prolonged period. Acute kidney injury may seem to be a dull subject. Especially, when it does not lead to instantaneous complete loss of function. A disease that is a little bit intangible, with no pill that can directly treat it. Nevertheless, deterioration of kidney function may have a tremendous impact on patients' life. Even when it does not lead to renal replacement therapy. That is why we should be interested in recognising it and giving our patients as much as we can to neutralise its consequences. Do you know enough about this complication? When to expect creatinine to start rising? Diuresis - do you know how much is not much enough? Let us dive together into this trite, yet important subject.

OBJECTIVE: Aim: This paper discusses the development of postural-motor, coordination, and reflex functions in children during their first year of life. It proceeds in an orderly manner in stages in which the child actively acquires new psychomotor skills (developmental milestones). These milestones are permanent and have a lifelong impact on health and fitness.. PATIENTS AND METHODS: Materials and Methods: An analysis of data from literary sources on child development in the first year of life was carried out. The review was based on the PubMed online database using keywords related to child development in the first year of life, childcare principles, and infant rehabilitation. The discussion follows an orderly progression through stages where children actively acquire new psychomotor skills, known as developmental milestones. Attention has been paid to the balance organ, which plays a significant role in the development of brain coordination and the disappearance of primitive reflexes. The analysis revealed that the development of postural-motor, coordination, and reflex functions follows a predictable pattern. Key developmental milestones include the gradual disappearance of primitive reflexes and the acquisition of new motor skills. The balance organ plays a crucial role in brain coordination and motor development. CONCLUSION: Conclusions: Monitoring child development in the first year of life by specialists and guidance for caregivers is important to ensure optimal life-long physical and mental development. The dexterity and physique of an adult result from the harmonious course of development during the first year of life.
